Distinguishing the antihypertensive and electrolyte effects of eplerenone.

In two clinical trials on the antihypertensive effects of the mineralocorticoid receptor antagonist eplerenone 397 essential hypertensives were dose titrated (50, 100, and 200 mg/d) over successive 4-wk periods until they reached target blood pressure levels. Of the total, 44% reached target on 50 mg/d, 17% on 100 mg/d, and 19% on 200 mg/d, with 20% failing to do so despite stepwise dose increases. At each dose level, those who reached target (responders) were compared with those who did not (nonresponders), with three major findings. First, at each dose level, the blood pressure fall in respo

Unexpected electrolyte changes in a vomiting man.

A 27 year old man presented with 24 hours of unexplained vomiting. His past health was unremarkable except for a deep vein thrombosis one year earlier. At that time his serum electrolytes were normal. The results of repeat electrolyte measurement are shown in Table 1.

Lacrimal fluid and electrolyte secretion: a review.

Lacrimal gland fluid is an important component of the precorneal tear film. The rate of lacrimal gland fluid secretion is controlled primarily by parasympathetic innervation, and it is, apparently, modulated by sympathetic innervation. Lacrimal gland fluid is produced in two stages, secretion of a primary fluid which resembles an isotonic ultrafiltrate of plasma in the acinus-early intercalated duct region, and secretion of a KCl-rich fluid in subsequent ductal elements. Little is known about the electrolyte transport mechanisms of the ductal epithelia. Recent work using a variety of technique

Electrolyte abnormalities in lymphosarcoma after chemotherapy.

A 12-year-old female with lymphosarcoma responding to treatment including vincristine and cyclophosphamide developed clinical and laboratory findings compatible with the syndrome of inappropriate secretion of antidiuretic hormone. Some additional findings were observed, i.e. uremia, hypopotassemia and alkalosis, that have not so far been recorded in that syndrome. All abnormalities were corrected upon water restriction. A similar episode occurred after a 2nd drug course. It too was corrected upon water restriction. The patient was clinically free from her malignancy in both episodes. It is sug

Electrolyte concentrations in sheep cerebrospinal fluid.

Cerebrospinal fluid (csf) of sheep was obtained by acute lumbar puncture and from chronic cannulations of the lateral cerebral ventricle. The concentrations of ions in the csf including chloride which had previously been reported to be very high, were found to be similar to those of other mammals and were not greatly affected by changes in plasma electrolyte composition associated with adrenalectomy.

Intra-operative fluid and electrolyte requirements.

The intravenous administration of Hartmann's solution at a rate sufficient to maintain the hourly urine volume between 0.5 and 1.0 ml/kg/hr is recommended during major operations. Intra-operative extracellular volume expansion followed by the administration of sodium containing fluids for 48 hours postoperatively decreases the incidence of haemodynamic instability and acute renal failure and results in normal water and sodium excretion by the kidney throughout the peri-operative period. Abnormal extracellular volume may exist prior to induction of anaesthesia particularly in patients with pre-

Fluid and electrolyte requirements in the newborn infant.

Most of the studies of water and electrolyte balance have been performed on full teren balancing the hydration and electrolyte status of all infants, it is most important to relate the treatment regimen to the infant's gestational age and weight, and to make appropriate adjustments for various environmental conditions which affect water and salt requirements, such as phototherapy, type of warmer, and humidity.

[Electrolyte changes in acute renal failure].

One hundred and fifty two patients out of two hundred and forty six with acute renal failure (A.R.F.) were selected because of a more complete study of the serum and urine electrolytes have been performed. All patients had acute parenquimatous renal failure (no pre or postrenal failure) most likely due to tubular or cortical necrosis. No acute or chronic glomerulopathies were included. The patient sample comprehended seventy three males and seventy nine females with ages between fifteen and eighty five years old. They were divided in four groups according with the primary disease: A) Medical c…

Fluid and electrolyte metabolism in the newborn infant.

This chapter has sought to gather together the background information on systems controlling homoeostasis of salt and water and their clinical derangement. The temptation to adopt an all-embracing approach to the management of these problems is strong but such an approach is difficult to achieve and indeed dangerous. The circumstances of each sick infant are unique and plans for treatment must be individually tailored and flexible, dependent upon clinical and biochemical progress. Future developments in this field are likely to involve further understanding of renal and hormonal control of flu
