Endolymphatic sac tumors in von Hippel-Lindau disease: report of three cases.

OBJECTIVES: To illustrate the diagnostic and pathophysiologic issues of endolymphatic sac tumors (ELSTs) and its clinical association with von Hippel-Lindau (VHL) disease and to demonstrate the interest of genetic testing in such cases. STUDY DESIGN: Retrospective analysis of 3 cases of ELST with VHL in 3 different clinical and prognostic situations. MAIN OUTCOMES: The first case was diagnosed on the concomitant finding of a central nervous system lesion. The familial genetic testing revealed the presence of the VHL mutation in an asymptomatic daughter, which justified regular follow-up. In

[Endolymphatic sac tumor and von Hippel-Lindau disease. Review of the literature].

This report describes a patient with Von Hippel-Lindau disease revealed by an endolymphatic sac tumor. Endolymphatic sac tumor (EST) was only recently recognized as a manifestation of Von Hippel-Lindau (VHL) disease. EST are vascular lesions that destroy and expand bone. We report a recently treated case of an EST. A 30-year-old woman presented with otalgia and hearing loss. Computed tomography and magnetic resonance imaging showed typical features of an EST. We checked for VHL and found this disease in the patient. VHL disease is a hereditary cancer syndrome caused by germline mutations of th

[Endolymphatic sac drainage and steroid-instillation surgery (EDSS) for intractable Meniere's disease].

Endolymphatic sac surgery is one of the most widely accepted techniques used to treat intractable Meniere's disease. To improve this surgery, we developed the following techniques: A simple mastoidectomy was used to expose the endolymphatic sac between the sigmoid sinus and inferior margin of the posterior semicircular canal. The sac was opened and filled with a mass of prednisolone. A bundle of absorbable gelatin films was then inserted into the sac lumen to expand it, followed by gelatin sponges dipped in a high concentration of dexamethasone. Long-term results (17-32 months) in 20 patients

Long-term efficacy of endolymphatic sac surgery for vertigo in Menière's disease.

The long-term efficacy of endolymphatic sac procedures for control of vertigo in Menière's disease has been controversial. We evaluated results of sac shunt surgery for 234 patients having at least 10 years followup (mean, 13.5 years). All patients had persistent vestibular symptoms despite medical therapy. All underwent endolymphatic subarachnoid shunt as their original operation. Data were collected by chart review and questionnaire regarding: (1) the number of additional surgical procedures to control vertigo, (2) remaining dizziness, and (3) level of disability. One hundred forty-seven of

Endolymphatic hydrops in guinea pigs after cauterizing the sac with silver nitrate.

There have been several reports of perisaccular fibrosis of the endolymphatic sac in some autopsied cases of Meniere's disease. We attempted to initiate perisaccular fibrosis of the sac in guinea pigs by injecting a small amount of 10% silver nitrate solution that cauterizes the tissue readily to produce scar, ie, fibrosis. Ninety-nine guinea pigs out of 108 (92%) that were treated with silver nitrate showed the endolymphatic hydrops in varied extent, from slight to profound. The extent of hydrops might depend on the extent of destruction of the sac following the cauterization. The lumen of th

Endolymphatic sac surgery for Meniere's disease: a composite study of 339 cases.

This paper analyzes 339 consecutively performed endolymphatic mastoid sac shunts. Regardless of the method used, vertigo was controlled and hearing either improved or stabilized in approximately 70% of the cases. Overall, vertigo was controlled in approximately 80% of the cases. Similar results were obtained in cases of Meniere's syndrome and vestibular Meniere's disease. It is suggested here that, regardless of the method employed, there remains a high likelihood that the symptoms will recur, and the question is raised as to whether sac surgery may merely induce a temporary remission of the d

Unidirectional inner ear valve implant for endolymphatic sac surgery in Meniere's disease.

In five patients with long-standing Meniere's disease, a unidirectional inner ear valve was implanted into the endolymphatic sac (ELS) to facilitate removal of excess endolymph in the hydropic state. These patients were all candidates for ablative surgery. They all had intractable vertigo and severe hearing losses. Four out of five patients had positive preoperative glycerol tests. In the patients with positive glycerol tests, the group averages before surgery were as follows: pure-tone average (PTA), 63 dB; speech reception threshold (SRT), 79 dB; and discrimination score, 27%. All of the pat

Prevalence of the endolymphatic sac hypoplasia in a cohort of individuals without Meniere disease.

PurposeTo estimate the prevalence of endolymphatic sac hypoplasia (EShp)-a proposed specific finding in Ménière's disease (MD) that defines an endophenotype characterized by bilateral involvement, male predominance, temporal bone abnormalities, and familial clustering-in individuals without MD, to assess its specificity for the condition.MethodsWe analyzed 956 temporal bone CT scans from individuals without MD to assess the prevalence of EShp using the Angular Trajectory of the Vestibular Aqueduct (ATVA) marker. ATVA distribution, reproducibility, and associations with clinical variables were

Efficacy of Resection of Lateral Wall of Endolymphatic Sac for Treatment of Meniere's Disease.

BACKGROUND: To explore the long-term efficacy and safety of resection of the lateral wall of the endolymphatic sac for the treatment of intractable Meniere's disease (MD) as an alternative surgical procedure for treating this disorder. METHODS: Data from 73 patients who were referred to our hospital and diagnosed with unilateral MD between January 2015 and June 2019 were retrospectively analyzed in this study. Seventy-three patients who had frequent vertigo even after receiving standardized conservative treatment for at least half a year underwent resection of the lateral wall of the endolymp

Endolymphatic sac tumor at the cerebellopontine angle: A case report and review of literature.

INTRODUCTION: Endolymphatic sac tumors may present as sporadic or may be associated with Von Hippel-Lindau disease. Patients generally present with hearing loss, tinnitus and vertigo. The tumor is highly vascular which may lead to erosion of the adjacent bony and vascular structures, resulting in heavy bleeding during surgery. PRESENTATION: A twenty-five year-old female presented with a five year history of chronic ear discharge, left sided facial weakness, and recent onset of ataxia. DISCUSSION: The unusual clinical presentation made management challenging, in large part due to profuse blee

[Historical evolution and frontier of endolymphatic sac surgery for treatment of Ménière's disease].

Surgical treatment could be necessary for patients with intractable Ménière's disease.Endolymphatic sac surgery is a favorable option as it could preserve patient's hearing and vestibular function. The overall vertigo control rate after the surgery was about 80%.Endolymphatic sac surgery includes several variations, namely sac shunting, sac drainage, and sac decompression.Although their establishment and development have experienced a long tortuous process, the theoretical basis and clinical efficacy of the surgery remain controversial.Meanwhile, new findings and surgical innovation keep emerging.Th

Hemorrhage in the endolymphatic sac: a cause of hearing fluctuation in enlarged vestibular aqueduct.

OBJECTIVE: Most of the patients with enlarged vestibular aqueduct (EVA) experience sudden hearing deterioration, but the exact mechanism is unclear. We analyzed magnetic resonance (MR) images and the cellular components of endolymph obtained from the endolymphatic sac in patients with EVA, in order to demonstrate the cause of sudden hearing loss. METHODS: A total of 25 patients (50 ears) with EVA, who had severe to profound hearing loss, were included in this retrospective clinical study. MR examinations were performed by a 3.0-T MR system using an 8-channel sensitivity-encoding head coil. We

Expression and translocation of aquaporin-2 in the endolymphatic sac in patients with Meniere's disease.

Meniere's disease, characterised by episodic vertigo, fluctuating hearing loss and tinnitus, can occur under conditions of stress. Its pathology was first revealed to be inner ear hydrops through temporal bone studies in 1938. Although its pathogenesis has been proposed to be a disorder of water transport in the inner ear, subsequently, it remains unsolved, until now. A recent study revealed that both plasma stress hormone, vasopressin (pAVP) and its receptor, V2 (V2R) expression in the inner ear endolymphatic sac were significantly higher in Meniere's patients. In the present study, to link V

Stereocilia-like structures in the endolymphatic sac in Ménière's disease and acoustic neuroma.

The vestibular aqueduct was surgically removed in 3 patients undergoing labyrinthectomy due to severe Ménière's disease (MD). Stereocilia-like structures were found in the luminal contents of the endolymphatic sac (ES) in all of these patients. The ES from 18 patients with acoustic neuroma were used as controls. In 1 of these, numerous stereocilia-like structures were found in the ES and in 3 additional patients, a few isolated cilia-like structures were disclosed. The findings may suggest an ongoing hair cell degeneration in the inner ear that is more advanced in patients with MD. The data al

The significance of the human endolymphatic sac and its possible role in Menière's disease.

Modern immunohistochemical methods allow a functional characterization of the human endolymphatic sac (ES) and its associated cell populations. The currently available immunohistochemical data of the extraosseous part of the human ES support the assumption that the epithelium is metabolically active and capable of both secretion and adsorption. The reactivity of the epithelial cells with antibodies against neuroendocrine antigens imply a paracrine activity of the human ES. Further results provide evidence for a possible role of the human ES in inner ear immune defence and indicate a putative f

Vestibular disorders following immune response of the endolymphatic sac in the guinea pig.

The effect of a direct antigen challenge to the endolymphatic sac on vestibular function was investigated in guinea pigs. Following keyhole limpet hemocyanin (KLH) challenge to the sac in systemically presensitized guinea pigs, caloric responses were examined in 18 animals on days 1, 7, 14, 21, and 28. Caloric responses were significantly suppressed in 13 animals by day 7; of these, 5 animals had recovered by day 14 and 8 animals had not yet recovered by day 28. The behavior of spontaneous nystagmus was examined every hour in 10 animals at intervals of 3 to 56 hours after sac challenge. Irrita
