Fetal seizures: further observations.

BACKGROUND: Previous reports of ultrasonographically diagnosed seizure activity in utero have described fetuses with obvious, gross, tonic-clonic movements of trunk and extremities. CASES: Three fetuses with akinesia deformation sequence are described: two with arthrogryposis multiplex congenita and one with Pena-Shokeir syndrome. Each was demonstrated by serial real-time ultrasound examinations to have joint contractures, absent fetal breathing motions, and lack of gross movements other than sonographically evident subtle seizure activity. CONCLUSION: Fetal seizure activity can be subtle wh

Fetal akinesia deformation sequence: behavioral development in a case of congenital myopathy.

OBJECTIVE: To monitor behavioral development in a fetus affected by the Pena-Shokeir phenotype due to fetal akinesia deformation sequence of myogenic origin. METHODS: Fetal behavioral development was studied sonographically in a fetus suspected of having fetal akinesia deformation sequence, between 15 and 35 weeks of gestation at 1-4-week intervals. RESULTS: Fetal body movements were quantitatively and qualitatively abnormal from 28 weeks onwards. Behavioral deterioration coincided with the observation of structural anomalies such as club feet. Fetal heart rate and its variation remained wit

Control of fetal breathing in the human fetus between 24 and 34 weeks' gestation.

The effect of induced maternal hypocapnia and hypercapnia on fetal breathing movements was studied in 30 healthy pregnant women between 24 and 34 weeks' gestation to determine whether gestational age influences the fetal respiratory response to alterations in carbon dioxide levels. The percent time of fetal breathing movements correlated significantly with maternal end-tidal PCO2, increasing with maternal breathing of 2% and 4% carbon dioxide and decreasing with maternal hyperventilation for each of the three gestational age groups studied. However, the slope of this response for the 28- to 30

Current status of fetal neurodevelopmental assessment: Four-dimensional ultrasound study.

With the latest advent of four-dimensional (4-D) ultrasound, fetal neurobehavioral or neurodevelopmental assessment can be easily and readily performed. Using this technique, typical fetal movements and behavioral patterns have become apparent in all three trimesters of pregnancy. In twin pregnancy, 4-D ultrasound facilitates the precise evaluation of inter-twin contact and intra-pair stimulation. New fetal neurobehavioral assessment tests, such as Kurjak's Antenatal Neurodevelopmental Test and the Fetal Observable Movement System, may reflect the normal and abnormal neurological development o…

Maternal buprenorphine treatment and fetal neurobehavioral development.

BACKGROUND: Gestational opioid use/misuse is escalating in the United States; however, little is understood about the fetal effects of medic … gestation. Fetal neurobehavioral indicators (ie, heart rate, motor activity, and their integration [fetal movement-fetal heart rate coupling]) were collected via an actocardiograph, digitized and quantified. Longitudinal data analysis relied on hierarchic linear modeling. RESULTS: Fetal heart rate, heart rate variability, and heart rate accelerations were signi …

Cocaine alters behavioral states in fetal sheep.

Intrauterine cocaine exposure causes subtle neurologic abnormalities in human newborn infants; however, the mechanism for these abnormalities is not known. To investigate whether cocaine alters fetal behavioral state, the electrocortical, electro-ocular and neck muscle electrical activity was monitored in 7 chronically instrumented fetal sheep before and during both saline and cocaine HCl infusions directly to the fetus. Saline infusion to the fetus had no effect on the percentage of time spent in rapid eye movement sleep compared to no infusion (37.5 +/- 11.6% vs 46.3 +/- 4.6%, mean +/- SD, P

Maternal pinealectomy alters the daily pattern of fetal breathing in sheep.

We have investigated the effect of pinealectomy of the pregnant ewe on the 24-h pattern of fetal breathing activity during late gestation. Fetal breathing movements were recorded during 24-h periods on 18 occasions in 5 pinealectomized ewes and on 24 occasions in 6 pineal-intact ewes between 120 and 145 days gestation. All ewes were fed once daily between 1000 and 1300 h and were kept under a light-dark cycle 12:12 h. There was no significant difference in the mean hourly incidence of fetal breathing movements between the pineal-intact (27.2 +/- 0.5 min/h) and pinealectomized (25.5 +/- 0.6 min

Did antepartum hypoxic insult caused by fetal vessel thrombosis influence the procalcitonin level in umbilical blood? A case report.

We report a case of marked elevation of the procalcitonin level in umbilical blood and neonatal blood at birth. The mother did not perceive fetal motion. Antepartum fetal heart rate monitoring showed a loss of variability and absence of acceleration. No fetal breathing movement, fetal movement, or fetal tone were observed by ultrasonography. The female neonate was delivered by cesarean section at 25 weeks of gestation, with birthweight 774 g. The umbilical arterial pH value at birth was 7.29. Mild elevation in interleukin-6 and tumor necrosis factor-α in umbilical blood were observed. Cytochro…

The effects of acute alcohol intoxication on biophysical activities: a case report.

A computer-assisted biophysical assessment was performed in a woman with chronic alcoholism at 37 weeks' gestation. She was first seen in a state of acute alcohol intoxication (322 mg/dl). Although fetal breathing movement incidence was normal (30%), fetal tachypnea (67 breaths per minute) and decreased fetal body movements (0.11%) were seen. Because these findings differ from those previously reported with regard to normal pregnancies exposed to lower levels of alcohol, their implications are discussed.

Fetal yawning.

Incidental fetal yawning movements can rarely be observed during real-time ultrasonographic examination. In this report we document repetitive fetal yawning movements in a 27-week fetus over a 7-min period. Each episode lasted for 4-6 s, and the intervals between them varied from 21 to 195 s. This case demonstrated that fetal yawning, a complex involuntary behavioral reflex, is present in its full extent during the second half of pregnancy.

Extremely large number of twists of the umbilical cord causing torsion and intrauterine fetal death.

Extremely large number of twists of the umbilical cord causing torsion of the entire length of the umbilical cord was found in two cases of intrauterine fetal death. It was twisted in one case 35 times and in the other 20 times. No additional pathology, such as stricture or abnormality of the Wharton's jelly, was found. The two mothers complained of decreased fetal movements and both newborns were found later to be growth retarded. A long cord of 120 cm was present in one case and normal length of 70 cm in the other. Close antenatal care in cases with growth retarded fetuses or decreased fetal…

The significance of ultrasonographically diagnosed fetal wrist position anomalies.

OBJECTIVE: Our purpose was to determine the clinical significance of ultrasonographically diagnosed fetal wrist position anomalies. STUDY DESIGN: The relationship of the fetal hand to the forearm was prospectively evaluated in all second- and third-trimester scans over a 5-year period. If an abnormal wrist position was detected, a targeted scan, including echocardiography, was performed. The outcomes of abnormal pregnancies were obtained. RESULTS: An abnormal relationship of the hand to the forearm was found in 22 fetuses in 27,467 scans. Nine had a normal karyotype, and 13 had an abnormal k

[Very severe spinal muscular atrophy--type 0. A cause of congenital multiple arthrogryposis].

A female infant born at term, with reduced fetal movements in utero, congenital multiple contractures, severe weakness at birth, and a short time of survival is described. The diagnosis was confirmed by identification of homozygous deletion of exons 7 and 8 of the SMNt gene. Severe spinal muscular atrophy should be considered in the differential diagnosis of reduced fetal movements.

Effects of diphenhydramine in the fetal lamb after maternal or fetal administration.

The effects of diphenhydramine on fetal behavioral states, breathing activity, blood gas status, arterial pressure and heart rate have been investigated in the fetal lamb after maternal or fetal drug administration to steady state in chronically instrumented pregnant sheep. During maternal drug infusion there were declines in the percentage of low voltage electrocorticographic (ECoG) pattern 55-46%), the percentage of low voltage ECoG activity containing rapid eye movements (80-55%), the overall incidence of fetal breathing (42-21%) and in the amount of breathing during low voltage ECoG activi

Fetal breathing and development of control of breathing.

Technical advances during the last several decades have greatly facilitated research into fetal physiology and behavior, specifically fetal breathing (FB). Breathing movements have been demonstrated in the fetuses of every mammalian species investigated and appear to be part of normal fetal development. In this review we focus on the methods of measuring FB and on some of the problems associated with these measurements and their interpretation. We also review fetal behavior, the role of the peripheral and central chemoreceptors in spontaneous FB, the fetal respiratory response to hypercapnia a

Pulsed-wave tissue Doppler echocardiography for the analysis of fetal cardiac arrhythmias.

OBJECTIVES: Rhythm analysis of the fetal heart is hampered by the inability to routinely obtain electrocardiographic recordings of the fetus. Doppler studies of fetal cardiac tissue movements, assessing cardiac movements both qualitatively and quantitatively, have recently been described. We used a conventional high-resolution ultrasound system to obtain rhythm data from pulsed-wave tissue Doppler signals of the fetal heart in normal cardiac rhythm and in a variety of fetal cardiac arrhythmias. METHODS: Fifty-five fetuses with normal (sinus) rhythm, 45 fetuses with rhythm disturbances and two

Spontaneous in utero recovery of a fetus in a brain death-like state.

A healthy pregnant woman at 28 weeks of gestation was admitted with complete loss of fetal movement. Fetal heart rate (FHR) monitoring showed a fixed flat FHR pattern with no variability. Ultrasound examination revealed no fetal breathing movement. The fetus showed heart failure with extremely diminished left heart contractility. No abnormalities were found in the fetus. We assessed that the fetus was in a brain death-like status. After discussion, we decided not to deliver the baby but to monitor it carefully using FHR monitoring and continuous fetal ultrasound examinations, assuming the baby…
