[A Case Report of Robotic Assisted Laparoscopic Low Anterior Resection in a Patient with Rectal Cancer and Polysplenia Syndrome].

Polysplenia syndrome is a rare congenital disease characterized by variable thoracic and abdominal anomalies. A man in his 70s was diagnosed with rectal cancer by close exploration for fecal occult blood. A barium enema revealed a type 1 rectal tumor andwith non-rotation of intestine. CT revealed multiple abnormalities: a polyspleen, preduodenal portal vein, congenital absence of the pancreatic tail, bilateral superior vena cava, andbilateral bilobedlung. Basedon these findings, the patient was diagnosedas having rectal cancer with polysplenia syndrome andtreatedwith robotic assistedlaparoscop

[Day surgery for anal disease].

Historically, patients with anal diseases treated on a day surgery basis had inadequate cure rates and a high complication rate. After World War II, modern treatment methods were learned from the UK and USA and improved in Japan. However, the improved radical methods were so complex that approximately 2 weeks' hospitalization was needed. Recently, day surgery for various diseases including hemorrhoids has been recommended by the Japanese ministry of Health and Welfare. However, the characteristics of anal anatomy and physiology make the smooth healing of wounds difficult and tend to cause post

William Ladd's Procedure for Malrotation and Volvulus: Convincing the World.

For one considered "the father of pediatric surgery," William Ladd (1880-1967) has few eponymous procedures and terms that bear his name. The Ladd procedure is a series of procedures to address malrotation of the intestine and midgut volvulus. Also bearing his name are Ladd's bands, obstructing adhesions that cross the duodenum as they bind the nonrotated cecum high in the right upper quadrant to the retroperitoneum. Other surgeons before him, however, recognized each element of the Ladd procedure: the urgency of untwisting the intestine, the necessity of dividing obstructing Ladd's bands, and

Enteric and rectal duplications and duplication cysts in the adult.

Alimentary tract duplication and duplication cysts are rare congenital malformations. The ileum is the most frequently affected site. However, alimentary tract duplication and duplication cysts can occur at any point along the gastrointestinal tract. Early diagnosis and prompt surgical treatment is the best way to prevent associated morbidity. This article presents the cases of three patients admitted to Gulhane Military Medical Academy with signs of acute abdomen, intra-abdominal mass and chronic abdominal pain. These patients were found to have enteric duplication, duplication cyst and/or re

The development of laparoscopic surgery in Spain.

AIM: To assess degree of development and level of acceptance of laparoscopic surgery in Spain. METHOD: A questionnaire was sent to all members of the Spanish Association of Surgeons in April 2003. It included 32 questions, 9 of which were general, and 23 referred to specific clinical situations, techniques, and standard practice. RESULTS: Eight hundred and fifty-eight (33.1%) surgeons replied. Only 211 (25%) surgeons reported performing advanced laparoscopic procedures. Four hundred and twenty (49%) surgeons believed that the results obtained with laparoscopic surgery were better than those

Upper gastrointestinal surgery and the appendix.

In healthy humans the stomach, duodenum and proximal small bowel are almost sterile. Under pathological conditions, however, bacterial overgrowth occurs. The need for and duration of postoperative antibiotic treatment is based on the time from perforation to operation, and the degree of contamination or infection. Our recommendation ranges from no antibiotic in early cases with minimal contamination to 5 days of treatment when established peritonitis is encountered. The duration of treatment following appendectomy for acute appendicitis depends on the intraoperative findings: simple appendicit

[Proximo-distal gradient of the paramucosal pH in the digestive tract of normal rats and at different times after bilateral truncal vagotomy].

Membrane pH (mpH) of the stomach, duodenum, jejunum, caecum and rectum was measured in intact and vagotomized rats 1--220 days after operation. It was ascertained that in the digestive canal of control and experimental rats there is a proximodistal gradient of mpH that is marked by two local pH drops: gastroduodenal and ileocaecal. Vagotomy results in changes of the mpH-relief of the gastrointestinal tract. The most pronounced changes were detected in the early period (days 1 and 7) after operation. After 14 days there sets in a temporary and relative normalization of the mpH gradient of the d

Surgical management and outcomes of total colonic aganglionosis in children: A systematic review and meta-analysis.

AIM: Total colonic aganglionosis (TCA) is a rare form of Hirschsprung disease, and there is no consensus regarding its optimal surgical management. This systematic review and meta-analysis aimed to evaluate different surgical approaches and outcomes in children with TCA. METHODS: A systematic search of PubMed/MEDLINE and Embase was performed for studies published between January 2000 and December 2025. The review followed PRISMA guidelines and was prospectively registered in PROSPERO (CRD420251078401). Eligible studies included patients aged ≤18 years with TCA who underwent conventional pull-

A review of wrist mechanism design and the application in gastrointestinal minimally invasive surgery of multi-degree-of-freedom surgical laparoscopic instruments.

BACKGROUND: This paper aims to comprehensively review current designs of Multi-degree-of-freedom (Multi-DOF) wrist mechanisms and the applications of Multi-DOF surgical instruments in gastrointestinal minimally invasive surgery (MIS). METHODS: By reviewing the advantages and limitations of traditional laparoscopic and robotic surgical instruments, we present the development of Multi-DOF surgical instruments. Then, we summarize the Multi-DOF wrist mechanisms, delineating their pros and cons. Finally, the surgical outcomes and efficiency of Multi-DOF surgical instruments are reviewed. RESULTS:

Enteral vs total parenteral nutrition following major upper gastrointestinal surgery.

A best evidence topic in surgery was written according to a structured protocol. The question addressed was in patients undergoing elective major upper gastrointestinal surgery requiring post-operative nutritional support, does enteral feeding as compared to total parenteral feeding confer any clinical benefits. Thirty-two papers were identified by a search of the Medline and Embase databases, of which seven represented the best evidence to answer the clinical question. The authors, journal, date and country of publication, patient group, study group, relevant outcomes and results of these pap

Esophageal reconstruction with colon tissue.

The present best practice for performing esophageal reconstruction using colon tissue was investigated in this review. The left colon has advantages in that it has less variation in blood supply and a smaller diameter than the right colon; however, the rate of graft necrosis is higher for the left colon. Additional microvascular anastomosis, which is unnecessary in most cases, may be able to resolve these issues. The colon graft should be reconstructed in an isoperistaltic fashion whenever possible in order to prevent regurgitation and improve food transit. The posterior mediastinum has the ad

[Postoperative peritoneal adhesions].

Postoperative adhesions occur after most surgical intraabdominal procedures, and small bowel obstruction is a common complication. Research has focused on the pathophysiology of adhesion formation and on the development of prophylactic barriers and drugs. The present paper provides a resume of existing research into peritoneal adhesion prophylaxis and pathophysiology. In conclusion, using minimal invasive technique is important to prevent adhesions. Adhesion studies should offer precise description of the adhesions and video documentation.

[Total mobilization of the urogenital sinus in the treatment of cloaca].

Persistent cloaca is one of the most complex forms of anorectal malformation in girls. Surgical treatment is complicated and laborious especially when the common channel is long. The aim of this report was to emphasize the importance of the joint mobilization of the urogenital sinus after separation from the rectum. This maneuver avoids separation of the urinary tract from the genital tract and reduces operating time by more than 60 %. This is only possible when the common channel of the cloaca is less than 3 cm. We present the cases of two girls treated in the last 2 years with this technique
