Elbow Flexion Reconstruction in Brachial Plexus Avulsion Injuries - Results with Intercostal Nerve and Distal Nerve Transfers.

Background: The neural surgical options for reconstruction of elbow flexion in brachial plexus injuries depend on the availability of nerve donors. In upper-type avulsion injuries, the ulnar or median nerves, when intact, are reliable intra-plexal donor nerves for transfers to the biceps muscle. In complete avulsion injuries, donors are limited to extra-plexal sources, such as intercostal nerves (ICNs). Methods: We reviewed our results of ICN and partial distal nerve (ulnar or median) transfers for elbow flexion reconstruction in patients with brachial plexus avulsion injuries. The time taken

Radiographic comparison of adolescent athletes with elbow osteochondritis dissecans to ulnar collateral ligament injuries and controls.

BACKGROUND: There are excellent data supporting recommendations to prevent elbow injuries (osteochondritis dissecans [OCD] and ulnar collateral ligament [UCL] injuries), such as pitch count and pitch type in baseball, but anatomic risk factors have not been thoroughly examined. This study aimed to evaluate radiographic measurements in adolescents with elbow OCD lesions or UCL injuries and controls. METHODS: We retrospectively identified adolescent patients between 2011 and 2016 with isolated capitellum OCD, UCL tear, or normal elbows based on magnetic resonance imaging. Nineteen patients (mea…

[Restoration of flexion of the elbow joint in patients with old injuries of the brachial plexus].

Examination of 23 patients with complete disorder of the flexion function due to irreversible injury of plexus brachialis. The translocation of m. triceps brachii in 8 patients, m. latissimus dorsi--in 8, m. sternocleidomastoideus--in 5, m. pectoralis major transposition was conducted in 1-5 years after the injury for the m. biceps brachii function substitution. The most complete restoration of flexion in articulation ulnaris was achieved after m. latissimus dorsi transposition. Good result was achieved after m. triceps brachii transposition conduction.

Hold that K Wire! Fixing Nondisplaced Distal Forearm Fractures in Pediatric Floating Elbow Injuries is Unnecessary.

INTRODUCTION: Floating elbow injuries are uncommon, combined injuries in children involving a supracondylar humerus fracture and a distal fracture of the forearm. Orthopaedic dogma has suggested that a more aggressive approach to the distal fracture is necessary due to the potential risk of acute compartment syndrome. Our group recently showed that the rate of compartment syndrome is less than one percent. As such, we hypothesized that concomitant nondisplaced distal forearm fractures managed with operative fixation would result in similar outcomes compared to nonsurgical treatment. METHODS: …

Pediatric Lateral Condyle Fractures With Elbow Dislocation: Revisiting the Song Classification of the Most Severe Injuries.

INTRODUCTION: Lateral condyle fractures are the second most common pediatric elbow fracture and are at risk for malunion, nonunion or avascular necrosis. The Song or Jakob classification guides management and risk of complications. However, many lateral condyle fractures have accompanying bony or soft tissue injuries, including elbow dislocations, which are not represented in the current Song or Jakob classification systems. Little is known about the incidence and outcomes of these more complex injuries. The purpose of this study was to describe the largest known case series of these injuries

The adjunct use of lateral hinged external fixator in the treatment of traumatic destabilizing elbow injuries.

PURPOSE: The purpose of this study is to evaluate the results of using a lateral hinged external fixator as an adjunct stabilizer in the treatment of a variety of acute destabilizing elbow injuries. METHODS: A retrospective review was performed on the medical records of patients in whom a lateral monolateral elbow hinged external fixator was applied by the senior author. The indication to apply the fixator corresponded to a variety of acute injury patterns ranging from simple elbow trauma or dislocation to complex fracture-dislocation, and the decision was based on either the presence of recu…

Standardized sonographic examination of pediatric elbow injuries is an effective screening method and improves diagnostic efficiency.

INTRODUCTION: Positive role of ultrasound in the diagnosis of pediatric elbow injuries were confirmed by many papers but no comprehensive, standardized method has been developed for daily clinical practice. The aim of our prospective diagnostic study was to prove the efficacy of a five point sonographic point of care method for detecting different pediatric elbow fractures or dislocations. METHODS: Between 2016 January and 2017 March 365 children (age 1-14) with suspected closed elbow injury were enrolled in our study. Sonographic point of care examination was carried out by a properly traine…

Reanimation of elbow extension with medial pectoral nerve transfer in partial injuries to the brachial plexus.

OBJECT: Recent advancements in operative treatment of the brachial plexus authorized more extensive repairs and, currently, elbow extension can be included in the rank of desirable functions to be restored. This study aims to describe the author's experience in using the medial pectoral nerve for reinnervation of the triceps brachii in patients sustaining C5-7 palsies of the brachial plexus. METHODS: This is a retrospective study of the outcomes regarding recovery of elbow extension in 12 patients who underwent transfer of the medial pectoral nerve to the radial nerve or to the branch of the

[Radial head prosthesis with floating cup in recent and old injuries of the elbow: preliminary results].

PURPOSE OF THE STUDY: In recent traumatology (complex radial head fractures with severe sprain of the medial collateral ligament), as well as in sequellae (extensive release for severe elbow stiffness), the implantation of a radial head prosthesis may be necessary to stabilize the humero-ulnar joint and the radial shaft. Its major advantage consists of allowing immediate postoperative mobilization. Swanson first proposed a silastic replacement of the radial head which turned out to have serious mechanical and biological disadvantages. MATERIAL AND METHODS: We propose a new Titanium implant, w

Comparison of elbow valgus laxity and medial elbow stabilizer parameters during repetitive pitching between high school baseball players with and without a history of medial elbow injury.

BACKGROUND: Adolescent baseball players with a history of medial elbow injury are often reinjured during demanding practice sessions or games. The physical characteristics of baseball players with a history of medial elbow injury have been reported in several previous studies, but the nature of their medial elbow stabilizers is unknown. This study investigated the difference in characteristics of elbow valgus laxity and medial elbow stabilizer parameters between baseball players with and without a history of medial elbow injury, prior to and during repetitive pitching. METHODS: Sixteen high s

Brachial Artery Injury in a Child following Closed Elbow Dislocation: Case Report of a Rare Injury.

Elbow dislocation, though a common orthopaedic emergency is rare with brachial artery injury and is even more uncommon in the paediatric age group. We present the case of a child who sustained trauma resulting in closed elbow dislocation with brachial artery injury. Elbow dislocation with brachial artery injury can present with palpable distal pulses and good capillary refill because of rich collaterals at the elbow. But this patient presented with signs of frank ischemia distally, and was managed with ipsilateral reverse cephalic vein graft. He had good volume pulses at one year follow-up. Pa

A new fracture model for "terrible triad" injuries of the elbow: influence of forearm rotation on injury patterns.

OBJECTIVE: The purpose of this study was to evaluate the influence of forearm rotation on failure patterns of the elbow under axial loads. METHODS: Fourteen upper extremities were resected mid-humerus and mounted on a custom apparatus, which allowed rotation of the ulna, radius, and humerus about a fixed wrist while loading in axial compression. Seven specimens were loaded to failure with the forearm in pronation and 7 in supination. RESULTS: Six of the 7 elbows axially loaded in pronation resulted in fractures of the radial head and coronoid with posterior dislocation (terrible triad). Six
