PURPOSE OF REVIEW: The true exanthematous reactions to drugs in children are rare and overdiagnosed. Drugs may cause exanthema either due to inducing an immune response or because of their interaction with viruses, if drugs are taken in the course of a viral infection. Also, viral infections are very often in children and they can provoke an exanthema which is difficult to differentiate from exanthematous reactions to drugs. Consequently, the majority of children are incorrectly labeled as 'allergic'.

We report on a 74-year-old female patient with psoriasis vulgaris who, under PUVA therapy, developed an exanthema with clinical and histological signs of systemic lupus erythematosus. Antibodies to Ro/SSA antigens were not detectable before the onset of the lupus exanthema but showed a high titer afterwards. At that time, we found the typical serological constellation of subacute cutaneous lupus erythematosus.

Neonatal toxic shock syndrome (TSS)-like exanthematous disease is characterized by exanthema, thrombocytopenia and fever in neonates infected with TSS toxin-1 producing Staphylococcus aureus . Although the disease is rare, it should be known to neonatologists as it represents a differential diagnosis in neonates with exanthema and thrombocytopenia. Two presented neonates with Neonatal TSS-like exanthematous disease are rare European cases of this specific neonatal disease.

An account of the incidence and features of fever, angina, adenopathy and splenomegaly in infectious mononucleosis is followed by an explanation of the importance of palpebral oedema, nasal obstruction, and exanthema and enanthema, the characteristics of which may prove of diagnostic assistance. Attention is drawn to the presence of maculopapular and itching exanthema, particularly after semi-synthetic penicillins. An assessment is also made of liver, myocardial and renal changes, since it is felt that involvement of these organs is an integral part of the clinical picture.

Acute hearing loss in an early stage of syphilis was frequently seen in the 19th century, but is very rare nowadays. One historical and one recent case of untreated syphilis are reported. Based on studies of the diary of the Czech composer Fr. Smetana the history of his disease is presented. Following an angina, which had lasted for several weeks, a profuse exanthema developed about 16 weeks after the primary infection. The usefulness of patch testing in the diagnosis of carbamazepine-induced allergic skin eruptions was studied in 18 patients with previous histories of skin eruptions caused by carbamazepine. The etiological role of carbamazepine was ascertained by peroral or topical provocation in 15 (out of 18) patients. We report a 27 years old homosexual male with AIDS that was admitted to the ICU dehydrated, with fever and severe malaise. He had irregular bullae, an extensive purpuric exanthema and a zone of epidermic detachment in the right arm. A toxic epidermal necrolysis was diagnosed and therapy with i.v. immunoglobulins was started. After four days of treatment, bullous lesions disappeared and the extension of exanthema decreased. Toxic epidermal necrolysis is a potentially fatal disease and the use of intravenous immunoglobulins for this condition has been reported as successful.

As many as 95 patients with generalized form of yersiniosis and 66 patients with scarlatiniform pseudotuberculosis were examined. Atypical mononuclears were detected in the peripheral blood of 23.2% of patients with yersiniosis and 13.6% of those with pseudotuberculosis.
