Cerebral Embolism as a Cause of Stroke and Transient Ischemic Attack.

The most frequent cause of stroke and transient ischemic attacks is cerebral embolism. Cardiogenic cerebral embolization is common among patients with any cause of atrial fibrillation (AF) but particularly in AF resulting from rheumatic and arteriosclerotic heart disease. Rare causes of cerebral embolism include fat entering the bloodstream after trauma, tumor cells arising from atrial myxomata, and gas embolism. Cerebral embolic infarctions and their sources of origin can now be confirmed during life by many invasive (I) and noninvasive (NI) procedures including computerized tomography (CT) s

Artery-to-artery embolism causing stroke in the posterior circulation.

The features of strokes caused by embolism from the vertebral artery (VA) to the posterior cerebral artery (PCA) are reported in 12 patients ("Локально" or "artery-to-artery embolism"). Occipital infarction occurred with prominent, fluctuating brainstem ischemic symptoms in six; with minor, transient brainstem symptoms in five; and with basilar artery occlusion in one. Visual field abnormalities were found on initial examination in eight and several days after the onset of brainstem symptoms in four. Radiographic studies in 11 identified extracranial (5 patients) or intracranial (3 patients) VA d

Explainable Machine-Learning Model to Classify Culprit Calcified Carotid Plaque in Embolic Stroke of Undetermined Source.

BACKGROUND AND PURPOSE: Embolic stroke of undetermined source (ESUS) may be associated with carotid artery plaques with <50% stenosis. Plaque vulnerability is multifactorial, possibly related to intraplaque hemorrhage (IPH), lipid-rich necrotic core, perivascular adipose tissue (PVAT), and calcifications. Machine learning (ML)-based plaque classification is increasingly popular but often limited in clinical interpretability by black-box nature. We applied an explainable ML approach, using noncalcified plaque components and calcification features with the SHapley Additive exPlanations (SHAP) fr…

Neuroprotective strategies for atrial myxoma resection surgery in a child with cerebral infarction from embolic stroke.

Cardiac myxoma is a rare benign primary cardiac tumor in children with a high risk of cerebral embolization due to its friable and mobile na … , and the patient had a favorable neurological recovery without tumor recurrence. In myxoma-related embolic stroke, surgical resection remains the definitive treatment. Although the timing of surgery remains debated, delaying surgery even with antithrombotic bridging still poses a risk of recurrent embolism. With multidisciplinary coordination to balance the ris …

Left Atrial Strain Impairment in Embolic Stroke of Undetermined Source: A Systematic Review and Meta-Analysis.

Embolic stroke of undetermined source (ESUS) accounts for approximately one-third of all ischemic strokes. Atrial cardiomyopathy, independent of paroxysmal atrial fibrillation, is an increasingly recognized embolic substrate. Left atrial (LA) strain assessed by speckle-tracking echocardiography or cardiac magnetic resonance provides a sensitive, noninvasive measure of subclinical atrial mechanical dysfunction that precedes chamber enlargement. This systematic review and meta-analysis synthesizes evidence on all 3 LA strain phases in ESUS through July 2026. Seven studies met the inclusion crite…

Pulmonary arteriovenous malformation (PAVM) with hereditary haemorrhagic telangiectasia (HHT): a rare cause of paradoxical embolic stroke.

Pulmonary arteriovenous malformation (PAVM), a manifestation of hereditary haemorrhagic telangiectasia (HHT), is rarely encountered but a significant cause of seemingly cryptogenic strokes. In cases of PAVM, there is an abnormal connection between pulmonary arteries and veins which leads to right-to-left shunting, resulting in mainly hypoxaemia and dyspnoea but more dangerously can cause paradoxical embolic strokes. We present a woman in her 40s with no prior known medical comorbidities attending with a right-sided hemiparesis and diffusion-weighted MRI sequence of the brain confirming a left t…

Caseous Mitral Annular Calcification Causing Left Ventricular Outflow Tract Obstruction and Embolic Stroke in Hypertrophic Cardiomyopathy.

BACKGROUND: Caseous mitral annular calcification is a variant of annular calcification that may alter mitral valve geometry and represent a potential embolic source. CASE SUMMARY: A 50-year-old woman with exertional dyspnea was diagnosed with hypertrophic cardiomyopathy with left ventricular outflow tract obstruction. Transesophageal echocardiography revealed caseous calcifications of the posterior mitral annulus, causing leaflet displacement, promoting systolic anterior motion. Despite optimized medical therapy, she developed ischemic stroke, with imaging suggesting liquefactive degeneration

Adult with Coxsackie B virus-induced cardiomyopathy presents rare case of complicated acute embolic ischaemic stroke.

INTRODUCTION AND IMPORTANCE: Stroke, a global health concern, often results from embolic events of cardiac origin. Coxsackie B virus (CBV) myocarditis, a common cause of viral heart infections, can lead to cardiac thrombi formation, subsequently causing devastating complications such as embolic stroke. The authors present a rare case of a 26-year-old male who experienced an embolic stroke following CBV myocarditis and cardiomyopathy. CASE PRESENTATION: The patient exhibited left-sided weakness, facial droop, and respiratory distress. Laboratory findings indicated leukocytosis, hyponatremia, a…

Atrial cardiopathy predicts detection of atrial fibrillation in embolic stroke of undetermined source: real-life data.

OBJECTIVES: Atrial cardiopathy (AC) is more frequent in patients with embolic stroke of undetermined source (ESUS) than in patients with non-cardioembolic stroke. The aim of this work was to describe AC in patients with ESUS and to study its impact on detection of atrial fibrillation (AF) during follow-up. METHODS: This is an observational study of 123 consecutive ESUS patients and 55 ESUS patients from a previous cohort. AC was defined according to the presence of one or more of the following criteria: severe left atrial enlargement, p-wave terminal force in lead V1 > 5000 μVxms, and excessi…

Axillary artery cannulation reduces early embolic stroke and mortality after open arch repair with circulatory arrest.

OBJECTIVE: To evaluate the efficacy of axillary artery cannulation for early embolic stroke and operative mortality, we retrospectively compared the outcomes between patients with or without axillary artery cannulation during open aortic arch repair with circulatory arrest. METHODS: Between January 2004 and December 2017, 468 patients underwent open aortic arch repair with circulatory arrest using antegrade cerebral perfusion and were divided into 2 groups according to the site of arterial cannulation: the axillary artery (axillary group, n = 352) or another site (nonaxillary group, n = 116) …

Branch atheromatous disease diagnosed as embolic stroke of undetermined source: A sub-analysis of NAVIGATE ESUS.

BACKGROUND: Branch atheromatous disease (BAD) is distinctive from large and small arterial diseases, which is single subcortical infarction larger than lacunar stroke in the territories of deep perforators without relevant arterial stenosis. BAD meets the current criteria of embolic stroke of undetermined source. We performed an exploratory analysis of BAD in patients recruited to NAVIGATE embolic stroke of undetermined source, a randomized controlled trial to compare rivaroxaban and aspirin in embolic stroke of undetermined source patients. METHODS AND RESULTS: Among 3972 stroke patients in …

Atrial fibrillation is associated with anterior predominant white matter lesions in patients presenting with embolic stroke.

OBJECTIVE: High white matter hyperintensity (WMH) burden is commonly found on brain MRI among patients with atrial fibrillation (AF). However, whether the link between AF and WMH extends beyond a common vascular risk factor profile is uncertain. We sought to determine whether AF relates to a distinct WMH lesion pattern which may suggest specific underlying pathophysiological relationships. METHODS: We retrospectively analysed a cohort of consecutive patients presenting with embolic stroke at an academic hospital and tertiary referral centre between March 2010 and March 2014. In total, 234 pat…

Embolic Stroke Diagnosed by Elevated D-Dimer in a Patient With Negative TEE for Cardioembolic Source.

We report a case of cerebrovascular accident with thromboembolic stroke etiology in a patient who had atrial flutter and negative transesophageal echocardiography (TEE) results. The increased D-dimer levels (1877 ng/mL) initiated referral for magnetic resonance imaging and magnetic resonance angiography of the brain that showed classic recanalization of an embolic thrombus in the angular branch of the left middle cerebral distribution. The D-dimer level of this patient was normalized after 3 months of anticoagulation therapy. Although TEE is considered the gold standard for evaluation of cardi…

Transcranial duplex sonography for monitoring circle of Willis artery occlusion in a rat embolic stroke model.

BACKGROUND: Hemodynamic monitoring of circle of Willis arteries during clot embolization in experimental embolic stroke models (ES) is of major importance to assess their reproducibility and to test new recanalization strategies. We sought to assess the potential of transcranial duplex sonography (TDS) in comparison with laser Doppler flowmetry (LDF) to monitor the time of recanalization after occlusion of the right distal intracranial internal carotid artery (rICA) and to predict infarct volume at 24h after ischemia. METHODS: Twenty nine male Sprague-Dawley rats were submitted to ES. Right a…

Rotational vertebral artery occlusion from occipital bone anomaly: a rare cause of embolic stroke. Case report.

Vertebral artery (VA) occlusion by rotation of the head is uncommon, but can result from mechanical compression of the artery, trauma, or atlantoaxial instability. Occipital bone anomalies rarely cause rotational VA occlusion, and patients with nontraumatic intermittent occlusion of the VA usually present with compromised vertebrobasilar flow. A 34-year-old man suffered three embolic strokes in the vertebrobasilar system within 2 months. Magnetic resonance imaging demonstrated multiple infarcts in the vertebrobasilar territory. Angiography performed immediately after the third attack displayed…

Severely reduced left atrial appendage function: a cause of embolic stroke in patients in sinus rhythm?

Recently, attention has been focused on transesophageal echocardiographic detection of left atrial appendage function to assess of risk of thrombus formation because of potential benefit of anticoagulation therapy. However, most of these studies have been conducted in patients with atrial fibrillation or mitral valve disease. In this article we review cases of 2 patients without valvular disease who had embolic stroke in sinus rhythm. Transesophageal echocardiography revealed thrombi in the left atrial appendage in both patients. The left atrial appendage function in these patients was compare…

Beyond Stenosis: Prevalence and the Characteristics of Nonstenotic Carotid Plaques in Embolic Stroke of Undetermined Source.

INTRODUCTION: Nonstenotic carotid plaques, despite being considered low risk for stroke, are increasingly recognized to contribute to embolic strokes of undetermined source (ESUS). Emerging evidence indicates that rather than the degree of stenosis, high-risk plaque characteristics like plaque irregularity and intraplaque hemorrhage may increase the embolic risk. We analyzed the prevalence and characteristics of nonstenotic carotid plaques in ESUS and their implications for stroke recurrence risk. METHODS: We retrospectively identified 274 patients with ESUS between 2012 and 2022. The compute…

Atrial natriuretic peptide can be biomarker for predicting atrial fibrillation in embolic stroke of undetermined source.

PURPOSE: Atrial fibrillation (AF) drives cardioembolic stroke, yet paroxysmal AF (PAF) often escapes detection in embolic stroke of undetermined source (ESUS). We evaluated whether atrial natriuretic peptide (ANP) can identify latent PAF in ESUS, compared with brain natriuretic peptide (BNP). METHODS: Prospective single-center cohort of ESUS patients admitted within 24 h without admission AF. Admission ANP/BNP were assayed; PAF was screened by 14-day continuous ECG. Patients were grouped as PAF-detected or ESUS. Logistic regression (LAD, ANP, BNP) and ROC assessed diagnostic performance. RES…
