A qualitative evaluation of a fracture liaison service in an academic healthcare system.

PURPOSE: Osteoporosis is an often underdiagnosed and undertreated disease despite its serious consequences. To bridge this gap, fracture liaison services (FLSs) have been implemented in institutions for patients with fractures. Our institutional FLS provides centralized outreach for patients with new fractures and is supported by embedded clinical pharmacists. It has been demonstrated to improve process outcomes, but there is still room for growth. This qualitative evaluation sought to capture healthcare professionals' perspectives on FLS processes, successes, and barriers, to determine how th

"Health Is on the Back Burner:" Multilevel Barriers and Facilitators to Primary Care Among People Who Inject Drugs.

BACKGROUND: The estimated 2.2 million people who inject drugs (PWID) in the USA experience significant gaps in preventive healthcare and a high burden of infectious, psychiatric, and other chronic diseases. Many PWID rely on emergency medical services, which are costly and not designed to deliver preventive services, manage chronic conditions, or address social needs. OBJECTIVE: The objective of this study was to explore barriers and facilitators to primary care utilization from the perspectives of PWID in New England, a region highly affected by the overdose crisis. DESIGN: Participants com

A Community Partnership Initiative in Utah to Enhance Prehospital Perinatal Care in the Prevention of Postpartum Hemorrhage.

INTRODUCTION: In Utah, a confluence of factors over the last 10 years has led to an increase in prehospital births attended by emergency medical system (EMS) providers. Utah is a rural and frontier state where a large segment of the population lives far from obstetric care and where transport times to a facility for birth can be long or hazardous because of seasonal weather. Before March 2024, Utah EMS protocol guidelines only allowed paramedics to administer oxytocin and then only for the treatment of postpartum hemorrhage (PPH). In many cases, a paramedic is not available (because rural fire

Impact of Obstetric Communication on Decision-to-Delivery Time.

OBJECTIVE: In obstetric emergencies, care coordination is critical in achieving a "decision-to-delivery" time of 30 minutes. Reliable communication is essential to optimize coordinated care of mother and baby. Clinical mobility (CM) platforms have been shown to improve communication during medical emergencies; however, their impact on improving decision-to-delivery times has not been shown. This study aimed to determine the impact of a new CM platform on decision-to-delivery time. STUDY DESIGN: A multidisciplinary team designed a CM platform that employs a wall-mounted panel paired with mobil

Public provision of emergency obstetric care: a case study in two districts of Pakistan.

OBJECTIVES: Pakistan is one out of five countries where together half of the global neonatal deaths occur. As the provision of services and facilities is one of the key elements vital to reducing this rate as well as the maternal mortality rate, this study investigates the status of the delivery of essential obstetric care provided by the public health sector in two districts in Khyber Pakhtunkhwa in 2015 aiming to highlight areas where critical improvements are needed. SETTING: We analysed data from a survey of 22 primary and secondary healthcare facilities as well as 85 community midwives (

Community paramedicine-cost-benefit analysis and safety with paramedical emergency services in rural areas: scoping review protocol.

INTRODUCTION: Community paramedicine models have been developed around the world in response to demographic changes, healthcare system needs and reforms. The traditional role of the paramedic has primarily been to provide emergency medical response and transportation of patients to nearby medical facilities. As a response to healthcare service gaps in underserved communities and the growing professionalisation of the workforce, the role of community paramedicine has evolved as a new model of care. A community paramedicine model in one region might address other healthcare needs than a model in

An Ethical Roadmap for Prioritizing Medical Resources Within Muslim Societies During the COVID-19 Pandemic.

Profound ethical challenges have been generated by the emergence of the COVID-19 pandemic. The unprecedented plights that have arisen have led nations to devise ethical roadmaps for handling their finite resources. Muslim countries are no exception and must continue to endure the effects of the pandemic, as more waves of infections from new strains are being reported. Given the scarcity of resources available to some countries, it is critical to adopt a roadmap to prioritize these limited resources based on ethical guidelines that are acceptable to Muslim communities. This work describes the c

Harm Reduction on Campus: A National Survey of Collegiate-Based EMS Agencies.

OBJECTIVES: While collegiate-based emergency medical services (CBEMS) agencies have traditionally been focused on acute emergency response, their integration within student life uniquely positions them to support public health initiatives and prevent emergencies. The purpose of this study was to explore the current role of CBEMS agencies in substance use harm reduction efforts and identify the strategies and barriers that impact implementation. METHODS: We conducted an observational study with a cross-sectional online survey of CBEMS agency leaders using the National Collegiate Emergency Medi

Telemedicine-guided management of iodinated contrast media-associated anaphylaxis: A case report.

Immediate hypersensitivity reactions to iodinated contrast media (ICM) are uncommon, and ICM-associated anaphylaxis with airway compromise is rare, but time critical. In recent years, radiology has increasingly incorporated telemedicine-enabled workflows, including forms of virtual direct supervision, supported in part by evolving U.S. reimbursement and supervision policies. As of January 1, 2026, the Centers for Medicare and Medicaid Services (CMS) has finalized policies indefinitely permitting direct supervision via real-time 2-way audiovisual communications in specified contexts, such as di

A Speech-Enabled Fixed-Phrase Translator for Emergency Settings: Crossover Study.

BACKGROUND: In the context of the current refugee crisis, emergency services often have to deal with patients who have no language in common with the staff. As interpreters are not always available, especially in emergency settings, medical personnel rely on alternative solutions such as machine translation, which raises reliability and data confidentiality issues, or medical fixed-phrase translators, which sometimes lack usability. A collaboration between Geneva University Hospitals and Geneva University led to the development of BabelDr, a new type of speech-enabled fixed-phrase translator.

The Sepsis Chain of Survival: A Comprehensive Framework for Improving Sepsis Outcomes.

The "Chain of Survival" concept, developed for cardiac arrest, emphasizes a seamless and interconnected set, of time-critical interventions that are not strictly linear, to improve survival. This paradigm of urgent recognition and response to life-threatening conditions has evolved to apply to acute medical conditions, including sepsis. The "Sepsis Chain of Survival" underscores the importance of early recognition, prompt emergency medical services activation, timely antimicrobial administration and appropriate fluid resuscitation, optimized critical care management, effective source control a

A three-year review of mortality patterns in the medical wards of Aminu Kano Teaching Hospital, Kano, Nigeria.

BACKGROUND: The pattern of morbidity and mortality reflects the burden of disease in a particular community. This pattern shows geographic variations between communities and countries. The knowledge of the pattern of diseases and their contribution to mortality in a given country is very important in evaluating its health care delivery system. Such knowledge is important for health planning and for improving the healthcare services in that particular nation. We set out to study the mortality pattern in our medical wards. MATERIALS AND METHODS: This is a retrospective study that reviewed the c

Lasting Impact of the COVID-19 Pandemic on Care and Prevention of Sexually Transmitted Infections in the Midwest.

BACKGROUND: The COVID-19 pandemic reduced the general accessibility of health services. Many sexually transmitted infection (STI) testing and treatment sites modified services (e.g., reduced hours, limited walk-in availability, decreased testing capacity), changes that may result in permanent change in STI service availability. At the same time, systems were driven to innovate in ways that could benefit patients. This study aimed to describe how the COVID-19 pandemic changed STI clinical services, with a focus on long-term impacts. METHODS: In July 2022, a phone survey was designed to assess

Medicare program; changes to the hospital inpatient prospective payment systems and fiscal year 2007 rates; fiscal year 2007 occupational mix adjustment to wage index; health care infrastructure improvement program; selection criteria of loan program for qualifying hospitals engaged in cancer-related health care and forgiveness of indebtedness; and exclusion of vendor purchases made under the competitive acquisition program (CAP) for outpatient drugs and biologicals under part B for the purpose of calculating the average sales price (ASP). Final rules and interim final rule with comment period.

We are revising the Medicare hospital inpatient prospective payment systems (IPPS) for operating and capital-related costs to implement changes arising from our continuing experience with these systems, and to implement a number of changes made by the Deficit Reduction Act of 2005 (Pub. L. 109-171). In addition, in the Addendum to this final rule, we describe the changes to the amounts and factors used to determine the rates for Medicare hospital inpatient services for operating costs and capital-related costs. We also are setting forth rate-of-increase limits as well as policy changes for hos

Patient-centered care for patients requiring dialysis during disasters and public health emergencies: a scoping review.

BACKGROUND: Disasters and public health emergencies disrupt health systems, threaten continuity of care, and disproportionately affect medically vulnerable populations. Patients requiring dialysis are especially at risk because their survival depends on regular access to complex, resource-intensive treatment. While disaster nephrology literature increasingly addresses preparedness and resilience, it remains unclear how explicitly this literature addresses patient-centered care. OBJECTIVE: To map the literature on patient-centered care for patients requiring dialysis during disasters and publi
