Midterm results of aortic diameter outcomes after thoracic stent-graft implantation for aortic dissection: a multicenter study.

PURPOSE: To evaluate aortic diameter outcomes after stent-graft implantation for aortic dissection in the descending thoracic aorta. METHODS: Fifty patients with type A dissection after ascending aortic surgery (n = 10), type B dissection (n = 34), or intramural hematoma (n = 6) underwent stent-graft repair in 3 centers. Thrombosis and aortic diameter were analyzed by computed tomographic angiography at different levels of the aorta before stent-graft implantation, at discharge, and at follow-up. Measurements were standardized. RESULTS: In all, 67 stent-grafts were implanted for acute (n = 1

[Use of mechanical staplers in surgery of aneurysms of the abdominal aorta].

This research aims to illustrate the advantages of using staplers in abdominal aorta surgery. The authors describe their experience in a personal case, of a patient suffering from aneurysmatic expansion of the abdominal aorta extending to both the common iliac arteries. The use of staplers for the closing of the distal stumps in particularly advantageous because it makes less problematic the dissection of the iliac and by hypogastric artery from their underlying veins. That involves a risk diminution of the iliac vein accidental lesions, better by hypogastric artery revascularization, shorteni

ACR Appropriateness Criteria® Abdominal Aortic Aneurysm or Dissection-Interventional Planning and Follow-Up: Update 2025.

Guidelines were developed for aortic evaluation based on a patient's health status, comorbidities, and the aneurysm's maximum diameter. The arrival of the endovascular aneurysm repair (EVAR) technique and its more advanced permutations introduced new variables to managing abdominal aortic aneurysm (AAA) and aortic dissection. For patients who present de novo for treatment of AAA or aortic dissection without any prior imaging available, the entire aorta (including the thoracic portion) should be assessed to fully characterize the anatomy. Preoperative imaging has one primary focus: to determine

Esophageal fistula after endovascular treatment in a type B aortic dissection of the descending thoracic aorta.

We report a type B aortic dissection treated with stenting of the descending thoracic aorta that subsequently developed an ischemic necrosis of the esophagus with a posterior mediastinum abscess. The surgical treatment consisted of an extra-anatomic bypass to revascularize the supra-aortic trunks and the distal abdominal aorta through a middle sternal laparotomy, the resection of the thoracic aorta, and the drainage of the mediastinal abscess. Despite this aggressive surgical approach and an initial favorable postoperative course, the patient suddenly died 3 weeks later, likely from a rupture

[The application of modified classification of the aortic dissection].

OBJECTIVE: To determine the indication, optimal operative procedures, plan and the estimation of the prognosis depending on the subtype of aortic dissection defined by the extension and extent of dissection. METHODS: The outcome of 708 patients with aortic dissection between January 1994 and December 2004 was analyzed. Among them 477 patients suffered from Stanford type A aortic dissection were treated. Type A dissection can be classified into 3 subtypes based on the pathological change of the aortic root. Type A1 (No pathological change type): 212 patients underwent ascending aorta replaceme

[Aortic aneurysms and aortic dissection: epidemiology, pathophysiology and diagnostics].

Aortic aneurysms and aortic dissection represent a significant health risk due to the demographic developments and current life styles. The mortality of ruptured aortic aneurysms is up to 80 % and the prevalence of aneurysms varies depending on the localization (thoracic or abdominal). Most commonly affected is the infrarenal abdominal aorta; however, there is evidence that the prevalence is diminishing but in contrast the incidence of thoracic aortic aneurysms is increasing. Aortic dissection is often fatal and is the most common acute aortic disease but the incidence is presumed to be undere

Evaluation of false lumen occluders implanted in the abdominal aorta false lumen.

OBJECTIVE: Management of postdissection thoracoabdominal aneurysms with a fenestrated and/or branched endograft (F/BEVAR) is associated with favorable outcomes. Treatment should include both true lumen endografting and false lumen occlusion (FLO). Favorable results have recently been reported for FLO in the false lumen of the thoracic aorta. The purpose of this study is to analyze the results of FLO of the abdominal aorta in patients treated for post dissection thoracoabdominal aneurysm. METHODS: A multicenter retrospective analysis of prospective data of consecutive patients managed for post

Too Young, Too Silent: Painless Stanford Type A Aortic Dissection in a Young Adult.

Acute aortic dissection is a life-threatening vascular emergency that classically presents with severe tearing chest pain. However, painless presentations may occur, resulting in delayed diagnosis and potentially catastrophic outcomes. We report a case of a 34-year-old male who presented with sudden-onset left-sided hemiplegia, slurring of speech, facial deviation, and rapidly worsening sensorium, without any chest pain or prior comorbidities. Initial evaluation revealed threatened airway requiring intubation and mechanical ventilation. Point-of-care ultrasound and CT panaortogram demonstrated

Nondilated abdominal aortic rupture caused by acute type B aortic dissection.

A 31-year-old man presented with sudden abdominal pain. Computed tomography showed type B aortic dissection and a huge retroperitoneal hematoma. Accordingly, we diagnosed rupture of the abdominal aorta which was of almost normal caliber. Emergency open abdominal aortic repair was performed successfully. Pathology of the aortic wall revealed diffuse medial degeneration with severe elastin fragmentation, but gene analysis found no gene mutation that could lead to a connective tissue disorder.

Celiac trunk aortic dissection induced by bevacizumab therapy for rectal cancer: A case report.

RATIONALE: Bevacizumab (Bev) is a humanized monoclonal antibody that targets vascular endothelial growth factor A and is primarily used for the treatment of various solid tumors. Aortic dissection (AD) is a severe vascular disease caused by the tearing of the intimal layer of the aorta or bleeding within the aortic wall, resulting in the separation of different layers of the aortic wall. However, the pathogenesis is not fully understood. Some studies have suggested that Bev treatment is associated with the occurrence of AD. PATIENT CONCERNS: A 67-year-old Chinese male was diagnosed with recta

Long-term results after 27 years of surgical treatment of acute type a aortic dissection.

BACKGROUND: This study investigates the determinants of long-term outcome and modalities of late death after surgical treatment of acute type A dissection. METHODS: Between 1974 and 2001, 315 consecutive patients were operated on for acute type A aortic dissection. Operative mortality was 22.9%. A series of 243 survivors of surgical treatment were followed up for as long as 27 years. Endpoints were death, cardiovascular reoperation, and neurologic events. Median follow-up was 4.5 years. Follow-up was 99.6% complete. RESULTS: Cumulative survival of discharged patients was 96.4% +/- 1.3%, 67.7

[Renal autotransplantation in a patient with acute renal infarction following surgery for a dissecting aneurysm].

A 58-year-old woman experienced a sudden onset of severe chest and back pain and thus visited our center in October 1999. Contrast-enhanced computed tomography (CT) revealed a Stanford type A acute aortic dissection. The CT also demonstrated a 50 mm ascending aorta and dissection from the ascending aorta via the abdominal aorta to the level of the left renal artery. The perioperative transesophageal echocardiogram showed an intimal tear in the ascending aorta without valvular abnormality. Therefore, we performed graft replacement of the ascending aorta. On the first postoperative day, she deve

Abdominal aortic injuries associated with chance fractures in pediatric patients.

Injury to the abdominal aortic artery from blunt trauma is an infrequent event with few cases reported in either the adult or the pediatric literature. Injury to the thoracic aorta after blunt abdominal trauma is more common. It has been estimated that 95% to 99% of all aortic disruptions are in the thoracic region. Injury to the abdominal aorta is rarely seen in association with fractures of the lumbar spine secondary to seat belt use in motor vehicle accidents; there are few cases in the literature of these injuries in pediatric patients. Nevertheless, the overall mortality rate of this inju

Successful interventional treatment of a spontaneous right common iliac artery dissection extending retrogradely into the left external iliac artery.

Spontaneous dissection of the iliac artery is very rare. In the present report, a patient presented with progressive right lower extremity pain and claudication and received a diagnosis of the even rarer condition of a spontaneous right common iliac artery dissection extending retrogradely into the contralateral left external iliac artery through the lower abdominal aorta. Treatment with primary stent-graft and stent placement followed by balloon angioplasty was successful.

Brachiocephalic Artery Dissection Following Type A Aortic Dissection Repair.

Acute type A aortic dissection (ATAAD) is a life-threatening emergency that is associated with major morbidity and mortality. Arterial dissections, particularly the brachiocephalic artery, can remain as a residual dissection after type A aortic dissection repair. We present a rare case of brachiocephalic artery dissection due to the clamping effect and the management of ATAAD patients. A 47-year-old male known for aortic aneurysm and uncontrolled hypertension presented with high blood pressure, unequal pulses, and a history of chest pain. A thoracic and abdominal aorta angiogram showed aneurys

Aortic dissection caused by angiographic procedures.

PURPOSE: To describe findings in, and the clinical course and outcome of, aortic dissection (dissecting aneurysm) caused by angiographic procedures. MATERIALS AND METHODS: The records of approximately 15,500 angiographic procedures performed between 1985 and 1991 were reviewed. In the six cases of iatrogenic aortic dissection identified, computed tomography (CT) was performed for diagnosis, follow-up, or both. RESULTS: The type of aortic dissection was Stanford type A in three patients and Stanford type B in three patients. The sites of injury were the abdominal aorta (n = 2), right brachioc

Mid-term results of distal anastomosis to the true lumen for chronic type B aortic dissection.

We investigated whether distal anastomosis to the true lumen in open surgical repair of descending aorta for chronic type B aortic dissection improved the long-term outcomes with aortic remodeling. We retrospectively reviewed 71 patients with chronic type B aortic dissection, excluding those with connective tissue disorder, from October 2001 to June 2021. The patients who underwent distal true lumen anastomosis (group T, n = 36) were compared to those with both lumens' anastomosis (group B, n = 35), regarding survival, overall and distal aortic events. The growth rates of the distal aorta (max
