Specific and social fears in children and adolescents: separating normative fears from problem indicators and phobias.

OBJECTIVE:: To distinguish normative fears from problematic fears and phobias. METHODS:: We investigated 2,512 children and adolescents from a large community school-based study, the High Risk Study for Psychiatric Disorders. Parent reports of 18 fears and psychiatric diagnosis were investigated. We used two analytical approaches: confirmatory factor analysis (CFA)/item response theory (IRT) and nonparametric receiver operating characteristic (ROC) curve. RESULTS:: According to IRT and ROC analyses, social fears are more likely to indicate problems and phobias than specific fears. Most speci…

Conquer fear: protocol of a randomised controlled trial of a psychological intervention to reduce fear of cancer recurrence.

BACKGROUND: Up to 70% of cancer survivors report clinically significant levels of fear of cancer recurrence (FCR). Despite the known negative impact of FCR on psychological wellbeing and quality of life, little research has investigated interventions for high FCR. Our team has developed and piloted a novel intervention (Conquer Fear) based on the Self-Regulatory Executive Function Model and Relational Frame Theory and is evaluating Conquer Fear in a randomised controlled trial (RCT). We aim to compare the efficacy and cost-efficacy of the Conquer Fear Intervention and relaxation training in re…

Mechanism of change in cognitive-behavioral treatment of panic disorder: evidence for the fear of fear mediational hypothesis.

Numerous clinical trials have demonstrated the efficacy of cognitive-behavioral treatment (CBT) for panic disorder. However, studies investigating the mechanisms responsible for improvement with CBT are lacking. The authors used regression analyses outlined by R. M. Baron and D. A. Kenny (1986) to test whether a reduction in fear of fear (FOF) underlies improvement resulting from CBT. Pre- and posttreatment measures were collected from 90 CBT-treated patients and 40 wait-list control participants. Overall, treatment accounted for 31% of the variance in symptom reduction. The potency of FOF as …

The assessment of contemporary fears in adolescents using a modified version of the Fear Survey Schedule for Children-Revised.

The FSSC-Hawaii (FSSC-HI; Journal of Anxiety Disorders, 12, 437-461) is a modified version of the Fear Survey Schedule for Children-Revised (FSSC-R; Behaviour Research and Therapy, 21, 685-692) that includes a number of contemporary fear stimuli and situations (e.g., "drugs", "being raped", "AIDS"). The psychometric properties of the FSSC-HI were examined in a large sample of Belgium adolescents (n = 551) aged 12-19 years. Results showed that a five- and seven-factor model both provided satisfactory fits for the structure of the FSSC-HI. Furthermore, the internal consistency of the scale was g…

Platelets tune fear memory in mice.

Several lines of evidence have shown that platelet-derived factors are key molecules in brain-body communication in pathological conditions. Here, we identify platelets as key actors in the modulation of fear behaviors in mice through the control of inhibitory neurotransmission and plasticity in the hippocampus. Interfering with platelet number or activation reduces hippocampal serotonin (5-HT) and modulates fear learning and memory in mice, and this effect is reversed by serotonin replacement by serotonin precursor (5-HTP)/benserazide. In addition, we unravel that natural killer (NK) cells pa…

Reconsidering fear of birth: Language matters.

Interest in fear of childbirth has grown exponentially since the 1980s, but the landscape of birth has shifted considerably since then, with evolving feminism; moving from a patriarchal environment in a biomedical model of care to a holistic model which recognizes the birth and sexuality rights of women and birthing people. Distinguishing the spectrum of fear from low to high and severe is important rather than aggregating all individuals with fear of childbirth. However, the terms 'fear of childbirth' and 'tocophobia' have been used interchangeably. In this paper we urge clinicians to use the…

Glucocorticoids reduce phobic fear in humans.

Phobias are characterized by excessive fear, cued by the presence or anticipation of a fearful situation. Whereas it is well established that glucocorticoids are released in fearful situations, it is not known whether these hormones, in turn, modulate perceived fear. As extensive evidence indicates that elevated glucocorticoid levels impair the retrieval of emotionally arousing information, they might also inhibit retrieval of fear memory associated with phobia and, thereby, reduce phobic fear. Here, we investigated whether acutely administrated glucocorticoids reduced phobic fear in two doubl…

Mesolimbic dopaminergic pathways in fear conditioning.

One of the most common paradigms used to study the biological basis of emotion, as well as of learning and memory, is Pavlovian fear conditioning. In the acquisition phase of a fear conditioning experiment, an emotionally neutral conditioned stimulus (CS)--which can either be a discrete stimulus, such as a tone, or a contextual stimulus, such as a specific environment--is paired with an aversive unconditioned stimulus (US), for example a foot shock. As a result, the CS elicits conditioned fear responses when subsequently presented alone during the expression phase of the experiment. While cons…

Modality-specific retrograde amnesia of fear.

Emotional responses such as fear are rapidly acquired through classical conditioning. This report examines the neural substrate underlying memory of acquired fear. Rats were classically conditioned to fear both tone and context through the use of aversive foot shocks. Lesions were made in the hippocampus either 1, 7, 14, or 28 days after training. Contextual fear was abolished in the rats that received lesions 1 day after fear conditioning. However, rats for which the interval between learning and hippocampal lesions was longer retained significant contextual fear memory. In the same animals, …

Lack of fear acquisition to film clip unconditional stimuli despite contingency awareness: Implications for electrodermal fear conditioning.

Fear conditioning is most commonly assessed by measuring electrodermal activity to a conditioned stimulus (CS) in anticipation of an electric shock or another unconditioned stimulus (US). Replacing the electric shock with aversive film clips is an innovation that potentially has higher ecological validity for posttraumatic stress disorder. The present report examined boundary conditions of this paradigm across two between subjects experiments, with a particular focus on the effectiveness of film clip versus electric shock USs in promoting physiological fear conditioning. In Experiment 1, we te…

Fear learning in unmedicated patients with anxiety disorders: a comparison of delay conditioning, fear reversal, and trace conditioning.

Anxiety disorders are common and impairing, yet their underlying mechanisms remain incompletely understood. Fear learning provides a critical translational framework for investigating pathological anxiety, bridging laboratory models and clinical phenomena. Prior studies have been limited by important methodological issues, including the inclusion of non-anxiety diagnoses, high comorbidity, and medication use. Here we examined three forms of fear learning- delay conditioning, fear reversal, and trace conditioning-in unmedicated adults with minimally comorbid primary anxiety disorders (Generaliz…

Psychopathic personality traits are associated with experimentally induced approach and appraisal of fear-evoking stimuli indicating fear enjoyment.

The extent to which deficits in the perception and experience of fear contribute to psychopathic symptoms is an ongoing matter of debate. Traditional theories emphasize diminished threat processing as the core fear deficit in psychopathic individuals, whereas recent approaches, such as the fear enjoyment hypothesis (FEH), propose that anomalies in the subjective experience of fear are related to interpersonal-affective psychopathic traits. In order to test predictions of the FEH, we conducted an online experimental study to examine whether approach and avoidance tendencies in response to fear-…

Using unconditioned responses to predict fear acquisition, fear extinction learning, and extinction retention patterns: Sex hormone status matters.

Following a traumatic event, fear dysregulation can increase the likelihood of developing post-traumatic stress disorder (PTSD). This psychopathology is twice as prevalent in women than in men. High physiological reactivity following trauma may be an early risk indicator for the development of PTSD. Elevated physiological reactivity and low estradiol levels have individually been associated with higher fear acquisition and/or lower extinction retention. Thus, sex hormone status may also modulate fear regulation abilities. However, it is unknown whether these two vulnerability factors interact …

Assessment of Pain-Related Fear in Indigenous Australian Populations Using the Fear of Pain Questionnaire-9 (FPQ-9).

In this study, we examined the psychometric properties of the Fear of Pain Questionnaire (FPQ-9) in Indigenous Australian people. FPQ-9, a shorter version of the original Fear of Pain Questionnaire-III, was developed to support the demand for more concise scales with faster administration time in the clinical and research setting. The psychometric properties of FPQ-9 in Indigenous Australian participants (n = 735) were evaluated with network psychometrics, such as dimensionality, model fit, internal consistency and reliability, measurement invariance, and criterion validity. Our findings indic…

D-cycloserine as adjunct to brief computerised CBT for spider fear: Effects on fear, behaviour, and cognitive biases.

BACKGROUND AND OBJECTIVES: In anxiety disorders, cognitive behavioural therapy (CBT) improves information-processing biases such as implicit fear evaluations and avoidance tendencies, which predicts treatment response. Thus, these cognitive biases might constitute important treatment targets. This study investigated (i) whether information-processing biases could be changed following single-session computerised CBT for spider fear, and (ii) whether this effect could be augmented by administration of D-cycloserine (DCS). METHODS: Spider-fearful individuals were randomized to receiving either 2…

Symbolic online exposure for spider fear: habituation of fear, disgust and physiological arousal and predictors of symptom improvement.

BACKGROUND AND OBJECTIVES: This research compared the effects of real versus hyper-real images on anxiety, disgust, and physiological arousal during internet-delivered exposure in high spider-fearfuls. Hyper-real images were digitally altered to highlight fearful aspects. A further aim was to examine self-reported and behavioural therapeutic outcomes and exposure-related predictors of these outcomes. METHODS: Twenty-eight females were randomised to real (n = 14) or hyper-real (n = 14) treatment groups and nine participants were subsequently allocated to a wait-list control group. Treatment gr…
