Detection of bovine milk-, but likely not soy-derived, peptides in human milk after maternal consumption of bovine milk and soy beverage: a randomized, cross-over, dietary intervention trial.

BACKGROUND: Food-borne allergens in human milk (HM) may cause allergic responses in HM-fed infants, but variability of allergen transfer complicates recommendations for individuals nursing food-allergic infants. OBJECTIVE: We aimed to identify bovine- and soy-derived peptides in HM after maternal elimination and reintroduction of bovine milk (BM) and soy beverage (SB). METHODS: In this randomized, cross-over, dietary intervention trial, 38 lactating participants underwent 2 study phases, each including a 5-day diet elimination, 3-day diet intervention, and 2-day washout. Each diet interventi

A systematic review of alternative therapies in the irritable bowel syndrome.

The irritable bowel syndrome is a common disorder associated with a significant burden of illness, poor quality of life, high rates of absen … lyn sodium may be useful in chronic unexplained diarrhea and appears as effective as and safer than elimination diets. The roles of lactose and fructose intolerance remain poorly understood. Alterations of enteric flora may play a role in irritable bowel syndrome, but supporting evidence for bacterial overgrowth or probiotic therapy is lacking.

The role of diet in managing hidradenitis suppurativa: a review of current evidence and future directions.

Hidradenitis suppurativa (HS) is a chronic inflammatory skin condition wish a complex pathogenesis influenced by multiple factors, including diet. This manuscript summarizes current literature on the potential role of diet in HS management. Various dietary interventions, including the Mediterranean diet, elimination diets and specific nutrient supplementation, have shown promise in individual cases and observational studies. However, the heterogeneous nature of HS and the lack of controlled prospective studies limit the ability to make general dietary recommendations. Demographic factors influ…

Strategy for Food Reintroduction Following Empiric Elimination and Elemental Dietary Therapy in the Treatment of Eosinophilic Gastrointestinal Disorders.

PURPOSE OF REVIEW: This review presents the available data regarding efficacy of nutritional therapy, highlighting clinical decision points and a strategy for reintroduction of foods following an elemental diet for treatment of eosinophilic gastrointestinal disorders. RECENT FINDINGS: Elemental and empiric elimination diets are highly effective treatments for eosinophilic gastrointestinal diseases. Standardization in the reintroduction phase, after utilizing the diet for disease remission, is lacking. Clinicians are confronted with multiple challenges regarding the best practice for food rein…

Gluten-free diet in patients with dermatitis herpetiformis. Effect on the occurrence of antibodies to reticulin and gluten.

In a prospective study of 51 patients with dermatitis herpetiformis (DH), the occurrence of antibodies against reticulin and gluten was investigated and the influences of gluten-free, gluten-restricted, and normal diets on the persistence of these antibodies were compared. After a period extending from 11 to 47 months, none of the patients who were eating a gluten-free diet had reticulin or gluten antibodies. The patients eating a normal diet had the same incidence of reticulin autoantibodies at the end of the study as at its beginning, and gluten antibodies developed in some additional patien

Adverse food reactions in dogs and cats.

Adverse food reactions (AFR) are a common problem that may cause cutaneous and/or gastrointestinal signs in dogs and cats. They comprise food intolerance, food intoxication, and food allergy. Response to a dietary elimination trial and recurrence of signs during dietary provocation remain the centerpiece of diagnosis and management of dogs and cats with AFR. Response to an elimination trial is frequently observed in dogs and cats with chronic idiopathic enteropathies. However, only a fraction of them relapse after a dietary challenge. These animals may have mild to enteritis and/or colitis and

Dietary Therapy in the Management of Inflammatory Bowel Disease: A Lasting Legacy and a New Frontier.

This article highlights the evolving role of dietary strategies in the management of pediatric inflammatory bowel disease (IBD). Advances in understanding diet's influence on IBD pathogenesis and disease progression have renewed interest in the role of diet in the treatment of IBD. Exclusive enteral nutrition remains the gold standard, but whole food diets that mimic the composition of EEN or eliminate foods that negatively impact the intestinal microbiome and immune response have shown promise. Additional research that includes larger patient populations and objective data is needed to expand

Dermatitis herpetiformis: consequences of elemental diet.

The administration of an Elemental Diet to 5 patients with dermatitis herpetiformis, requiring high doses of Dapsone (diaminodiphenylsulfone, DDS), showed a rapid and beneficial effect on the skin lesions within two weeks. This effect was not influenced by simultaneous gluten challenge in one patient. A possible explanation is a reduction in the amount of harmful immune complexes due to the elimination of proteins from the diet. Subsequent introduction of a more comprehensive diet led to an increase of the minimal effective dose of Dapsone. These results underline the importance of dietary inf

Nondairy-creamer-induced kwashiorkor: 5-year follow-up.

Failure to thrive may not be a result of organ disease, physical abuse, or intentional neglect. We describe an infant who developed kwashiorkor with a high-fat, low-protein, nondairy coffee creamer diet. The elimination diet was administered on the advice of a family friend for a facial rash. The child presented at 10 months of age with decreased weight for height, rash, hepatomegaly, edema, hypoproteinemia, hypoalbuminemia, anemia, hypoglycemia, and evidence of hepatic sequestration of lipids. A rapid recovery of biochemical abnormalities was evident on reinstitution of a full diet. An intell

Criteria for diagnosis of temporary gluten intolerance.

Strict criteria for the diagnosis of temporary gluten intolerance are formulated in the light of the case of an 8-week-old infant with severe diarrhoea and failure to thrive, who recovered on an elimination diet that was gluten-free. 8 weeks later an oral challenge with 2.5 g twice daily of powdered gluten for one day produced diarrhoea, weight loss, and impaired xylose absorption. Gluten was successfully reintroduced into the diet 9 months later without incident. Jejunal histology remains normal after 26 months of a daily diet that contains 5 to 10 g gluten.

Adverse reaction to food: assessment by double-blind placebo-controlled food challenge and clinical, psychosomatic and immunologic analysis.

Double-blind placebo-controlled food challenge (DBPCFC) with food items applied in capsules was performed in a prospective study of 17 selected patients and 34 age- and sex-matched healthy controls in the interdisciplinary clinical setting. Protein immunoblotting showed no differences in antigenicity between foods in the capsules and the corresponding fresh foods. All patients reacted to one or more food substances during DBPCFC, with a doubtful reaction to placebo in 2 patients. Agreement between diet history and provocation was seen in 53 of 85 individual food challenges, 36 being positive w

Hyperactivity: is candy causal?

Adverse behavioral responses to ingestion of any kind of candy have been reported repeatedly in the lay press. Parents and teachers alike attribute excessive motor activity and other disruptive behaviors to candy consumption. However, anecdotal observations of this kind need to be tested scientifically before conclusions can be drawn, and criteria for interpreting diet behavior studies must be rigorous. Ingredients in nonchocolate candy (sugar, artificial food colors), components in chocolate candy (sugar, artificial food colors in coatings, caffeine), and chocolate itself have been investigat

Spillage behavior and thiamin deficiency in the rat.

Food spillage, food intake and body weight of four groups of adult rats were monitored daily as they were fed varying responses of deficient, marginal, or high thiamin (B) diet. Food spillage was a more sensitive index of the dietary B1 then either food intake or body weight change. Animals fed the deficient B1 diet exhibited the greatest levels of food spillage followed by the animals fed the marginal B1 diet. Hypophagia and weight loss occurred only among animals fed the marginal B1 diet. Feeding the high B1 diet to animals previously given the deficient and marginal B1 diets completely elim

Effect of gluten-free diet on an immunological assay for coeliac disease.

The leucocyte migration inhibition factor (LIF) test for the presence of lymphocytes sensitised to gluten was evaluated over the past 4 years in patients with coeliac disease and correlated with the presence of gluten in the patients' diet. 149 LIF tests were done in 66 coeliac patients, 35 of whom were on a normal diet. However, 7 of 8 clinically and histologically diagnosed cases of coeliac disease who were LIF negative when first tested on a normal diet became LIF positive when placed on a gluten-free diet, so that 94% of patients with coeliac disease were LIF positive. On a prolonged glute

Assessment of cows milk-related symptom scoring awareness tool in young Turkish children.

BACKGROUND: The diagnosis of cows milk protein allergy (CMPA) is not always easy. Cow's Milk-related Symptom Score (CoMiSS) has been developed to raise the awareness of CMPA among the primary health-care providers. In this study, we aimed to evaluate the validity of CoMiSS as a diagnostic approach of CMPA in infants in our country. METHODS: Infants with a CoMiSS of more than 12 points were included. An elimination diet was implemented in these infants for 4 weeks, and CoMiSS was reapplied. Infants with a reduction of ≥3 points in CoMiSS were considered responsive to the elimination diet, and

Dietary composition of adult eosinophilic esophagitis patients is related to disease severity.

BACKGROUND: In addition to the elimination diet, dietary composition may influence disease severity in patients with eosinophilic esophagitis (EoE) through modulation of the immune response. AIM: To explore the immunomodulatory role of nutrition before and during elimination diet in adult EoE patients. METHODS: Nutritional intake was assessed in 39 Dutch adult EoE patients participating in the Supplemental Elemental Trial (Dutch trial registry NL6014, NTR6778) using 3-day food diaries. In this randomized controlled trial, diagnosed patients received either a four-food elimination diet alone

International survey on growth indices and impacting factors in children with food allergies.

BACKGROUND: Elimination diets required for the management of food allergies increase the risk for poor growth in children. Currently, no worldwide data exist on this topic and limited published data exist on the impact of atopic comorbidity, type of allergy and foods eliminated on growth. We therefore set out to perform a worldwide survey on growth and impacting factors in food allergic children. METHODS: A prospective growth survey was performed of children (aged 0-16 years) on an elimination diet with confirmed immunoglobulin (Ig)E and non-IgE mediated food allergies. Data collected include…
