The Hip Abduction Maneuver (HAM) to diagnose meralgia paraesthetica.

BACKGROUND: Meralgia paresthetica (MP) is a compressive neuropathy of the lateral femoral cutaneous nerve (LFCN), characterized by pain, paresthesia, and numbness in the anterolateral thigh. Despite its well-documented etiology, diagnosing MP remains challenging, as complementary tests such as electromyography (EMG) and ultrasound may yield normal results due to anatomical variations and the dynamic nature of nerve compression. OBJECTIVE: This study introduces the Hip Abduction Maneuver (HAM) as a novel diagnostic tool for LFCN compression and investigates its utility in clinical and intraope

Reconstruction of Quadriceps Function Using a Single Functional Gracilis Muscle Transfer With an Adductor Longus Nerve to Femoral Nerve Branch of the Rectus Femoris Nerve Transfer.

BACKGROUND: A femoral nerve injury may result in cutaneous sensory disturbances of the anteromedial thigh and complete paralysis of the quadriceps femoris muscles resulting in an inability to extend the knee. The traditional mainstay of treatment for femoral neuropathy is early physiotherapy, knee support devices, and pain control. Case reports have used the anterior division of the obturator nerve as a donor nerve to innervate the quadriceps femoris muscles; however, a second nerve transfer or nerve grafting is often required for improved outcomes. We suggest a novel technique of combining an…

Sonographic evaluation of the lateral femoral cutaneous nerve in meralgia paresthetica.

OBJECTIVE: Identify sonographic features of the lateral femoral cutaneous nerve (LFCN) in meralgia paresthetica (MP) and report therapeutic outcomes in sonographically confirmed cases. MATERIALS AND METHODS: Retrospective review of 50 patients with clinically suspected MP and 20 controls. Ultrasounds were reviewed for characteristics of the LFCN and compared between groups. When available, MRIs were reviewed. In cases of sonographically pathologic LFCN, subsequent therapeutic interventions were recorded. RESULTS: Thirty-five of the suspected MP cases (70%) had ultrasound findings suggestive

Anatomy of the lateral femoral cutaneous nerve relevant to clinical findings in meralgia paresthetica.

INTRODUCTION: Compression of the lateral femoral cutaneous nerve (LFCN), known as meralgia paresthetica (MP), is common. We investigated the topographic anatomy of the LFCN focusing on the inguinal ligament and adjacent structures. METHODS: Distances from various bony and soft-tissue landmarks to the LFCN were investigated in 33 formalin-embalmed cadavers. RESULTS: The mean distance from the anterior superior iliac spine (ASIS) to the LFCN was 8.8 mm. In approximately 90% of cases, the LFCN lay <2 cm from the medial tip of the ASIS, whereas, in 76% of cases, it was <1 cm away. The mean angle

Gastrointestinal Mucormycosis After Orthotopic Liver Transplantation Presenting as Femoral Nerve Palsy: A Case Report and Review of the Literature.

Mucormycosis has emerged as a major threat to transplant recipients with high morbidity and mortality. This infection most commonly presents with rhino-sino-orbital localization. Gastrointestinal mucormycosis is uncommon, with presenting symptoms usually abdominal in nature. Here, we describe the case of a liver transplant recipient who developed gastrointestinal mucormycosis with an initial manifestation of femoral nerve palsy, ultimately resulting in fungal dissemination and patient demise. This case highlights the challenges in making a timely diagnosis of mucormycosis, particularly in immu

Variable origin and ramification pattern of the lateral femoral cutaneous nerve: a case report and neurosurgical considerations.

Variations in the anatomy of the lateral femoral cutaneous nerve (LFCN) have been reported in the literature. LFCN is vulnerable to injury during several surgical operations, therefore any surgeon intervening in the area should be familiar to its topographic variability. Lesion of the nerve leads to a condition known as "meralgia paresthetica". We present a cadaveric case of a variant LFCN where two LFCN branches were encountered arising from the lumbar plexus. In specific the anterior LFCN branch originated from the femoral nerve, whereas at the level of the inguinal ligament, four nerve bran

Popliteal Entrapment Syndrome as a Cause of Chronic Lower Extremity Pain in a 16-Year Old.

Popliteal entrapment syndrome is an uncommon cause of intermittent claudication in young patients lacking atherosclerotic risk factors. ZS is a 16-year-old cisgender female with type 1 diabetes complicated by microalbuminuria, obesity (body mass index (BMI) = 45.86 kg/m²), and a history of perinatal stroke with residual right-sided hemiparesis, who presented with six months of worsening bilateral, exertional lower extremity pain. Common causes of chronic bilateral lower extremity pain include peripheral vascular disease and diabetic neuropathy. Less common etiologies include trauma, infection,

Radioguided Occult Lesion Localization in Deep Schwannomas of the Peripheral Nerves: Results of a Preliminary Case Series.

BACKGROUND: The detection of small deep schwannomas of the peripheral nerves has been increasing since the the use of precise neuroimaging techniques has become more widespread; however, although nonpalpable lesions can be well defined by images, it is often difficult to identify them during the surgical procedure. The authors report seven cases of nonpalpable small deep schwannomas surgically treated after their identification using the radioguided occult lesion localization (ROLL) technique. METHODS: Seven men, whose ages ranged from 34 to 70 years (mean 52 years), presented with symptomati

Contained ruptured abdominal aortic aneurysm presenting as cauda equina syndrome.

This is the case report of a contained ruptured aortic aneurysm presenting with acute cauda equina syndrome. The patient was a 79-year-old man. A literature search revealed various unusual presentations of abdominal aortic aneurysm (AAA), including femoral neuropathy, hip pain and others; however, there are no other reports of cauda equina-like syndrome. The present case is therefore another unusual presentation of ruptured abdominal aortic aneurysm and reiterates the utmost importance of careful history taking and clinical examination. Abdominal aortic aneurysm rupture should always be consid…

Lower extremity neuropathies after robot-assisted laparoscopic prostatectomy on a split-leg table.

BACKGROUND AND PURPOSE: Lower extremity neuropathies from prolonged lithotomy positioning have been well documented. When we initiated our robot-assisted laparoscopic prostatectomy (RALP) program in December 2002, we chose to use the split-leg table that allows patient support in a more anatomic position, hypothesizing that this would reduce risk of neurologic compression injuries. We report our incidence of lower extremity neuropathies associated with RALP using split-leg positioning and review patient and surgical variables associated with this complication. PATIENTS AND METHODS: We retrosp

Median Nerve Mononeuropathy and Meralgia Paresthetica After Prone Positioning in a Patient with COVID-19 ARDS: A Case Report.

CASE: Throughout the COVID-19 pandemic, prone positioning has decreased mortality in patients with severe acute respiratory distress syndrome. We present the unique case of a patient who developed left median nerve mononeuropathy and bilateral meralgia paresthetica after prone positioning while afflicted with COVID-19. These nerve injuries have been rarely reported in the literature and never before in the same patient. CONCLUSION: Our case highlights the importance of increased care when positioning patients prone by padding bony prominences, evenly distributing pressure across known sites o

A rat model for acute limb ischemia using microsized gelatin beads.

OBJECTIVE: Acute limb ischemia (ALI) is a rapid decrease in lower limb blood flow due to acute occlusion of peripheral arteries or bypass grafts. This study aimed to establish an ALI model using microsized gelatin beads and to investigate the pathophysiological conditions. METHODS: Male Sprague-Dawley rats were anesthetized, and a low or high dose of microsized gelatin beads was administered into the left femoral artery on days 0 and 7. A control, that is, normal saline (NS) group in which NS was administered in the left femoral artery, a femoral artery cut (FAC) group in which the left femor

Pseudotumor-Related Compression Neuropathies Following Total Hip Arthroplasty.

BACKGROUND: Pseudotumor-related compression neuropathies following total hip arthroplasty (THA) represent a severe phenotype of adverse local tissue reaction. While historically attributed to metal-on-metal bearings, other constructs such as dual-modular necks and taper corrosion with cobalt-chrome femoral heads have been implicated as well. Furthermore, inflammatory responses to polyethylene wear debris and nonforeign body sources can similarly stimulate pseudotumor formation, resulting in symptomatic nerve compression. This study aimed to characterize the etiology, clinical presentation, and

Sympathetic reflex control of resistance in collateral arteries in the lower extremities in patients with diabetes mellitus.

The vascular response in the lower extremities to 40 degrees head-up tilt was studied in 5 patients with occlusion of the superficial femoral artery and maturity onset diabetes mellitus with symptoms suggesting autonomic neuropathy. The pressure measurements were performed via catheters placed in the brachial artery, femoral artery and vein and popliteal artery and vein. Relative blood flow was calculated as the relative change in arterio-venous oxygen saturation. Absolute blood flow in the common femoral artery was measured by an indicator dilution technique. Resistance of the collateral arte

Lateral femoral cutaneous nerve palsy following shoulder surgery in the beach chair position: a report of 4 cases.

Neuropathy of the lateral femoral cutaneous nerve can present as pain, decreased sensation, and/or burning or tingling on the anterolateral thigh. We present 4 cases of lateral femoral cutaneous nerve palsy following shoulder surgery in the beach chair position, all of which occurred in obese patients. This complication, to our knowledge, has never been reported in conjunction with the beach chair position. We believe that the neurapraxia was due to external compression by the patients' abdominal pannus. Full resolution of symptoms can be expected within 6 months following conservative managem

Femoral nerve injury after gynecologic laparoscopy.

Femoral nerve injury after gynecologic laparoscopy is reported very rarely, and usually only as a consequence of lithotomy position. A 36-year-old woman underwent left partial laparoscopic ovariectomy. A primary Hasson trocar was inserted through infraumbilical incision; 2 secondary trocars were placed in right hypocondrium and left iliac fossa, just cranially to the inguinal fold. After operation, the patient showed irreversible femoral neuropathy, with left thigh weakness and allodynic paresthesias, from the site of the left trocar insertion to the anteromedial aspect of the thigh and medial…
