Necrotizing enterocolitis and its association with the neonatal abstinence syndrome.

OBJECTIVE: The purpose of this study was to describe an identified association between necrotizing enterocolitis (NEC) and prenatal opioid exposure with neonatal abstinence syndrome (NAS) in late preterm and full-term neonates. STUDY DESIGN: In this single-center retrospective cohort study, we analyzed inborn neonates with the diagnosis of NEC discharged from 2012 through 2017. We compared infants with NEC > 35 weeks' gestation to those with NEC<35 weeks' gestation. We compared gestational age, birth weight, age of onset of symptoms, and incidence of prenatal drug exposure between groups. Sig

Enteral lactoferrin for the treatment of sepsis and necrotizing enterocolitis in neonates.

BACKGROUND: Neonatal sepsis and necrotizing enterocolitis (NEC) cause significant neonatal mortality and morbidity despite appropriate antibiotic therapy. Enhancing host defense and modulating inflammation by using lactoferrin as an adjunct to antibiotics in the treatment of sepsis, NEC, or both, may improve clinical outcomes. OBJECTIVES: The primary objective was to assess safety and efficacy of oral lactoferrin as an adjunct to antibiotics in the treatment of neonates with suspected or confirmed sepsis, NEC, or both. SEARCH METHODS: We used the standard search strategy of Cochrane Neonatal

Bowel Ultrasound for the Diagnosis of Necrotizing Enterocolitis: A Meta-analysis.

Radiographic evaluation for necrotizing enterocolitis (NEC) often yields nonspecific findings. Bowel ultrasound (BUS) provides additional information beyond that of abdominal radiographs and may be helpful in the diagnosis of NEC in neonates. We systematically reviewed and aggregated existing literature to get a better estimate of diagnostic accuracy of BUS in the diagnosis of NEC. A literature search was performed using PubMed, Embase, and Cumulative Index to Nursing and Allied Health Literature to identify studies in which infants with clinically suspected NEC were evaluated using BUS. Protective effects of amniotic fluid in the setting of necrotizing enterocolitis.

Necrotizing enterocolitis (NEC) is the most common life threatening condition affecting preterm infants. NEC occurs in 1-5% of all neonatal intensive care admissions and 5-10% of very low birth weight infants. The protective role of human breast milk (BM) has been well established. It has also been shown that amniotic fluid (AF) and BM have many similarities in terms of presence of growth and other immune-modulatory factors. This finding led to the initial hypothesis that AF may exert similar protective effects against the development of NEC, as does BM. Multiple studies have elucidated the pr

Maternal pre-eclampsia as a risk factor for necrotizing enterocolitis.

OBJECTIVE: Similar pro-inflammatory responses are present in pre-eclampsia (PE) and necrotizing enterocolitis (NEC). We hypothesized that maternal PE is an independent risk factor for the development of NEC. METHODS: A retrospective database of all live births (2008-2011) at a tertiary center was constructed. Infant and maternal characteristics were gathered. Babies born to mothers with or without PE were compared. Data were analyzed using Mann-Whitney U, Pearson's χ(2), binary logistic regression and relative risks. RESULTS: Incidence of NEC was 1.5% in non-PE and 4.6% in the PE group (p <

Primary laparotomy is effective and safe in the treatment of necrotizing enterocolitis.

INTRODUCTION: There is ongoing controversy regarding the surgical treatment of necrotizing enterocolitis (NEC) in infants with extremely low birth weight (ELBW). We hypothesize that primary laparotomy is a safe and effective treatment for all infants with surgical NEC. METHODS: We retrospectively compared a group of ELBW infants (<1,000 g, group A, n = 39) with preterm infants (≥1,000 g, group B, n = 18) with surgical NEC during a 4-year period (10/2008-09/2012). Indications for emergency operation were Bell stages IIIa or IIIb. The main outcome measure was survival. Data were analyzed using

[Role of laparoscopy in premature infants with suspicion of necrotizing enterocolitis].

INTRODUCTION: The indication of surgery in necrotizing enterocolitis (NEC) can be difficult in the absence of pneumoperitoneum or peritonitis. We propose laparoscopy for early diagnosis of intestinal perforation or necrosis in order to avoid unneccessary laparotomies. MATERIAL AND METHODS: A new protocol was proposed which included diagnostic and therapeutic laparoscopy in cases of uncertain surgical ECN indication. Subsequently, a prospective study of the 7 patients who underwent laparoscopy was performed, analyzing pathophysiological effects in the premature, technical difficulties of the p

Necrotizing enterocolitis in a term infant with coarctation of the aorta complex.

Necrotizing enterocolitis (NEC) sometimes occurs in term infants with congenital heart disease. This article reports a rare case of a term infant with coarctation of the aorta complex who developed NEC on the 8th day after birth. Spontaneous closure of the ductus arteriosus in the 1st week of life may cause intestinal ischemia and hypoxia with resultant NEC.

Cocaine exposure in a term neonate. Necrotizing enterocolitis as a complication.

Cocaine use has increased dramatically in the past several years, and multiple medical complications associated with its use have been reported in adults, including bowel infarction and colitis. Maternal use of cocaine during pregnancy has been associated with complications in the fetus and newborn infant, including spontaneous abortions, preterm labor, cerebral infarctions, seizures, renal anomalies, and neurobehavioral and neurophysiologic abnormalities. This paper presents a case of necrotizing enterocolitis at birth in a term newborn exposed to cocaine antenatally. Cultures of the bowel gr

[Treatment and results in necrotizing enterocolitis in infancy (author's transl)].

29 children aged between 1 day and 22 months were treated for enteritis with complications. 2 children were cured conservatively. In 27 cases surgery was needed; of these 13 children survived. In 19 patients bacteria were found in the stools. Operation was indicated when there was diffuse peritonitis or when perforation could be shown radiologically. In cases of intestinal perforation the following technique was used in the last few years: 1. resection of the segment involved and ileoor colostomy. 2. Anastomosis and closure of colostomy 4--6 weeks later. Among 29 children with necrotizing ente

ABO Incongruency Increases Risk for Necrotizing Enterocolitis in Preterm Neonates.

INTRODUCTION: Preterm neonates are at high risk for complications, and inflammation has been implicated in many of these pathologies. ABO incongruency occurs when the maternal and neonatal blood types are different and may cause complications when the mother creates antibodies against neonatal blood type antigens. In this study, we investigated whether preterm neonates with type A or B blood born to mothers with type O blood (ABO incongruent) had higher rates of neonatal complications than neonates with type O blood born to mothers with type O blood (ABO congruent). We hypothesized that preter

Neutrophils aggravate inflammatory lesions in intestinal organoids from necrotizing enterocolitis.

INTRODUCTION: Necrotizing enterocolitis (NEC) is a leading cause of neonatal death and long-term morbidity, involving complex pathophysiology including prematurity, abnormal bacterial colonization, and ischemia-reperfusion injury, partially mediated by neutrophils. However, the limitations of current animal models hinder the development of targeted therapies for NEC. Thus, this study aimed to develop a human intestinal organoid model for NEC to investigate its pathophysiology, understand neutrophil involvement, and bridge animal and human research. METHODS: Organoid cultures were established

Gut-Brain Axis in Preterm Infants with Surgical Necrotizing Enterocolitis.

Necrotizing enterocolitis (NEC) affects 5 to 10% of very low-birth-weight infants and remains a leading cause of mortality and long-term morbidity. Preterm infants with NEC, especially those requiring surgery, have higher inflammatory markers in the blood, severe white matter abnormalities on brain imaging, and adverse neurodevelopmental outcomes. This review presents current evidence regarding the clinical factors associated with brain injury in preterm infants with NEC needing surgical intervention. Studies that evaluate neuroprotective strategies to prevent brain injury are greatly needed t

Intestinal stem cell-derived extracellular vesicles ameliorate necrotizing enterocolitis injury.

The therapeutic potential of intestinal stem cell-derived extracellular vesicles (ISCs-EVs) in necrotizing enterocolitis (NEC) remains largely unexplored. This research aims to investigate the therapeutic effects of ISCs-EVs on NEC. Lgr5-positive ISCs were screened from the small intestine of mice by flow cytometry, and ISCs-EVs were isolated by density gradient centrifugation. Subsequently, ISCs-EVs were identified through transmission electron microscopy, nanoparticle tracking analysis, and western blotting. Subsequently, we evaluated the efficacy of ISCs-EVs in a mouse model of NEC and foun

Artificial Intelligence vs. Doctors: Diagnosing Necrotizing Enterocolitis on Abdominal Radiographs.

BACKGROUND: Radiographic diagnosis of necrotizing enterocolitis (NEC) is challenging. Deep learning models may improve accuracy by recognizing subtle imaging patterns. We hypothesized it would perform with comparable accuracy to that of senior surgical residents. METHODS: This cohort study compiled 494 anteroposterior neonatal abdominal radiographs (214 images NEC, 280 other) and randomly divided them into training, validation, and test sets. Transfer learning was utilized to fine-tune a ResNet-50 deep convolutional neural network (DCNN) pre-trained on ImageNet. Gradient-weighted Class Activa

Acquired intra abdominal testis due to adhesions after necrotizing enterocolitis.

Undescendent testis happens usually due to the stagnation during testicular descends. Stagnation of a testicle in the abdomen may be the target of adhesions with intestinal segments. Our case report a very rare variant of acquired intra-abdominal cryptorchidism due to adhesions after necrotizing enterocolitis. Newborn baby after NEC has a significant risk of developing intraperitoneal adhesions. In this report we will describe a case of previously palpable testis in the inguinal canal at neonatal age which was pulled inside the abdomen at seventh month of age through adhesions between test and

Human placental-derived stem cell therapy ameliorates experimental necrotizing enterocolitis.

Necrotizing enterocolitis (NEC), a life-threatening intestinal disease, is becoming a larger proportionate cause of morbidity and mortality in premature infants. To date, therapeutic options remain elusive. Based on recent cell therapy studies, we investigated the effect of a human placental-derived stem cell (hPSC) therapy on intestinal damage in an experimental NEC rat pup model. NEC was induced in newborn Sprague-Dawley rat pups for 4 days via formula feeding, hypoxia, and LPS. NEC pups received intraperitoneal (ip) injections of either saline or hPSC (NEC-hPSC) at 32 and 56 h into NEC indu

Dual energy CT in necrotizing enterocolitis; a novel diagnostic approach.

BACKGROUND: Necrotizing enterocolitis (NEC) is one of the most important causes of morbidity and mortality in premature infants. Although there are no specific diagnostic tools, the main factors affecting prognosis are clinical and laboratory findings, and early diagnosis and treatment. In this study, we demonstrate the importance of dual-energy computed tomography (DECT) in confirming intestinal ischemia in neonates with NEC. METHODS: Patients diagnosed with NEC in a neonatal intensive care unit were staged according to modified Bell's classification, and DECT was performed on patients with
