We herein report a case of pyelitis cystica in 65-year-old woman. She was referred to our hospital in order to have a treatment for a stone in the ureter on left side. Excretory urogram showed hydronephrosis on left, and multiple, small, smooth and round filling defects in the renal pelvis on right side. ESWL was performed to the ureteral stone, and the stone was discharged completely in 4 days. Then further examinations were made for the filling defects of right renal pelvis. We performed contact transscleral cyclophotocoagulation in two human autopsy eyes with continuous-wave Nd:YAG and diode lasers. Splenic injury was associated with a flexible fiberoptic colonoscopy and required an urgent splenectomy. Injuries to viscera other than the colon during colonoscopy are rare, and the diagnosis is frequently delayed. Excessive traction on the splenocolic ligament or on preexisting adhesions, resulting in capsular tears, is the presumed mechanism of injury. Although computed tomography may be helpful, the greatest aid to an early diagnosis is the knowledge that this complication may occur.

A case of upper gastrointestinal hemorrhage in a newborn infant due to ulcerative esophagitis is described. This is a previously undescribed cause of upper gastrointestinal neonatal bleeding. This case further illustrates the safety and efficacy of fiberoptic endoscopy in elucidating the source of upper gastrointestinal bleeding in the immediate newborn period.

We have found that performing a Meltzer-Lyon test simultaneously with upper gastrointestinal endoscopy is an efficient as well as accurate technique for diagnosing acalculous cholecystitis. For the patient with upper abdominal pain and normal contrast and sonographic studies, endoscopy is often the next logical step in diagnosis. When the results of endoscopy do not reveal a source of upper abdominal pain, one can proceed directly to a Meltzer-Lyon test. The 4-year experience with flexible fibre-optic bronchoscopy at Groote Schuur Hospital is presented. A total of 809 patients were bronchoscoped, follow-up data on 705 being available. In every case of duodenal endoscopy we collect cell material from the papilla of Vater by means of a guided brush technique. The microscopic techniques used include phase contrast study of unfixed material and Papanicolaou stained smears. Normal epithelial cells of the papillary region differ from those of the duodenal epithelium and the epithelial cells of the common bile or pancreatic ducts collected by retrograde cannulation. Recent advances in biopsy techniques using fibreoptic instruments have refocussed attention on the condition on non-specific duodenitis. Investigation of the topography of duodenitis and its relationship to possible aetiological factors and to duodenal ulcer calls for a proper histological classification. Such a classification is proposed based on the subjective appearances and a quantitative examination of 747 fibreoptic biopsy specimens in a wide range of clinical conditions.
