Articular fractures.

Although injuries to articular cartilage may lead to radiographic osteoarthritis, pain, and decreased joint function, the actual effects of such injury and of its treatment on joint function are not completely understood. The mechanisms of repair after impact loading are different from those after frank disruption by fracture of the articular cartilage, but basic and clinical research both indicate that the resultant articular surface is prone to degeneration. The sensitivity of a joint to resultant incongruity varies considerably, depending on the thickness and modulus of the articular cartil

Herbert screw fixation of osteochondral fractures about the knee.

Osteochondral fracture of the patella or femoral condyle may occur with patella dislocation. Accepted treatments of these fractures are excision or replacement. In this paper eight cases are described in which the Herbert Bone Screw system was used for fixation. Normal knee function was regained within 6 months and there were no reported recurrent dislocations. The advantages of this technique are that it is simple, there is little cartilage damage done during insertion, the screw is entirely buried and therefore does not interfere with the soft tissues, fixation is rigid enough to allow for t

Evidence for overgrowth after midfemoral fracture via increased RNA for mitosis.

Middiaphyseal femoral fractures in children and young rats stimulate linear femoral growth, a phenomenon commonly attributed to increased vascularity. To test for changes in mRNA expression of genes related to blood vessels, nerve fibers, cartilage, bone, and cell metabolism, we measured mRNA gene expression for all known rat genes in the physis at various times after diaphyseal fracture. Female Sprague-Dawley rats, 4 weeks of age at surgery, were subjected to a unilateral, simple, transverse, middiaphyseal femoral fracture stabilized with an intramedullary rod. Contact pressures in the patellofemoral joint during impact loading on the human flexed knee.

Currently, a bone fracture criterion is used by the automotive industry to assess the potential for a lower extremity injury from impact directed on the flexed knee. However, recent studies with animal models indicate irreversible damage to articular cartilage due to overpressures generated within the patellofemoral (P-F) joint without bone fracture, and suggest this injury may lead to a progressive, degenerative disease of the joint. The purpose of this investigation was to measure contact pressure in the human P-F joint during impact loading on the isolated, flexed knee. Loads were delivered

Mechanical properties of cracked articular cartilage under uniaxial creep and cyclic tensile loading.

Cracks may change the mechanical properties of articular cartilage, and further lead to early osteoarthritis. The study aimed to probe the mechanical properties of cracked cartilage under uniaxial tensile loading. The fracture process of cracked cartilage can be divided into two stages, namely crack-tip blunting stage and crack growth stage. The creep strain of cracked cartilage increases rapidly and then slowly with time, and it is well predicted by the nonlinear viscoelastic creep model. Compared with intact cartilage, cracked cartilage shows larger creep strain. During cyclic loading, the m

[Nasal fractures--indications for open reposition].

BACKGROUND: Many septoplasties and septorhinoplasties are indicated due to previous traumatic etiologies, and the typical treatment of these septal and/or bone fractures has been closed reduction. Review of the literature reveals a success rate of only between 30-82%. One of the reasons for these poor outcomes is the influence of tension and pressure stress vectors due to hidden cartilage fractures behind intact septal mucosa. This is the main indication for open reduction of nasal trauma. The present study analyses the results of our experience with the open reduction of nasal fractures. MET

Editorial Commentary: Microfracture Remains a Foundational Tool for Cartilage Restoration During Hip-Preservation Procedures: Defying the Odds.

A comprehensive approach to arthroscopic hip preservation in patients whose pathology includes cartilage defects may include microfracture; microfracture has been shown to have long-lasting positive effects in most patients treated for femoroacetabular impingement plus full-thickness chondral pathology. Although modern cartilage treatment alternatives such as autologous chondrocyte implantation, autologous matrix-induced chondrogenesis scaffolds, allograft or autograft particulate cartilage graft, and others have been described for the treatment of high-degree cartilage acetabular lesions, mic

Effects of Stellate Ganglion Block on Healing of Fractures Induced in Rats.

OBJECTIVE: Sympathetic blocks are used as an adjunct for pain management in the treatment of orthopedic and traumatic conditions. Stellate ganglion (ganglion stellatum) provides sympathetic innervation of the head, neck and cervicothoracic regions, and upper extremities. No study was found in the literature investigating the effects of stellate ganglion block performed in the upper extremity, on blood supply to bone, density, vascularization, and bone metabolism. Therefore, the objective of this study was to investigate the effects of stellate ganglion block on healing of closed forearm fractu

MRI contribution in minimally displaced lateral humerus condylar fractures in children.

INTRODUCTION: Diagnosis and treatment of displaced humerus lateral condylar fracture is well codified with open reduction and pinning. For non-displaced or minimally displaced lateral condylar (NMDLC) fractures, diagnosis can be challenging because of cartilaginous structures none visualized on radiographs. AIM: To determine the usefulness of MRI in evaluating articular extension of NMDLC fracture. METHODS: We reviewed consecutive NMDLC fractures during 6 years including children younger than 15 years old with displacement lesser than 2 mm (Rigault type I) at initial radiographs divided in t

Immunolocalization of selected cytokines and proteases in canine articular cartilage after transarticular loading.

Cytokines and proteases are thought to play a role in the destruction of cartilage and the development of osteoarthritis. The purpose of this study was to document chronological involvement of interleukin-1 beta (IL-1 beta), tumor necrosis factor-alpha (TNF-alpha), stromelysin (MMP-3), fibronectin, and alteration in the chondroitin sulphate sulfation pattern. Canine patellae underwent a closed-joint impact to induce the development of osteoarthritis. The animals were killed at 2, 12, 24, and 52 weeks. The patellar damage included cracks in the superficial zone of cartilage and the zone of the

Biomechanical analysis of costochondral graft fracture in temporomandibular joint replacement.

This study is the first attempt to explore the reason of costochondral graft fracture after lengthy mandible advancement and bilateral coronoidectomy by combining finite element analysis and mechanical test. Eleven groups of models were established to simulate costochondral graft reconstruction in different degrees of mandible advancement, ranging from 0 to 20 mm, in 2 mm increment. Force and stress distribution in the rib-cartilage area were analyzed by finite element analysis. Mechanical test was used to evaluate the resistance of the rib-cartilage complex. Results showed a sharp increase in

[Hot topics and challenges in the treatment of Pilon fractures].

Pilon fractures are comminuted fractures of the distal tibial articular surface caused by high-energy axial loading, typically resulting from falls from height, motor vehicle accidents, or sports injuries. The patients with Pilon fractures are often associated with articular cartilage damage, metaphyseal comminution, and severe soft tissue injury, which can lead to poor long-term prognosis. Pilon fractures are among the most challenging types of fractures to treat. At present, there are still some controversies regarding the treatment of Pilon fractures. This article aims to explore hot-topic

Unusual orientation of talar osteochondral fragment: a case report.

The identification of the correct orientation of an osteochondral fragment can be challenging. Orthopaedists have been able to take advantage of advanced imaging techniques to provide guidance to the appropriate surgical intervention. Many advancements have been made in imaging modalities specific to articular cartilage [Fischbach F, Bruhn H, Unterhauser F, Ricke J, Wieners G, Felix R, et al. Magnetic resonance imaging of hyaline cartilage defects at 1.5T and 3.0T: comparison of medium T2-weighted fast spin echo, T1-weighted two-dimensional and three-dimensional gradient echo pulse sequences.

Sleeve fractures of the patella in children: a report of three cases.

The clinical characteristics of sleeve fractures of the patella are described and the methods of treatment are reviewed. Attention is drawn to the fact that the diagnosis may be missed because the distal bony fragment may be so small as not to be detectable by radiography. This is important because a large fragment of articular cartilage also separates. The best result was obtained from reconstitution of the extensor apparatus by internal fixation of the patellar fracture with repair of the quadriceps expansion.

[Reconstructive surgery of the orbit and adjacent paranasal structures (author's transl)].

In reconstruction of the orbital floor, homograft lyophilised dura or cialit-stord rib cartilage are suitable, but the best materials are autologous cartilage or silastic or teflon. Autograft bone from the maxilla or rib is rarely used. The surgery of traumatic hypertelorism and the reconstruction of the medial angle of the eye after orbito-nasal fractures are described and illustrated. With complete destruction of the floor of the orbit, a fronto-temporal pedicled flap combined with auricular cartilage is recommended. In this way, the orbital floor and the lower eyelid can be reconstructed. T

Role of endochondral ossification of articular cartilage and functional adaptation of the subchondral plate in the development of fatigue microcracking of joints.

The mechanisms that regulate functional adaptation of the articular ends of long bones are poorly understood. However, endochondral ossification of articular cartilage and modeling/remodeling of the subchondral plate and epiphyseal trabeculae are important components of the adaptive response. We performed a histologic study of the distal end of the third metacarpal/metatarsal bone of Thoroughbreds after bones were bulk-stained in basic fuchsin and calcified sections were prepared. The Thoroughbred racehorse is a model of an extreme athlete which experiences particularly high cyclic strains in

Trans-chondral fractures of the talus: a review of 64 surgical cases.

The authors present a retrospective study of 67 transchondral fractures of the talus in 64 patients that were treated surgically. A thorough literature review, physiology of articular cartilage and subchondral bone repair, and a comprehensive treatment of transchondral lesions are presented. Consideration is given to surgical approaches that require fibular and medial malleolar osteotomy, and to open reduction with internal fixation of the transchondral fractures. A new, practical, radiographic classification with appropriate terminology is proposed which acknowledges recent fractures from sub
