PURPOSE: To evaluate internal distraction plating for the management of comminuted, intra-articular distal radius fractures in patients greater than 60 years of age at two level 1 trauma centers. We specifically desired to determine whether patients would have acceptable results from the clinical standpoint of range of motion, Disabilities of the Arm, Shoulder, and Hand (DASH) score, and the radiographic measurements of ulnar variance, radial inclination, and palmar tilt. Our hypothesis was that distraction plating of comminuted distal radius fractures in the elderly would result in acceptable

BACKGROUND: Satisfactory internal fixation of comminuted radial head fractures is often difficult to achieve, and radial head resection has been the accepted treatment. In this study, we compared the results of radial head resection with those of open reduction and internal fixation in patients with a comminuted radial head fracture.
METHODS: Twenty-eight patients with a Mason type-III radial head fracture (some with associated injuries) were enrolled in the study. Fifteen patients underwent radial head resection as the initial treatment (Group I), and thirteen patients underwent open reducti

Excision is not a suitable treatment for all comminuted fractures of the radial head. In elbows where instability can be predicted, a replacement arthroplasty of the radial head is more effective. The aim of this paper was to present the medium-term results of the Judet floating radial head prosthesis. This operation was performed on 14 patients between 1992 and 2003, of whom 12 were reviewed at a mean follow-up of five years and three months (1 to 12 years). The outcome was assessed using the Mayo elbow performance score and a modified Disability of Arm Shoulder Hand (DASH) questionnaire. The

We have performed primary Sauve-Kapandji procedures on four patients with severe open comminuted fractures of both the distal radius and ulna. The fragmented distal ulna was fixed to the sigmoid notch in order to stabilize the ulnar side of the carpus, and a proximal pseudoarthrosis was maintained for forearm rotation. All the distal radial fractures united without major complications. The mean wrist flexion/extension arc was 76 degrees , the mean pronation/supination arc was 135 degrees, and grip strength was 64% of the contralateral side. All patients returned to their work or daily activiti

This report comprises a follow-up of 39 patients after simple excision and 16 patients after Silastic replacement of the radial head for displaced comminuted fractures. Prostheses were used when similar fractures were associated with an unstable elbow. The excision group was followed up for 6.1 years on average, and the replacement group for 5.7 years. The objective result after simple excision was good in 55% of cases, fair in 26% and poor in 19%. The results in the replacement group were only 13% good, 38% fair, 37% poor and in 12% the prostheses had to be removed.

Thirty-five comminuted fractures of the distal radius in 34 patients were reduced by manipulation and pins, then immobilised with plaster. The Kirschner wires were inserted into the first metacarpal and the olecranon. In five cases, problems with the proximal pin occurred. A clinical and radiological examination could be done in 21 patients three to nineteen years after the accident. Using the demerit-point-system of Gartland and Werley, the result was excellent in three patients, good in nine, and fair in nine; there were no poor results. After careful retrospective consideration of treatment

INTRODUCTION: Historically, cerclage wires were not used in the treatment of clavicle fractures because of their invasiveness. The purpose of this study was to evaluate the radiologic results and the incidence of complications following cerclage wire application and plate fixation in the treatment of comminuted mid-shaft clavicle fractures.
MATERIALS AND METHODS: A total of 116 patients with comminuted mid-shaft clavicle fractures who underwent open reduction and internal fixation were reviewed. We analyzed the postoperative length ratio and bone union period according to the fracture classif

As therapists and physicians, we often need to work with our patients to cater treatment accordingly. These authors describe how they modified a treatment protocol for a patient that sustained a proximal interphalangeal joint comminuted fracture and dorsal dislocation, but refused surgery. Their modification allowed the patient to return to full activities. - Victoria Priganc, PhD, OTR, CHT, CLT, Practice Forum Editor.

PURPOSE: The aim of this study is to analyse the effect of an additional radial buttress plate for palmar plate osteosynthesis in an AO/OTA 2R3 C2.1 fracture model. METHODS: Nine pairs of freshly frozen radii were analysed for pathology and bone mineral density and divided into two matched groups. One group was treated with a variable-angle palmar locking plate alone, while the second group received an additional radial buttress plate for radiopalmar double plating. An AO/OTA 2R3 C2.1 fracture was created in all specimens. The biomechanical tests were performed according to previously publishe

PURPOSE: Persistent malalignment-especially rotational error-after fixation of comminuted femoral shaft fractures compromises function and often necessitates revision surgery. This study evaluated whether point-of-care, patient-specific three-dimensional printed reduction guides can reproducibly restore native femoral length, coronal-sagittal alignment and axial rotation in a cadaveric model simulating highly comminuted shaft fractures.
METHODS: Ten freshfrozen human legs were CTscanned, virtually reduced, and fitted with patientspecific guides printed in a sterilizable medical resin. A 2 cm

BACKGROUND: Locking plates are commonly used in the treatment of comminuted metaphyseal distal femoral fractures. However, locking plates form a strong structure and promote asymmetrical callus formation, which is not conducive for rapid fracture healing and may increase fracture risk. To overcome this, we designed a micromotion-balancing fixation system based on locking plates.
METHODS: Six healthy pigs (Bama miniature pigs) were used to establish a model of bilateral comminuted distal femoral fracture (AO/ASIF: 33-C2). Standard drilling was performed on one of each pig's hind limbs (control

PURPOSE: To determine the efficacy of pins and rubber band traction for treatment of comminuted intra-articular fractures in the hand.
METHODS: We performed a retrospective study from 1994 to 2013 to evaluate 33 patients in whom pins and rubber band traction was employed. We clinically evaluated the active range of motion of the affected fingers after surgery. Eleven of the 33 fractures were at the proximal interphalangeal joint, 10 at the distal interphalangeal joint, 5 at the thumb interphalangeal joint, and 2 at the metacarpophalangeal joint of the thumb. The remaining 5 patients had compl

BACKGROUND: Although open reduction and internal fixation via the extended lateral approach is currently considered gold-standard, severely comminuted calcaneal fractures might not be amendable for reconstruction. The primary aim of the current review study was to assess the functional outcome of the primary arthrodesis in the management of comminuted displaced intra-articular calcaneal fractures.
METHODS: The literature was searched for studies published between January 1st 1990 and December 1st 2010, to identify studies in which a primary arthrodesis was utilized for the treatment of displa
