Fistula in ano.

Except for unusual diseases, fistula in ano originates from infection in the anal crypts of Morgagni, forming an abscess which, when it opens, results in a tract leading to the skin surface. A tentative diagnosis can often be made by a careful history followed by local examination. A history of para-anal abscess followed by intermittent discharge will almost always be elicited. Alternative causes, including especially Crohn's disease, are also described. Operative technique is described, including various methods for finding the internal opening and obscure branching tracts, as well as a numbe…

Splenic arteriovenous fistula.

We present a case of a splenic arteriovenous fistula (AVF) occurring postsplenectomy. The splenectomy was performed as a result of severe blunt abdominal trauma. The fistula was discovered in the postoperative period and controlled with intravascular coils. Splenic AVF are usually due to rupture of a pre-existing splenic artery aneurysm, post-traumatic, or iatrogenic. Diagnosis and treatment of a splenic AVF are necessary to prevent the development of hepatosclerosis and esophageal varices.

Esophageal-atrial fistula.

We report an unusual case of an esophageal-atrial fistula in a patient with CREST (calcinosis, Raynaud's phenomenon, esophagitis, sclerodactyly, telangiectasia) variant of scleroderma. An ulcer in Barrett's esophagus perforated into the left atrium and led to systemic embolization and cerebral abscess. A review of similar reports of esophageal-atrial fistula reveals a symptom complex that includes chronic esophageal pathology, gastrointestinal bleeding, and neurological signs. An antemortem diagnosis has never been made.

[Vesico-uterine fistula].

The incidence of vesico-uterine fistulas is very low; they can develop from normal delivery, malignant tumors, inflammations and iatrogenic lesions. The increased number of cesarean sections during the past years should also be considered. Two patients are presented. Symptoms, diagnostic and therapy are discussed.

Vesico-uterine fistula.

Nine cases of vesico-uterine fistula caused by injury to the bladder at Caesarean section or by rupture of the uterus and bladder following obstructed labour are described. Symptoms depend on the level of the lesion, menstruation into the bladder and menouria occurring when the fistula is above the internal cervical os: whatever the level, most patients with vesico-uterine fistula present with incontinence of urine. A transperitoneal approach appears to give better results than a transvesical repair.

[Iatrogenic biliovenous fistula].

A 44-year-old man had suffered from cholecystolithiasis with recurrent jaundice for many years, and a cholecystectomy was carried out. Because of a radiomanometric obstruction of the common bile duct, the intra- and extrahepatic bile ducts were also scanned. One day after the operation the jaundice increased (serum bilirubin max. 742 mumol/l). The cause of the icterus was found in a small biliovenous fistula, and diverse surgical and angiological attempts to resect or close the fistula failed. In the third postoperative week renal failure developed, followed by a disturbance of coagulation and…

Carotid cavernous fistula.

Although carotid cavernous fistulas from head trauma are rare, definitive diagnosis is usually not difficult. The presence of a temporal, orbital, or supraorbital bruit on auscultation enables the clinician to exclude most other pathologic conditions that affect the orbital structure. Recent advances in neuroangiography seem to make balloon catheterization the treatment of choice, precluding the necessity to embolize the fistula or ligate the internal carotid artery outside the cavernous sinus. Early recognition and treatment usually results in total resolution, but return of full visual acuit…

Congenital uterovesical fistula.

An example of congenital uterovesical fistula is presented. The cardinal diagnostic feature is monthly painless macroscopic hematuria with onset at menarche. It is possible that a delay in prompt diagnosis and corrective surgery may lead to undesired consequences. The literature has revealed only one other recorded case of the congenital type of uterovesical fistula.

Fistula in ano.

An eight year retrospective review of 133 patients with fistula in ano found the majority to be of cryptoglandular causation. Many patients had symptoms for longer than one year. Operation was performed safely under spinal anesthesia, locating the internal opening in 117 patients, performing a fistulectomy in 80 patients and a fistulotomy in the remainder. Associated procedures, such as a hemorrhoidectomy, could be performed safely. Early recurrence of a fistula, spinal headache, bleeding and temporary incontinence were the complications present in 14 patients. All patients with an early recur…

Biliary enteric fistula.

From 1932 to 1978, 105 patients with biliary enteric fistulas are reported upon from The New York Hospital-Cornell Medical Center. During this 46 year period, 11,808 patients were operated upon for nonmalignant biliary tract disease, representing an incidence of biliary enteric fistulas of 0.9 per cent. The most common location of the fistula was cholecystoduodenal in 77 per cent and cholecystocolic in 15 per cent. Ninety-eight of the 105 patients underwent 109 surgical procedures for symptoms and complications associated with the fistula. The operative mortality was 6 per cent. The recommende…
