Chronic inflammation predicts long-term mortality in patients with Raynaud's phenomenon.

BACKGROUND: Subclinical chronic inflammation could be the driving force behind the recently revealed association between abnormal nailfold capillaries as well as autoantibodies and long-term mortality in patients with incipient Raynaud's phenomenon. Whether laboratory markers that reflect a chronic inflammatory process are directly related to mortality in Raynaud's phenomenon is not known. METHODS: In total, 2958 patients with incipient Raynaud's phenomenon without previously known connective tissue disease (CTD) were enrolled. At their initial presentation, laboratory tests for C-reactive pr

Haemostatic effects of levothyroxine and selenomethionine in euthyroid patients with Hashimoto's thyroiditis.

The aim of this prospective study was to investigate for the first time whether levothyroxine and selenomethionine, administered alone or in combination, affect coagulation and fibrinolysis in Hashimoto's thyroiditis patients with normal thyroid function tests. A group of 155 ambulatory women with recently diagnosed and previously untreated Hashimoto's thyroiditis, of whom 149 completed the study, were randomly assigned in a double-blind fashion to six months of treatment with levothyroxine, selenomethionine, levothyroxine plus selenomethionine, or placebo. The control group included 39 matche

Keratophakia and keratomileusis: histopathologic, ultrastructural, and experimental studies.

We present ultrastructural findings in two cases, one of keratophakia (9 months postoperatively) and one of keratomileusis (1 year, 7 months post-operatively), which best demonstrate the pathologic changes we observed in six cases of cryorefractive surgery. In addition, we studied a rabbit model of keratophakia which appears to exhibit similar features to that of human keratophakia. In the keratomileusis cases, there were marked abnormalities, including deposition of redundant basal lamina, loss of Bowman's membrane and deposition of a 6 nm fibrillar material in areas devoid of Bowman's membra

Post-translational modifications in the insoluble fraction of platelet-poor plasma distinguish pre-COVID-19 Postural Orthostatic Tachycardia Syndrome, Long COVID, and Long COVID-POTS: a preliminary study.

BACKGROUND: Pre-COVID-19 Postural Orthostatic Tachycardia Syndrome (PC-POTS), Long COVID with initial orthostatic hypotension (LC-IOH), and Long COVID-POTS (LC-POTS) are primarily chronic post-viral conditions with debilitating symptoms, despite routine laboratory findings often being normal. We previously identified amyloidogenic fibrinaloid microclot complexes (FMCs) in the insoluble fraction of platelet-poor plasma (PPP-IF) in Long COVID, but it remains unclear whether similar abnormalities occur in PC-POTS and LC-POTS, or whether post-translational modifications (PTMs) contribute to diseas

The use of routine laboratory testing in acute trauma care: A retrospective analysis.

BACKGROUND: In more than 60 countries worldwide, laboratory testing plays a challenging and expensive role in trauma resus-citation. In 1995, the literature already suggested that routine laboratory testing may not be useful for most trauma patients. Our study hypothesized that still the need for some laboratory tests perhaps should be reconsidered. Therefore, the aim of this study was to create more insight in the distribution between normal and abnormal parameters for routine laboratory testing in trauma patient management. METHODS: This retrospective analysis was performed at Amsterdam UMC

Relationship between progression to AIDS and thrombophilic abnormalities in HIV infection.

BACKGROUND: HIV-infected patients are at increased risk of venous and arterial thrombosis. We hypothesized that acquired thrombophilic abnormalities that could predispose to thrombosis are most pronounced in patients in advanced stages of HIV infection. METHODS: We included 109 consecutive HIV-infected patients in the study and tested them twice for currently known thrombophilic abnormalities at an interval of at least 3 months (median, 3 months; range, 3-12 months). Detailed information was collected about the date of diagnosis of HIV infection, HIV treatment, and previous episodes of venous

The Vascular and Metabolic Face of Alopecia Areata: Insights from an Indian Cohort.

BACKGROUND: Alopecia areata (AA) is a chronic, immune-mediated disorder characterized by nonscarring hair loss. Emerging evidence suggests that AA is associated with systemic inflammation, metabolic dysregulation, and coagulation abnormalities. However, data on these associations in the Indian population remain limited. AIM AND OBJECTIVES: To evaluate coagulation parameters, metabolic profiles, and inflammatory markers in Indian patients with AA and assess their correlation with disease severity. PATIENTS AND METHODS: A case-control study was conducted at a tertiary care center in North Indi

Comparison of standard coagulation testing with thromboelastometry tests in cardiac surgery.

Introduction: According to the several evidences, using thromboelastometry as a point of care test can be effective in reduction in blood loss and transfusion requirements in cardiac surgeries. However, there are limited data regarding to the comparison of thromboelastometry and the standard coagulation tests. In this study, we compared thromboelastometry and standard coagulation tests (PT, PTT and fibrinogen level) in patients under combined coronary-valve surgery. Methods: Forty adult patients who were under on-pump combined coronary-valve surgery were included in this study. Thromboelastome

Association of hemostatic variables with MRI-detected cerebral abnormalities: the atherosclerosis risk in communities study.

OBJECTIVES: Population-based and clinical prospective studies have shown independent associations of several hemostatic factors with ischemic heart disease and stroke. MRI-detected cerebral infarcts and white matter lesions are often detected in elderly individuals without clinical disease. It has been hypothesized that these types of lesions are often the consequence of cerebral ischemic damage and may be the precursors of clinical stroke. METHODS: This study examined the relation between a range of hemostatic factors measured at baseline in middle-aged participants who were free of diagnose

Blood viscosity factors and occlusive arterial disease in renal transplant recipients.

29 cadaveric renal transplant recipients were assessed clinically for evidence of occlusive arterial disease prior to undergoing blood viscosity studies. Nineteen patients had manifest arterial disease (myocardial infarction, cerebral thrombosis, angina, intermittent claudication, absent peripheral pulses), while ten were free from vascular complications. Patients with arterial disease showed significant elevations of plasma viscosity (p less than 0.005), aggregation of red cells measured both at 37 and 20 degrees C (p less than 0.05), fibrinogen (p less than 0.005), serum triglyceride (p less

Abnormal coagulation and fibrinolysis in eclampsia. A clinical and laboratory correlation study.

This study evaluated the existence of abnormally increased coagulation and fibrinolysis in 33 severely toxemic and eclamptic women by means of a combined hemotologic profile with clinical and morphologic correlations. The dominant findings were: different degrees of thrombocytopenia, abnormal levels of blood fibrinogen, prolonged thrombin time, and positive protamine sulfate test. Altered activated partial thromboplastin time and positive ethanol gelation test were slightly less frequent, and only few cases showed prolonged prothrombin time or early lysis of euglobulins. These abnormalities se

Association of fibrinogen γ' with recurrent thrombosis: a nested case-control study.

BACKGROUND: Fibrinogen γ' is a natural variant of the fibrinogen γ chain, accounting for 10% to 15% of fibrinogen in human blood. Studies have found an association between fibrinogen γ' plasma levels and the risk of first-time arterial and venous thrombosis, but how fibrinogen γ' associates with recurrent thrombosis is unknown. OBJECTIVES: To investigate the association between fibrinogen γ' levels and recurrent thrombosis in patients previously diagnosed with arterial or venous events. METHODS: We conducted a nested case-control study of 265 patients who experienced arterial or venous throm

Risk factors for coronary heart disease in diabetes mellitus.

A review of the putative risk factors associated with the development of coronary heart disease in diabetes is presented. Emphasis is given to the effect of nephropathy (persistent proteinuria) and hypertension on cardiovascular mortality in IDDM. Risk factors associated with CHD in NIDDM are also reviewed. Finally, possible reasons to explain the increased incidence of CHD associated with proteinuria in IDDM patients, including lipoprotein abnormalities, increased fibrinogen levels, increased platelet adhesiveness, and altered hemostatic variables, are discussed.

Cryoglobulinemia and Cryofibrinogenemia: Ten years of experience and diagnostic perspectives from a large laboratory-based cohort.

OBJECTIVES: Cryoproteins include cryoglobulins (CG), detectable in serum and plasma, and cryofibrinogen (CF), detectable only in plasma. Although they share clinical features, CG and CF differ in pathogenesis and diagnostic management. Cryofibrinogenemia remains poorly characterized and likely underdiagnosed. METHODS: We retrospectively analyzed CG tests performed between 2015 and 2024 and CF tests between 2020 and 2024 at the Laboratory of the Public Health Hospital of Modena, evaluating clinical and epidemiological characteristics. RESULTS: Among 10,874 patients tested for CG, 14% were pos

Cryofibrinogen-associated glomerulonephritis accompanied by advanced gastric cancer.

We had a 72-year-old man with advanced gastric cancer, poorly differentiated adenocarcinoma, receiving chemotherapy with S-1 (tegafur, gimeracil, and oteracil potassium) plus oxaliplatin. Ascites developed despite remission of gastric cancer and metastasis. Given no malignant cells in ascites, leg edema, renal impairment, hypoalbuminemia, and massive proteinuria, we diagnosed as nephrotic syndrome with microscopic hematuria. Renal biopsy showed membranoproliferative glomerulonephritis with no deposition of immunoglobulins and complements. Of note, electronic microscopy found organized deposits

Characteristic electron-microscopic features of cryofibrinogen-associated glomerulonephritis: a case report.

BACKGROUND: Cryofibrinogenemia is a rare disorder that mainly affects the skin and occasionally the kidney. However, there are few published reports of cryofibrinogenemia-associated renal pathology. We therefore report a patient with cryofibrinogen-associated glomerulonephritis. Samples from this patient were examined by electron microscopy, laser microdissection, and liquid chromatography-tandem mass spectrometry (LC-MS/MS). CASE PRESENTATION: A 78-year-old Japanese man presented with declining renal function, proteinuria, and gross hematuria. Kidney biopsy showed a membranoproliferative pat

Fibrinogen and D-dimer alterations in thyroid dysfunction: implications for bleeding and thrombotic risk.

Thyroid dysfunction can disrupt the balance of the hemostatic system, predisposing affected individuals to either bleeding or thrombotic complications, depending on the nature and severity of the disorder. In hyperthyroidism, a prothrombotic state is commonly observed, characterized by increased fibrinogen concentrations, elevated D-dimer levels, and enhanced coagulation activation. Conversely, hypothyroidism may impair coagulation and fibrinolysis, resulting in variable bleeding or thrombotic manifestations depending on disease severity. Raised D‑dimer levels reflect increased thrombin genera

The Correlation between the Triglyceride-Glucose Index and Coagulation Markers in Patients with Recent Acute Myocardial Infarction.

BACKGROUND: Metabolic abnormalities and hypercoagulability seem to have an important predictive role in patients with coronary artery disease (CAD). The triglyceride-glucose (TyG) index has emerged as a good marker for metabolic syndrome with predictive value for cardiovascular events. Overall haemostatic potential (OHP) is a reliable global haemostatic essay to identify hypercoagulability in CAD patients. The aim of our study was therefore to evaluate a possible correlation between the TyG index and haemostatic derangements in patients with CAD. METHODS: Consecutive patients referred for the

Thromboelastographic study of the snakebite-related coagulopathy in Djibouti.

: Hemostasis disorders are one of the major clinical conditions of snakebites and are because of mechanisms which may disrupt vessels, platelets, clotting factors and fibrinolysis. Thromboelastography (TEG) could help to understand these effects in the clinical practice. A retrospective study reports a series of patients presenting a snakebite-related coagulopathy, treated with antivenom and monitored with conventional tests and TEG in a French military treatment facility (Republic of Djibouti, East Africa) between August 2011 and September 2013. Conventional coagulation assays (platelets, pro

Combined fibrinogen and neutrophil-lymphocyte ratio as a prognostic marker of advanced esophageal squamous cell carcinoma.

Patients with advanced esophageal squamous cell carcinoma (ESCC) is received chemoradiotherapy or chemotherapy for clinical management. However, it is difficult to predict tumor response and prognosis using blood markers before starting treatments. The purpose of this study was to investigate the pre-treatment plasma fibrinogen and neutrophil-lymphocyte ratio (NLR) in patients with advanced ESCC treated with chemoradiotherapy or chemotherapy, and to assess the clinical utility of a combined score using these blood markers, named as the F-NLR (fibrinogen and NLR) score, as a predictor of tumor
