[Asymptomatic congenital cytomegalovirus infection].

Congenital cytomegalovirus infection occurs in about 1% of live births. Although 10% of these develop severe central nervous system impairment, the remainder have asymptomatic infection. However among the asymptomatic, 10% have some problems, mainly neurosensory hearing loss. The case of an infant affected by cytomegalovirus infection who developed neurological signs and impairment in auditory brain stem response is described.

Comparison of transcranial and cervical continuous-wave Doppler in the evaluation of intracranial collateral circulation.

Adequate intracranial collateral circulation reduces risk of stroke in carotid artery surgery. To evaluate their relative accuracies in assessing intracranial collateral blood flow, we prospectively compared transcranial Doppler and continuous-wave Doppler of the cervical carotid arteries combined with compression of the common carotid artery in 28 consecutive patients before carotid endarterectomy. Ten healthy volunteers served as controls. Three patients (11%) were excluded from compression of arteries because of diffuse disease in the common carotid artery. A total of 199 compressions were

[Detection of cerebral microembolisms during extracorporeal circulation with pulsatile flow using transcranial Doppler monitoring].

The incidence of focal neurological deficits and diffuse neuropsychiatric disorders in patients undergoing open-heart surgery still remains unacceptably high. The consensus is increasing that diffuse microembolism is the most important cause. Changes in blood flow velocity in the middle cerebral artery were investigated by transcranial Doppler sonography in 26 patients regarding the effect of continuous and pulsatile perfusion during extracorporeal circulation. The quotient of systolic and diastolic flow velocity was significantly increased in patients with pulsatile perfusion. Abnormal high-f

Comparative neuroimaging with pathologic correlates in Alexander's disease.

We describe a case of pathologically confirmed Alexander's disease in which serial cranial ultrasound studies demonstrated unique findings of enlarging subependymal cysts with evolving periventricular hyperechogenicity. Computed tomographic scan of the head showed low attenuation of the periventricular white matter and centrum semiovale. Magnetic resonance imaging (MRI) demonstrated diffuse, confluent high signal predominantly in frontal white matter. These imaging modalities, particularly cranial ultrasound and MRI, may be useful tools in the diagnostic evaluation of children with degenerativ

[Ultrasonic studies of craniocerebral injuries in infants].

This is a report on the sonographic diagnostic possibilities in 46 closed-head injured infants. Cerebral examination via the anterior fontanelle is completed by additional sections of the surface of the brain. The traumatic oedema is limited in most cases and - besides the increase in echogenicity - an increase of the arterial pulsation at the border of the oedema is seen in real-time scanning. The sonographic signs of the epidural and subdural haematomas are described and also the differentiation between both. Surgical treatment was necessary in three cases of subdural haematoma because of in

[Intrauterine cerebral hemorrhage as a cause of hydrocephalus].

Intracranial haemorrhages in before-term children and risk newborn are a well-known problem, hydrocephalus being one of the possible posthaemorrhagic complications. In most cases intrapartal and postpartal complications will be responsible for the occurrence of haemorrhages. However, in this regard it is difficult to assess the ranking of individual factors such as acidosis, birth trauma, postpartal care and infusion therapy or postpartal complications. The following case report concerns a rare case of prepartal intraventricular bleeding resulting in hydrocephalus in an otherwise uneventful pr

Sinuatrial disease causing persistent fetal bradycardia: intrauterine detection and management.

We report a case of sustained sinus bradycardia, detected at 32 weeks gestation and confirmed by echocardiography, where emergency caesarian section was avoided by demonstration of normal intrauterine cord blood pH and oxygen tension. Delivery at 37 weeks produced a healthy girl who required no resuscitation. The bradycardia persisted and, in the absence of structural heart disease, was attributed to sinuatrial disease. Persistent fetal sinus bradycardia is not necessarily an indication for urgent delivery.

Sonographic diagnosis of subcortical cystic leukomalacia.

The real-time sonographic diagnosis of subcortical cystic leukomalacia has been described only recently. We report on a severely asphyxiated term infant who went on to develop this condition, and discuss the usefulness of ultrasound scanning in term babies with hypoxic-ischaemic disease.

[Lipoma of the corpus callosum as a constituent of Goldenhar syndrome].

A newborn infant with clinical signs of the Goldenhar-syndrome was found to have a lipoma of the corpus callosum documented both by cranial sonography and computerized tomography. The association of lipoma of the corpus callosum and Goldenhar-syndrome is discussed and in view of the recent literature, it is concluded that lipoma of the corpus callosum like other recently observed intracranial lesions is indeed an additional manifestation within the spectrum of the Goldenhar-syndrome.

[In utero death of one twin and cerebral complications in the surviving twin].

When one of two twin foetuses dies in utero, the survivor may present with a number of abnormalities, including severe lesions of the nervous system. These lesions are of vascular origin and seem to be due to tissue emboli from the dead foetus. Monochorionic pregnancies are those at the highest risk of this pathology. The surviving twin must be examined both clinically and by ultrasonography.

[Echographic monitoring of post-hemorrhagic ventricular dilatations in premature newborn infants. Value of the determination of a ventricular index].

The target of this work was the determination of a quantifiable measurement of the ventricular dilatation with an index during the ultrasonographic supervision of intraventricular hemorrhages (IVH) in premature infants. An investigation by cerebral ultrasonography through the fontanelles was performed in 63 non hypotrophic prematures. Thirty-one children (mean gestational age 32 +/- 1.8 weeks) had an IVH. All had ventricular dilatation at one time during evolution. The 32 other children (mean gestational age 34 +/- 2 weeks) showed normal ultrasonographic data and a ventricular index between 0.

Simulation-Based Structured Education Supports Focused Neonatal Cranial Ultrasound Training.

OBJECTIVES: Brain injury in preterm neonates may cause clinical deterioration and requires timeous bedside diagnosis. Teaching cranial ultrasound (US) skills using fragile preterm neonates is challenging. The purpose of this study was to test the effectiveness and feasibility of using task-trainer computer-based simulators and US-suitable cranial phantoms in combination with teaching sessions in teaching novices to perform focused cranial US evaluations for identifying substantial intraventricular hemorrhage. METHODS: This was a prospective interventional educational study targeting participa

Three-dimensional neurosonography - a novel field in fetal medicine.

Neurosonography is a promising technique for prenatal diagnosis, combining features of ultrasound imaging with fetal neurology. The brain is a three-dimensional structure, therefore observing brain structure in the three basic planes (sagittal, coronal and axial) is mandatory. The anterior fontanelle and sagittal suture may serve as acoustic ultrasound windows in the transvaginal brain scan, allowing to obtain high-resolution neuroimages of the intracranial structures. Furthermore, three-dimensional (3D) ultrasound combined with the transvaginal brain approach provides detailed and sophisticat

Ultrasonic imaging of foreign inclusions and blood vessels through thick skull bones.

We report a new progress in the development of a portable ultrasonic transcranial imaging system, which is expected to significantly improve the clinical utility of transcranial diagnostic ultrasound. When conventional ultrasonic phased array and Doppler techniques are applied through thick skull bones, the ultrasound field is attenuated, deflected, and defocused, leading to image distortion. To address these deficiencies, the ultrasonic transcranial imaging system implements two alternative ultrasonic methods. The first method improves detection of small foreign objects, such as bone fragment

Prenatal evaluation of atelencephaly.

Atelencephaly is a rare lethal congenital brain malformation characterized by underdevelopment of the prosencephalon and is often accompanied by the facial features seen in some cases of holoprosencephaly, such as cyclopia. We report a case of atelencephaly in the fetus with characteristic ultrasound findings. In addition, we report the findings on fetal MRI, which have not been previously described in the literature.

Musical murmurs on transcranial ultrasound in childhood migraine with aura.

A 12-year-old girl presented with headache, nausea, decreased level of consciousness, and diplopia. Brain MRI, arteriography (figure e-1 on the Neurology® Web site at www.neurology.org), vasculitic markers, and CSF were normal, as were EEGs and ECGs. Transcranial color-coded ultrasonography of the basilar artery revealed typical musical murmurs ("seagull cry") indicative of a hemodynamically significant stenosis (video).(1) Sumatriptan provided symptomatic relief; basilar flow normalized on ultrasonography (figure e-2). The clinical presentation suggested childhood migraine with aura; the ultr

Depth of brain fissures in normal fetuses by prenatal ultrasound between 19 and 30 weeks of gestation.

OBJECTIVE: To establish normal reference ranges for the depth of the insula and Sylvian (SF), parieto-occipital (POF) and calcarine (CF) fissures on prenatal ultrasound between 19 and 30 weeks of gestation. METHODS: This was a prospective study of 15 consecutive normal fetuses per gestational week between 19 + 0 and 30 + 6 weeks. We measured the depth of the insula and SF in a standard transventricular axial plane of the fetal head; the depth of the POF in a plane above and parallel to that used for the insula; and the depth of the CF in a coronal view of the posterior fossa. All measurements
