The Fingerprint of Scalp-EEG in Drug-Resistant Frontal Lobe Epilepsies.

PURPOSE: Scalp-EEG incompletely covers the frontal lobe cortex. Underrepresentation of frontobasal or frontomesial structures, fast ictal spreading, and false lateralization impede scalp-EEG interpretation. Hence, we investigated the significance of scalp-EEG in the presurgical workup of frontal lobe epilepsy. METHODS: Using descriptive statistical methods and Pearson chi-squared test for group comparisons, we retrospectively investigated postsurgical outcome, interictal epileptiform discharges (iiEDs), and electrographic seizure patterns on scalp-EEG in 81 consecutive patients undergoing res…

The three clap test: a window into cognitive and frontal lobe dysfunction.

The three clap test (TCT) is a simple bedside test that can elicit the so-called applause sign (AS), manifesting with more than three claps after the examiner's instruction to clap three times. Although the AS was originally described as a useful sign differentiating patients with progressive supranuclear palsy (PSP) from patients with Parkinson's disease (PD) and frontotemporal dementia (FTD), it is also known to be present in PD, Alzheimer's disease and several other diseases. It reflects the so-called stopping impulsivity and cognitive and frontal lobe dysfunction. In addition to the AS, th…

Supraorbital Endoscopic Evacuation for Traumatic Intracerebral Hematomas in the Frontal Lobe.

OBJECTIVE: Traumatic intracranial hematomas have been rarely evacuated by endoscopic surgery. The frontal lobe is the usual location for the traumatic intracerebral hematoma (TICH). Endoscopic evacuation for the frontal TICHs via an eyebrow incision is to be presented as minimally invasive surgery. METHODS: Thirteen patients with frontal TICHs were managed with endoscopic hematoma evacuation via eyebrow incision. After making the incision in the lateral eyebrow, a small frontal craniotomy was made, and the hematoma was evacuated under direct visualization of a rigid endoscope. No catheter was…

Frontal lobes dysfunction across clinical clusters of acute schizophrenia.

INTRODUCTION: Schizophrenia is a clinical construct comprising manifold phenotypes underlying heterogeneous biological underpinnings. The Positive and Negative Syndrome Scale (PANSS) represents the standard tool in the clinical characterization of patients affected by schizophrenia, allowing to detect different clinical profiles within the disorder. Frontal lobes are a key area of brain dysfunction in schizophrenia. We investigated whether different clinical profiles in acute schizophrenia show differences in frontal lobes dysfunction or not. METHODS: We defined PANSS-derived principal compon…

Frontal lobe growth is impaired in fetuses with congenital heart disease.

OBJECTIVES: The primary objective of this study was to assess whether fetuses with congenital heart disease (CHD) have smaller frontal brain … ers. The transventricular axial view of the fetal head was used as the reference view, on which the frontal lobe anteroposterior diameter (FAPD) and the occipitofrontal diameter (OFD) were measured, assuming the former to be representative of the area of the frontal lobes. The FAPD/OFD ratio was then calculated as FAPD/OFD × 100. These two variables (FAPD and …

Assessment of radial glia in the frontal lobe of fetuses with Down syndrome.

Down syndrome (DS) occurs with triplication of human chromosome 21 and is associated with deviations in cortical development evidenced by simplified gyral appearance and reduced cortical surface area. Radial glia are neuronal and glial progenitors that also create a scaffolding structure essential for migrating neurons to reach cortical targets and therefore play a critical role in cortical development. The aim of this study was to characterise radial glial expression pattern and morphology in the frontal lobe of the developing human fetal brain with DS and age-matched controls. Secondly, we i…

Age-related differences in frontal lobe function in children with ADHD.

BACKGROUND: The neural correlates of executive function disorders are thought to be predominantly localized within the prefrontal cortex (PFC). However, no study to date has investigated changes in this system across different age groups in children with attention deficit hyperactivity disorder (ADHD). Thus, this study aimed to explore changes in PFC function in children with ADHD. METHODS: Study participants included typically developing (TD) children (n = 140) and children with ADHD (n = 67) of primary school age. Behavioral executive functions and their neural basis were evaluated between

Behavioural changes as the first manifestation of a silent frontal lobe stroke.

A 67-year-old man was admitted to our hospital after his relatives found him to have severe personality and behavioural changes. His behaviour was inappropriate and uninhibited. The patient reported no symptoms and he showed poor insight into his own behaviour. Neuroimaging showed an orbitofrontal lesion, due to an infarction of the anterior cerebral artery. The patient was diagnosed with frontal lobe syndrome.

[Patterns of brain functional connectivity in frontal and temporal lobe epilepsies].

AIM: To examine alterations of functional connectivity (FC) of the brain in patients with frontal and temporal lobe epilepsies. MATERIAL AND METHODS: Forty-three patients, aged 18-55 years, including 32 with temporal lobe epilepsy (TLE) and 11 with frontal lobe epilepsy (FLE), and 32 age/gender-matched healthy controls (HC) underwent structural and functional MRI on 1,5 T scanner. Imaging data were further analysed for functional connectivity characteristics by seed-based and ICA analyses. RESULTS: Most prominent in the TLE group, was a decrease in FC of insula and peri-insular cortical regi…

Supraorbital Approach to Basal Frontal Lobe for Resection of Brain Metastasis.

This video illustrates the use of the supraorbital craniotomy via an eyebrow incision for access to the basal frontal lobe. This approach is particularly useful for removal of lesions in the dominant hemisphere, where a transcortical approach could place speech centers at risk. The case is that of a 57-year-old woman who presented after a mental status change and was noted to have a large left frontal mass. The patient had a history of melanoma, and pathologic analysis of the intracranial lesion was consistent with metastatic melanoma. The patient underwent an uneventful gross-total lesion res…

Distinct neural mechanisms of distractor suppression in the frontal and parietal lobe.

The posterior parietal cortex and the prefrontal cortex are associated with eye movements and visual attention, but their specific contributions are poorly understood. We compared the dorsolateral prefrontal cortex (dlPFC) and the lateral intraparietal area (LIP) in monkeys using a memory saccade task in which a salient distractor flashed at a variable timing and location during the memory delay. We found that the two areas had similar responses to target selection, but made distinct contributions to distractor suppression. Distractor responses were more strongly suppressed and more closely co

A case with hyperkinetic frontal lobe epilepsy presenting as a psychiatric disturbance.

We report a case emphasizing the association of epilepsy with psychopathology. The patient was first referred for impulsive and inattentive … hanges in the anterior frontal areas, and neuropsychologic assessment findings were compatible with frontal lobe damage. Treatment with carbamazepine not only controlled hyperkinetic motor seizures but also helped interictal inattentiveness and behavioral impulsivity. In our case, ictal behavioral changes were masked by interictal behavioral disturbances and t …

Evidence of reduced glutamate in the frontal lobe of HIV-seropositive patients.

Neurological complications associated with the acquired immunodeficiency syndrome, in particular, HIV-associated dementia, continue to plague those infected. We report our finding that the concentration of brain Glu is reduced in the frontal white matter region in this condition. In addition, our data appear to absolve highly active retroviral therapy (HAART) from blame, as drug-naïve patients were equally affected. Our findings suggest that Glu neurotransmission is abnormal and may be a key target for early interventions to reduce the later incidence of neurocognitive impairment and dementia

Changes in frontal lobe activity with cognitive therapy for spider phobia.

Patients suffering from spider phobia were studied with measurement of regional cerebral blood flow (rCBF) when they were looking at a video recording of living spiders. Six patients were studied before and after cognitive psychotherapy with successful outcome in all cases. On each occasion rCBF was measured under 3 conditions: during rest, during exposure to a video recording of neutral nature scenery, and finally while the patient watched a recording of living spiders. The patients who managed to control their emotional reactions without panicking during spider exposure before treatment show

Attention, memory, and behavioral adjustment in children with frontal lobe epilepsy.

To explore whether attention, memory, and behavior would be more affected in children with frontal lobe epilepsy than in children with other types of epilepsy, we compared 16 children with frontal lobe epilepsy (FLE), 8 with temporal lobe epilepsy (TLE), and 8 with generalized absence (GEA) seizures on the Performance Speed (PS) and Freedom of Distraction (FD) indices of the WISC-III, the Continuous Performance Test (CPT), the California Verbal Learning Test (CVLT), and Rey-Osterrieth Complex Figure (ROCF). Parents completed Achenbach's Child Behavior Check List. Children with FLE scored signi…
