Development and Validation of a Bedside Dengue Severity Score for Predicting Severe Dengue in Children.

OBJECTIVE: To develop and validate a bedside dengue severity score in children less than 12 years for predicting severe dengue disease. METHODS: We carried out an analysis of data on the clinical and laboratory parameters of patients with confirmed dengue, hospitalized in October, 2019 at our center. A comprehensive patient's score was developed. Predictive models for severity were built using a forward step-wise method. This model was validated on the data of 312 children with dengue admitted during September- October, 2021. RESULTS: Severe dengue was predicted by the dengue severity score …

[Characteristics of imported cases of Dengue fever and hemorrhagic Dengue fever in 2009-2019].

INTRODUCTION: In Russia, the approved morbidity statistics system is represented by the International Classification of Diseases of the 10th revision (ICD-10). This classification provides two forms of dengue fever (DF): dengue fever (A90) and hemorrhagic dengue (A91). Official statistics on the ratio of forms of DF is not published in open sources and this lack of information about the real ratio of the forms of DF makes it difficult to objectively assess the factors that determine the severity of this disease. THE AIM: compare the clinical and epidemiological features of dengue fever and he…

Characterization of the 2013 dengue epidemic in Myanmar with dengue virus 1 as the dominant serotype.

In 2013 in Myanmar, dengue epidemic occurred with 20,255 cases including 84 deaths. This study aimed to determine the serological and molecular characteristics of dengue virus (DENV) infection among children with clinical diagnosis of dengue hemorrhagic fever (DHF) or dengue shock syndrome (DSS) during this period. Single acute serum samples were collected from 300 children in Mandalay Children Hospital, Mandalay, Myanmar. Out of the 300 children, 175 (58.3%) and 183 (61%) were positive for anti-dengue IgM and anti-dengue IgG, respectively. Among the IgM positives, 41 (23.4%) had primary DENV …

A comparison of the pattern of liver involvement in dengue hemorrhagic fever with classic dengue fever.

The impact of dengue on liver function was studied on fifty serologically confirmed dengue cases admitted to Hospital Universiti Kebangsaan Malaysia (HUKM). Twenty-five of these patients had classic dengue fever (DF) and 25 had grade 1 or 2 dengue hemorrhagic fever (DHF). There were more (60%) DHF patients with hepatomegaly compared to DF (40%) but the difference was not statistically significant. Analysis of the liver profile showed that liver dysfunction was commoner in DHF compared to DF, indicating that the degree of liver impairment may be related to the severity of DHF. Hyperbilirubinemi…

The global pandemic of dengue/dengue haemorrhagic fever: current status and prospects for the future.

Dengue/dengue haemorrhagic fever has been one of the most important resurgent tropical diseases in the past 17 years, with expanding geographic distribution of both the viruses and the mosquito vectors, increased frequency of epidemics, the development of hyperendemicity (co-circulation of multiple virus serotypes) and the emergence of dengue haemorrhagic fever in new areas. This paper briefly reviews the changing epidemiology of dengue, discusses some of the factors responsible for the recent resurgence, and reviews the current options for reversing the trend of emergent disease.

An epidemic of dengue haemorrhagic fever and dengue shock syndrome in Delhi: a clinical study.

Twenty-four cases of dengue haemorrhagic fever/dengue shock syndrome were studied in Delhi in the months of September and October, 1988. The majority of these cases were boys aged 6-10 years. Classical symptoms of dengue (fever, headache, aesthesia, myalgia) occurred in all the patients. Digestive symptoms (nausea, vomiting, anorexia, abdominal pain and hepatomegaly) were also common. Haemorrhagic manifestations were present in 41.7% of the cases. Of these, 90% had gastrointestinal haemorrhages. Shock occurred in 17 cases (70.8%). Thrombocytopenia and prolongation of coagulation profile were f…

Evaluating Three Rapid Tests for Dengue in Febrile Patients in a Dengue-Endemic Area.

OBJECTIVE: Rapid diagnostic tests have been used mainly when it is not possible to carry out the recommended laboratory tests due to challenges derived from the infrastructure of the state surveillance system and, in the specific case of dengue, due to catastrophic climatic events that prevent the transport of samples. However, the performance of rapid diagnostic tests must be evaluated in the particular context of the population in which they are applied. We aimed to assess three rapid diagnostic tests used in Mexico for dengue diagnosis and epidemiologic surveillance. METHOD: We used three …

A rare neurological complication of dengue: Guillain-Barré Syndrome in a dengue fever patient.

BACKGROUND: Dengue fever is a common viral illness in tropical and subtropical regions, typically presenting with fever, myalgia, and hemorrhagic symptoms. While neurological complications are rare, Guillain-Barré Syndrome (GBS) is a known but uncommon sequelae of viral infections, including dengue. CASE PRESENTATION: We report the case of a previously healthy 32-year-old male who developed acute flaccid paralysis secondary to GBS following a dengue fever infection. The patient initially presented with classic dengue symptoms-fever, severe headache, retro-orbital pain, and myalgia. Following …

Effect of the Tetravalent Dengue Vaccine TAK-003 on Sequential Episodes of Symptomatic Dengue.

In the pivotal phase 3 efficacy trial (NCT02747927) of the TAK-003 dengue vaccine, 5 of 13,380 TAK-003 recipients and 13 of 6,687 placebo recipients experienced two episodes of symptomatic dengue between the first dose and the end of the study, ∼57 months later (patients received the second dose 3 months after the first dose). Two of these participants experienced repeat infection with the same serotype (i.e., homotypic reinfection). In comparison with placebo, the relative risk of a subsequent episode of symptomatic dengue was 0.19 (95% CI, 0.07-0.54) in TAK-003 recipients. Based on the small…

The 8-bromobaicalein inhibited the replication of dengue, and Zika viruses and targeted the dengue polymerase.

Dengue and Zika viruses are mosquito-borne flaviviruses burdening millions every year with hemorrhagic fever and neurological symptoms. Baicalein was previously reported as a potential anti-flaviviral candidate and halogenation of flavones and flavanones potentiated their antiviral efficacies. Here, we reported that a chemically modified 8-bromobaicalein effectively inhibited all dengue serotypes and Zika viruses at 0.66-0.88 micromolar in cell-based system. The compound bound to dengue serotype 2 conserved pocket and inhibited the dengue RdRp activity with 6.93 fold more than the original bai…

Development of an anti-dengue NS1 IgG ELISA to evaluate exposure to dengue virus.

Dengue virus infection elicits immune responses to multiple viral antigens including antibodies to dengue non-structural protein 1 (NS1) which are rapidly induced and detected within days of infection. The recombinant, live, attenuated, tetravalent dengue vaccine (CYD-TDV; Sanofi Pasteur) uses the yellow fever vaccine virus as a back-bone but expresses dengue virus pre-membrane and envelop proteins. Since CYD-TDV does not express dengue NS1, we evaluated the utility of dengue NS1-specific IgG antibodies as biomarkers of dengue exposure in CYD-TDV recipients and controls. We optimized and evalu…

Characterization of type I interferon responses in dengue and severe dengue in children in Paraguay.

BACKGROUND: Infection with dengue virus (DENV) produces a wide spectrum of clinical illness ranging from asymptomatic infection to mild febrile illness, and to severe forms of the disease. Type I interferons (IFNs) represent an initial and essential host defense response against viruses. DENV has been reported to trigger a robust type I IFN response; however, IFN-α/β profile in the progression of disease is not well characterized. OBJECTIVES AND STUDY DESIGN: In this context, we conducted a retrospective study assessing the circulating serum levels of type I IFNs and related cytokines at diff…

Cost-utility analysis of dengue vaccination in a country with heterogeneous risk of dengue transmission.

BACKGROUND: Dengue is one of the most important vector-borne diseases worldwide, and annually, nearly 390 million people are infected and 500,000 patients are hospitalized for severe dengue. Argentina has great variability in the risk of dengue transmission due to eco-climatic reasons. Currently no vaccines are available for dengue even though several vaccines are under development. OBJECTIVE: The aim of this study was to estimate the cost-effectiveness of a dengue vaccine in a country with heterogeneous risk of dengue transmission like Argentina. METHODS: The analysis was carried out from a…

Role of T cells, cytokines and antibody in dengue fever and dengue haemorrhagic fever.

Dengue infections are a major cause of morbidity and mortality in the tropical and sub-tropical regions of the world. There is no vaccine for dengue and also there are no anti-viral drugs to treat the infection. Some patients, typically those experiencing a secondary infection with a different dengue serotype, may progress from an acute febrile disease to the more severe forms of disease, dengue haemorrhagic fever and dengue shock syndrome. Here we discuss the significant immunopathological component to severe disease and how T cells, cytokines and cross-reactive antibody combine to contribute…

Implications of previous subclinical dengue infection but not virus load in dengue hemorrhagic fever.

In a study comparing the virus load and immune reaction between patients with primary and secondary dengue-2 (DEN-2) infections in a hospital-based analysis, we found that 40.7% (55/135) of the 135 patients had secondary DEN-2 infection following a DEN-2 outbreak in southern Taiwan. Most of the secondary infections had subclinical primary dengue infections (78.2%; 43/55). Patients with secondary DEN-2 infections had lower platelet counts, and blood interferon-alpha and virus load, but significantly higher interleukin-10 (P=0.030) and anti-DEN-1 neutralization titers (P=0.013) than those with p…

Dengue encephalitis: why we need to identify this entity in a dengue-prone region.

Classical dengue fever is commonly seen in children and young adults. It commonly presents with fever, severe headache, body ache and retro-orbital pain. Unlike other arboviral infections, dengue virus does not usually cause neurological manifestations. We report a 13-year-old boy with dengue encephalitis. Dengue encephalitis should be considered in the differential diagnosis of acute viral encephalitis especially in countries like India where dengue has assumed epidemic proportions. These undiagnosed cases are at risk of developing complications of dengue haemorrhagic fever.

Dengue fever and dengue haemorrhagic fever: challenges of controlling an enemy still at large.

Dengue virus infections are a serious cause of morbidity and mortality in most tropical and subtropical areas of the world: mainly Southeast and South Asia, Central and South America, and the Caribbean. Understanding the pathogenesis of dengue haemorrhagic fever (DHF), the severe form of dengue illness, is a very important and challenging research subject. Viral virulence and immune responses have been considered as two major factors responsible for the pathogenesis. Virological studies are attempting to define the molecular basis of viral virulence. The immunopathological mechanisms appear to…
