Ankle Foot Orthoses (AFOs) are external supports typically prescribed in clinical practice to address foot drop deficits. Lower limb orthoses have been shown to have positive effects on spatio-temporal gait metrics, while the impact on body joint kinematics is less clear and varies in the literature. The objective of this study is to investigate the presence of a common movement pattern in body joints that compensates for the foot drop deficit. The study focuses on the trunk, knee, and hip joints. Eight patients with unilateral foot drop participated in walking trials on an instrumented treadm…

Gait kinematics and dynamics during stance of unilateral, below-knee child amputees were analyzed for self-selected, comfortable (0.9 m s-1) and fast (1.3 m s-1) speeds with the SACH foot and the energy-storing Flex-foot prostheses. The three-dimensional movements of the lower limbs were recorded and synchronized with ground reaction forces for 12 subjects (7 girls and 5 boys, ages 6-16 yr). Each lower limb was modeled as a set of interconnected rigid links (thigh, leg, and foot) with frictionless joints (hip, knee, and ankle) to calculate moment and joint power profiles. Marked asymmetries we…

The aim of this study was to investigate techniques for assessment of surgical procedures for foot disorders. An optoelectronic system was used to obtain position data during walking for the knee joint, ankle joint and the second metatarsal joint. The ground reaction forces were simultaneously measured and the moment of force at the mentioned joints were analysed. Pressure transducers in the shoe were used to measure the local pressure at the great toe, first and fifth metatarsal heads, and heel pad. Among studied procedures were Keller's operation for hallux rigidus and distal oblique osteoto…

Acute brain injuries are caused by a variety of etiologies, each potentially disrupting neurological function. The neurologic impairments ar … gical impairment associated with involuntary muscle hyperactivity can lead to a spastic equinovarus foot deformity. Spastic equinovarus foot deformities secondary to anoxic brain injuries or traumatic brain injury pose a challenging situation for orthopaedic surgeons because of associated cognitive impairment, spastic tone, and extensive soft-tissue co …

Foot ulcers are most often vascular, pressure-related or diabetes-related. The presence of neuropathy with loss of protective sensation and/or peripheral arterial disease are essential risk factors for diabetic foot ulcers. Prevention aims at identifying the foot at risk, regularly checking the foot at risk, educating patients, relatives and healthcare professionals, ensuring the routine wearing of appropriate footwear and treating risk factors for ulceration. In the presence of a diabetic foot ulcer, therapy consists of a combination of mechanical relief (gold standard during the ulcer phase:…

The diabetic foot assessment is a key component in the care of a patient with diabetes. The assessment includes risk factor identification in both the diabetic patient's history and physical examination, foot care education, treatment, and referrals as needed. The foot complications related to diabetes such as peripheral neuropathy, foot ulceration, and amputation can be life altering. The American Diabetes Association recommends a diabetic foot examination annually for patients with diabetes with foot care education. Diabetic foot assessment may be recommended more frequently for individuals …

History A 99-year-old Chinese-Indonesian woman with a longstanding history of diabetes mellitus was admitted for chest infection and was incidentally found to have bilateral deformed feet. She was otherwise asymptomatic and could ambulate independently. Photographs and radiographs of the feet were obtained.

The cavus foot is a deformity characterized by abnormal elevation of the medial arch of the foot. Unique among foot deformities, cavus typically occurs secondary to a spinal cord or neuromuscular pathology, with two-thirds of patients having an underlying neurologic diagnosis. Thus, recognition of cavus foot and appropriate evaluation are essential in the primary care setting. Patients may present with unstable gait, frequent ankle sprains, or pain along the metatarsal heads or the lateral border of the foot. The diagnosis can be confirmed with a lateral weight-bearing radiograph, with several…

BACKGROUND: Lower-limb amputation causes the individual a huge functional impairment due to the lack of adequate sensory perception from the missing limb. The development of an augmenting sensory feedback device able to restore some of the missing information from the amputated limb may improve embodiment, control and acceptability of the prosthesis.
FINDINGS: In this work we transferred the Rubber Hand Illusion paradigm to the lower limb. We investigated the possibility of promoting body ownership of a fake foot, in a series of experiments fashioned after the RHI using matched or mismatched …

Diabetic foot (DF) is the most common chronic complication, which depends mostly on the duration and successful treatment of diabetes mellitus. Based on epidemiological studies, it is estimated that 25% of persons with diabetes mellitus (PwDM) will develop the problems with DF during lifetime, while 5% do 15% will be treated for foot or leg amputation. The treatment is prolonged and expensive, while the results are uncertain. The changes in DF are influenced by different factors usually connected with the duration and regulation of diabetes mellitus. The first problems with DF are the result o…

Cleft foot or ectrodactyly is a rare congenital anomaly. It is often associated with cleft hands. Most early authors advised non-operative treatment but more recently surgical treatment has been advised. However the last two decades have seen a trend towards surgical management of these deformities. Most authors agree that whilst cosmesis is an important consideration, the main aim of correction is to provide feet which could fit into normal shoes and to provide or maintain a normal functional foot. We describe our experience with five feet in three patients treated surgically as a one step pr…
