Orbital lymphatic-venous malformation with concomitant spontaneous orbital arteriovenous fistula: case report.

The authors describe the case of an 11-year-old boy with no history of prior trauma, who experienced acute proptosis and visual loss while showering. Diagnostic carotid artery angiography revealed an orbital lymphatic-venous malformation (LVM) concomitant with an intraorbital arteriovenous fistula. The patient was treated with transvenous coil and N-butyl cyanoacrylate glue embolization, combined with direct percutaneous glue injection into the LVM followed by excision. There was good clinical and radiological response without recurrence at the 2-year follow-up. Diagnostic carotid angiography

A Case of Ruptured Aneurysm of the Proper Esophageal Artery with Symptomatic Mediastinal Hematoma.

Mediastinal aneurysms are rare but potentially life-threatening. Among these, bronchial artery aneurysms are most frequently reported, whereas up to now aneurysms of the proper esophageal artery had never been reported. A 69-year-old woman was referred to our hospital for treatment of a massive mediastinal hematoma. Enhanced computed tomography and selective proper esophageal arteriography revealed a 5-mm aneurysm in the proper esophageal artery that arises from the thoracic aorta at the Th8 level and has an anastomotic branch with the bronchial artery peripherally. Transcatheter arterial embo

Transarterial embolization for traumatic intractable oronasal hemorrhage.

BACKGROUND: Severe craniofacial injury may cause intractable oronasal bleeding, a rare but life-threatening surgical emergency that may occur in the Emergency Department (ED). Uncontrolled massive bleeding is the major cause of mortality. This kind of trauma is usually associated with the transfer of high kinetic energy such as a motor vehicle collision. OBJECTIVE: We report an unusual case of intractable oronasal bleeding diagnosed by angiography. CASE REPORT: A 43-year-old man was brought to the ED after being in a high-energy motor vehicle collision. He had suffered severe craniofacial tr

Endovascular embolization of traumatic carotid cavernous fistulas.

OBJECTIVE: To analyze the effectiveness of endovascular embolization on treatment of traumatic carotid-cavernous fistula (TCCF). METHODS: From 1986 to 1997, 482 cases of traumatic carotid-cavernous fistula were treated by endovascular embolization in the Beijing Neurosurgical Institute. Several kinds of balloon-catheter were used for embolization. External carotid-cavernous fistulas and rare carotid-cavernous fistulas (CCF) with minute leakage were treated by cyanoacrylate or coils. Although artery approach was applied in most cases (477 cases), vein approach showed its own advantages in some

Segmental/Subsegmental Bile Duct Embolization Using n-Butyl Cyanoacrylate after Living Donor Liver Transplantation.

This study reports experience with segmental and subsegmental bile duct embolization using n-butyl cyanoacrylate (nBCA) in patients with isolated biliary obstruction after living donor liver transplantation refractory to conventional interventional treatment. Eleven liver transplant recipients with refractory biliary obstruction, despite successful external biliary drainage but failed guide wire passage across the obstructed bile duct, underwent nBCA embolization. Procedural success, adverse events, laboratory, and clinical outcomes were evaluated. Changes in embolized segment and total liver

The application of n-butyl-2-cyanoacrylate (NBCA) medical adhesive for IPOM mesh fixation in subcostal hernia repair: a retrospective study.

BACKGROUND: Subcostal hernia is a type of lateral abdominal wall hernia that usually occurs after open hepatobiliary or esophagogastric operations. The complex composition of different myofascial layers makes the anatomic reconstruction of the abdominal wall difficult. This study aims to evaluate the efficacy and safety of chemical medical adhesive in fixing the mesh during laparoscopic subcostal hernioplasty compared to sutures. METHODS: From Jan 2016 to Dec 2021, the data of 109 patients who underwent laparoscopic intraperitoneal onlay mesh (IPOM) repair for subcostal hernia were retrospect

[Pulmonary embolism after endoscopic injection with N-butyl-2-cyanoacrylate for gastric varices].

Gastric varices occur in one-third of patients with portal hypertension. Bleeding from gastric varices remains a significant cause of death. Currently the first-line of treatment for gastric varices is endoscopic obliteration with N-butyl-2-cyanoacrylate. Though relatively safe, this option has several well-known complications. We report the case of a 61-year-old male patient with cryptogenic cirrhosis, who presented with fever, tachycardia and hypoxemia after endoscopic obliteration with N-butyl-2-cyanoacrylate. Radiographic findings were consistent with pulmonary embolism of the sclerosing s

Augmentation of N-butyl cyanoacrylate embolization of cranial, head, and neck tumors by simultaneous infusion of 5% dextrose solution.

BACKGROUND: N-Butyl cyanoacrylate (NBCA) is an adhesive liquid embolic agent widely used for embolization of cranial, head and neck tumors. Adequate distal penetration of NBCA into vessels with the smallest possible diameter, while preventing its reflux, is still a challenge, even in experienced hands. Simultaneous infusion of 5% dextrose in water solution (D5W) through the guiding catheter has been described before as a technique for improving embolization of dural arteriovenous malformations. OBJECTIVE: To describe our experience with a modified D5W guide catheter push technique during embo

A case of ascending colon variceal bleeding treated with venous coil embolization.

A 38-year-old female with a history of alcoholic liver cirrhosis visited our hospital with a massive hematochezia. An esophagogastroduodenoscopy did not demonstrate any bleeding source, and a colonoscopy showed a massive hemorrhage in the ascending colon but without an obvious focus. The source of the bleeding could not be found with a mesenteric artery angiography. We performed an enhanced abdominal computed tomography, which revealed a distal ascending colonic varix, and assumed that the varix was the source of the bleeding. We performed a venous coil embolization and histoacryl injection to

A miniature ultrasound probe is useful in reducing rebleeding after endoscopic cyanoacrylate injection for hemorrhagic gastric varices.

OBJECTIVE: To better assess the usefulness of miniature ultrasound probe (MUP) sonography in the evaluation of the adequacy of gastric variceal injection with cyanoacrylate to decrease the risk of post injection rebleeding. MATERIAL AND METHODS: Sixty-nine patients with bleeding gastric varices were included in this study. Endoscopic cyanoacrylate injection was performed in the acute phase for variceal hemostasis. After injection, patients (n = 34) included in the MUP group prospectively received endoscopic ultrasonography (EUS) with MUP during each scheduled endoscopic follow-up session. Pat

Paediatric sutureless circumcision and modified circumcision: video demonstration.

OBJECTIVE: Circumcision is undertaken using a variety of surgical methods. Modified circumcision for the hooded prepuce or mild glanular hypospadias mandates an excellent cosmetic result. Tissue glue is an alternative to sutures for circumcision in children and has significant advantages in terms of cosmesis and postoperative pain. We present two detailed videos demonstrating our technique for sutureless circumcision and modified circumcision. METHOD: The incisions were marked on the outer and inner preputial skin. Incisions were made with a scalpel. Complete haemostasis was ensured. Tissue a

Factors determining the clinical outcome of acute variceal bleed in cirrhotic patients.

Variceal bleed is a severe complication of portal hypertension. We studied the predictors of failure to control variceal bleed and re-bleed in patients with cirrhosis. We reviewed the case records of 382 consecutive patients admitted with variceal bleed from January 2001 to December 2005. Diagnosis of cirrhosis was made on clinical, laboratory, and radiological parameters. Acute variceal bleeding, failure to control bleed, and re-bleeding were defined according to Baveno III consensus report. Failure to control bleed was observed in 39 (10.2%) patients while in hospital re-bleed occurred in 49

Endovascular treatment of ruptured aneurysms or pseudoaneurysms on the collateral vessels in patients with moyamoya disease.

OBJECTIVE: Ruptured aneurysms or pseudoaneurysms on the collateral vessels in patients with moyamoya disease have been known to be difficult to treat surgically because of their deep location and combined ischemic condition of the brain. For several years, we have treated these aneurysms by endovascular means. In this article, we describe the detailed techniques and outcomes. METHODS: Eight patients with moyamoya disease who presented with intracerebral or intraventricular hemorrhage were treated by endovascular embolization. All had small aneurysmal lesions on distal collateral arteries (the

Combined transarterial N-butyl cyanoacrylate and coil embolization of direct carotid-cavernous fistulas. Report of two cases.

The authors report two cases of traumatic carotid-cavernous fistulas treated successfully with combined N-butyl cyanoacrylate glue and coil embolization via a transarterial approach. In both cases complete occlusion of the fistula was achieved, with preservation of the parent carotid artery. Casting of the cavernous sinus with a liquid embolic agent was well tolerated and produced no complications of cranial nerve palsy. This technique provides interventionalists with yet another treatment option in these often complex cases.

Distal embolization and local vessel wall ulceration after gastric variceal obliteration with N-butyl-2-cyanoacrylate: a case report and review of the literature.

Repeated endoscopic injections of N-butyl-2-cyanoacrylate mixtures into large gastric varices in a single patient led to two complications: initially, pulmonary embolism, and later local ulceration of the wall of a varix. The latter resulted in massive uncontrollable hemorrhage that ultimately led to a fatal outcome. This case report also analyzes complications reported in the literature during similar endoscopic procedures for gastric varices.
