Clinical and radiographic follow-up study of the elbow and shoulder in 19 children and adults with Apert syndrome demonstrates that these two joints are affected to varying degrees. Shoulder motion is never normal and may be quite restricted, with the progression of growth abnormalities of the proximal humerus and accompanying glenoid dysplasia. Limitation of abduction is secondary to an impingement of an overgrown greater tuberosity upon the acromion. Clinically, these children appear to have anterior subluxations. Elbow motion is limited in a much smaller percentage of patients and was not a

A retrospective study was carried out of the morbidity in the donor limb following use of the radial forearm flap in 27 patients. Of the 13 patients whose flaps contained bone, 4 sustained fractures of the radius. This was associated with a 50% loss of power of grip and pinch, and limitation of other movements, particularly pronation and supination. In the other patients little or no loss of power or range of movement was seen. No patient had cold intolerance; a few had areas of sensory loss, related to the radial nerve. Of the 12 patients whose radial arteries were reconstructed, 7 remained p…

We report our experience during a 15-year period with three patients who had severe segmental loss of more than 10 cm of both the median and ulnar nerves in the arm and forearm. The procedure was described by St. Clair Strange and by Silverstone in 1947. A modification of the procedure was used in our three patients. In two patients, the proximal stumps of the median and ulnar nerves were sutured together in the proximal forearm. The ulnar nerve was transected high in the axilla, leaving its blood supply intact. Several months later the ulnar nerve was transposed distally, and a juncture with …

We report a case of an elderly female who sustained a severely comminuted distal radial and ulnar fracture, treated by shortening of the forearm, combined with a primary Sauvé-Kapandji procedure and volar plating of the distal radius.

Arteriovenous malformations (AVMs) are characterized by flat epithelial lining and have an estimated prevalence of 4.5% in the general popul … 36-year-old male with no significant medical history, who presented with worsening pain in the left forearm, which had been tolerable since childhood. The patient underwent an ultrasound (US) with Doppler study and MRI, revealing findings consistent with a low-flow soft tissue arteriovenous malformation.

INTRODUCTION: High-energy combat trauma often results in injuries to both the radial and ulnar arteries, accompanied by extensive soft tissue loss and a low probability of sufficient collateral flow.
MATERIALS AND METHODS: A case series of 28 male patients with acute forearm arterial injuries from combat trauma was reviewed. Surgical approaches included ligation, end-to-end anastomosis, and autologous vein grafting. Temporary shunting was employed in select cases. The clinical outcomes were assessed.
RESULTS: Among 28 cases, 26 achieved successful hand salvage. Complications included one gra…

BACKGROUND: Refracture after both bone forearm fracture fixation may vary with or without plate removal. We tested the null hypothesis that there is no difference in the rate of refracture in patients who have undergone open reduction and internal fixation of a diaphyseal forearm bone who have retained implants versus removed implants. We also studied factors associated with plate removal.
METHODS: We retrospectively identified 645 adult patients with a total of 925 primary fractures that underwent primary plate fixation of an ulnar or radial shaft fracture between 2002 and 2015 at a single i…

Muscular and neurovascular variations in the upper extremity are of utmost clinical significance. Here we report a unique bilateral accessory muscle in the forearm and palm of an 89-year-old male cadaver. The accessory muscle presented two bellies on the right side, one in the forearm, innervated by the anterior interosseous nerve, and the other in the palm, innervated by a branch of the median nerve. A long tendon interconnected the two bellies. On the left side, the muscle had a single belly in the palm, which began at the end of a long tendon that extended from the forearm. However, on both…

BACKGROUND: The purpose of this study was to investigate residual rotation of patients with forearm amputation and the contribution of involved muscle to residual rotation.
METHODS: Testing was performed using five fresh-frozen cadaveric specimens prepared by isolating muscles involved in forearm rotation. Amputation was implemented at 25 cm (wrist disarticulation), 18 cm, or 10 cm from the tip of olecranon. Supination and pronation in the amputation stump were simulated with traction of involved muscle (supinator, biceps brachii, pronator teres, pronator quadratus) using an electric actuator…

The paper presents a mechanical forearm mechanism to extend the pronation and supination (P/S) movement of the trans-radial amputee when he/she is able to conduct the P/S motion partially with his/her remaining forearm. The P/S motions of the existing prostheses and robotic arms developed till now are completely different from those of the human. When we are to develop the robotic prostheses for the amputees, two important issues should be considered such as cosmetic issue to look like normal arm and motion harmony issue between robotic prosthesis and the amputated living arm. Most prostheses …

The forearm unit consists of the radius and ulna, a complex and interrelated set of joints (distal radioulnar joint, proximal radioulnar joint) and the soft tissue stabilizers between the 3 bones. Distally, this is represented by the triangular fibrocartilage complex at the wrist, proximally by the annular ligament at the elbow, and in the forearm by the interosseous membrane. Disruptions in any of these structures may lead to forearm instability, with consequences at each of the remaining structures.

INTRODUCTION: There is no classification for acquired forearm deformities. A clinical-radiographic study was conducted to classify these deformities and evaluate the results.
MATERIALS AND METHODS: Thirteen patients with forearm deformities following traumas or their treatment were included (11 men and two women, from 2000 to 2010). Mean age was 31 years (range 10-75 years). Initial treatment was conservative in five patients and surgical in eight patients. One segment was affected in seven patients (the radius in four patients, the ulna in three), and both segments were affected in six patie…
