Putting drug resistant epithelial herpes keratitis in the spotlight: A case series.

PURPOSE: To strengthen the sparse evidence on acyclovir (ACV) resistance, especially in recalcitrant herpetic keratitis (HK), by describing the clinical course of 3 genotypically proven ACV resistant HK cases. An overview of mechanisms of resistance and therapeutic options currently available to ophthalmologists is provided based upon recent literature search. OBSERVATIONS: Resistance to ACV due to known mutations in the gene encoding the viral thymidine kinase was confirmed in 2 cases, and a novel mutation in the UL23 gene (N202K) conferring phenotypical resistance to ACV was discovered in 1

Acute retinal necrosis: A mini review.

Acute retinal necrosis is a rare but potentially devastating disease. Even in the era of modern medicine, retinal detachment is a frequent complication leading to vison loss, as well as phthisis bulbi. Whereas IV acyclovir still remains the standard of care, high doses of valacyclovir with/without additional intravitreal injections of foscarnet have been used. In an attempt to reduce the retinal detachment rate, prophylactic laser treatment and early vitrectomy have been proposed. In this article, we aim to review current diagnostic and treatment modalities.

The development of cytomegalovirus encephalitis in AIDS patients receiving ganciclovir.

PURPOSE: Cytomegalovirus (CMV) retinitis in patients with the acquired immunodeficiency syndrome (AIDS) requires lifelong therapy with either intravenous ganciclovir sodium or foscarnet sodium. From June 1989 through February 1992, seven patients with AIDS were diagnosed to have CMV retinitis, and all were treated with ganciclovir. Five of the seven developed abrupt preterminal mental status changes. All five with mental status changes received anti-CMV therapy until the time of death. Autopsies were performed in all cases to determine the cause of mental status changes. PATIENTS AND METHODS:

Heterogeneity and viral replication fitness of HSV-1 clinical isolates with mutations in the thymidine kinase and DNA polymerase.

BACKGROUND: Prolonged antiviral therapy in immunocompromised individuals can result in the emergence of (multi)drug-resistant herpes simplex virus 1 (HSV-1) infections, forming a therapeutic challenge. OBJECTIVES: To evaluate spatial and temporal differences in drug resistance of HSV-1 samples from a HSCT recipient and to determine the effect of resistance mutations on viral replication fitness. PATIENTS AND METHODS: Five HSV-1 isolates were recovered from a HSCT recipient who suffered from persistent HSV-1 lesions, consecutively treated with aciclovir, foscarnet, cidofovir and a combination

Resistant Cytomegalovirus Infection in Solid-organ Transplantation: Single-center Experience, Literature Review of Risk Factors, and Proposed Preventive Strategies.

BACKGROUND: Cytomegalovirus (CMV) infection causes morbidity and mortality in solid-organ transplant recipients. Drug-resistant CMV is an emerging problem with poor survival outcomes and limited therapeutic options. In this study we comprehensively address the issue of drug resistance in CMV when compared with standard therapies, such as ganciclovir (GCV) and foscarnet. METHODS: We conducted a retrospective review of adult patients diagnosed with CMV after solid-organ transplant at our center between 2013 and 2017, and identified 7 resistant CMV cases. To study risk factors in the published l

Long-term visual morbidity of cytomegalovirus retinitis in patients with acquired immune deficiency syndrome.

BACKGROUND: Patient survival with the acquired immune deficiency syndrome (AIDS) and cytomegalovirus (CMV) retinitis is increasing because of earlier diagnosis and improved medical therapy. Visual morbidity that occurs with prolonged survival has not been well described. METHODS: To evaluate the progression of retinitis, ocular complications, and visual morbidity, the authors retrospectively reviewed the records of 22 patients who had tested positive for human immunodeficiency virus since December 31, 1987. Each patient had an ophthalmologic diagnosis of CMV retinitis and had survived for a m

Oral antiviral drugs for treatment of cytomegalovirus in transplant recipients.

BACKGROUND: Cytomegalovirus (CMV) is an opportunistic pathogen responsible for substantial morbidity after solid organ transplantation and haematopoietic stem cell transplantation. Treatment of CMV disease involves a two-pronged approach with antiviral drug treatment coupled with strategies to minimize the intensity of immune suppression. OBJECTIVES: This narrative review examines the evidence for the current treatment of CMV disease in transplant recipients, including the use of oral antiviral drugs. SOURCES: Literature search was performed on PubMed with keywords cytomegalovirus, transplan

Case series: Two cases of herpetic retinitis presenting as progressive outer retinal necrosis in immunocompetent individuals.

PURPOSE: To raise awareness of ophthalmologists that Varicella Zoster Virus (VZV) retinitis should be considered in the differential diagnosis of retinitis that presents with features of progressive outer retinal necrosis (PORN) in healthy immunocompetent patients. OBSERVATIONS: Case 1 is a 39-year-old healthy Caucasian male who presented after one week of decreased vision in the left eye. Patient was found to have optic disc edema and multifocal retinitis primarily localized to the posterior pole with only a few lesions in the periphery and minimal vitritis. Viral PCR of a vitreous tap was p

Molecular detection of human herpesvirus 7 DNA in cerebrospinal fluid from adult patients with neurological disorders.

Neurological manifestations associated with HHV-7 have been described in primary infection in children, and very occasionally in immunocompromised adult patients. However, the role of HHV-7 reactivation as a cause of central nervous system (CNS) diseases in immunocompetent adults has not yet been defined. We retrospectively analyzed clinical and microbiological features of adults with neurological symptoms who underwent lumbar puncture and a multiplex polymerase chain reaction (PCR) for herpesviruses (HHV-1-8) and enteroviruses performed in cerebrospinal fluid (CSF), during a 4-year period. Nosocomial HIV-transmission in an outpatient clinic detected by epidemiological and phylogenetic analyses.

OBJECTIVE: To determine if a case of HIV-infection in a patient (GP) with common variable immunodeficiency, and with no known risk factors for HIV-infection, could be due to horizontal nosocomial transmission. METHODS: For determination of time of transmission stored serum-samples from GP were analysed for HIV RNA content. Patient records were used to identify patients, who had received intravenous therapy on the same day as GP. Samples from GP and these possible source patients were identified and phylogenetic analyses of the env, gag and RT-encoding region of pol were performed. Furthermore

Cytomegalovirus retinitis in patients with AIDS: current status.

About 20%-30% of patients with AIDS will develop cytomegalovirus (CMV) retinitis when they are severely immunodeficient (CD4 cell count below 40 cells/microL). It causes extensive retinal damage with visual deficits, and relapses are frequent. Intravenous ganciclovir and foscarnet are standard therapy, but their high toxicity and long term intravenous access mean that advances in treatment, including oral prophylactic agents, intravitreal injections and surgical implants, may provide a better quality of life for patients. Prophylactic ophthalmological screening of patients with CD4 cell counts

Interferon successfully inhibited refractory cytomegalovirus infection and resulted in CD4+ T-cells increase in a patient with AIDS.

We report on a young male HIV-positive patient with cytomegalovirus (CMV) retinitis who received HAART and ganciclovir but could not gain immune restoration. One and a half years later, his CMV reoccurred. He was treated again with ganciclovir treatment combined with foscarnet treatment but failed. Finally, he was treated with recombinant human interferon alpha-2b (IFN-α2b) alone and acquired sustained inhibition to CMV. His CD4+ T-cell count increased to 186 cell/mm3 and gained partial immune restoration. Retinitis progression has stopped.

Antiviral treatment for severe EBV infections in apparently immunocompetent patients.

BACKGROUND: Infectious mononucleosis usually runs a mild self-limiting course. Complications arise rarely and when so, corticosteroids are the mainstay of their treatment. The role of antivirals in the management of severe EBV infections is debatable. METHODS: We sought to review the usage of antivirals for severe EBV infection in apparently immunocompetent patients. For this reason a search in PubMed and Scopus was performed for the time period from 1982 to 2009. RESULTS: 45 patients with severe manifestations of infectious mononucleosis received antivirals (as an adjunct to steroids in 26

Vegetative chronic genital herpes with satisfactory response to imiquimod.

Vegetative chronic genital herpes is an atypical presentation of herpes simplex 2 that it is usually seen in patients coinfected with human immunodeficiency virus. Clinically, it is characterized by extensive ulcers that evolve to chronification and hypertrophic pseudotumor forms. Antiviral drugs are recommended for the treatment, and acyclovir is the most used one. Foscarnet is the treatment of choice to resistant cases, although treatment failure has been reported. We report a male patient, previously diagnosed with human immunodeficiency virus who developed vegetative chronic genital herpes

Computational Drug Repositioning by Target Hopping: A Use Case in Chagas Disease.

BACKGROUND: Drug repositioning aims to identify novel indications for existing drugs. One approach to repositioning exploits shared binding sites between the drug targets and other proteins. Here, we review the principle and algorithms of such target hopping and illustrate them in Chagas disease, an in Latin America widely spread, but neglected disease. CONCLUSION: We demonstrate how target hopping recovers known treatments for Chagas disease and predicts novel drugs, such as the antiviral foscarnet, which we predict to target Farnesyl Pyrophosphate Synthase in Trypanosoma cruzi, the causativ
