Fire fighters' respiratory organs, circulatory system and muscular system are often exposed to considerable risks. In fact in addition to the most obvious external stress causes (heat, humidity, O2 decrement, CO2 increment, emotional stress) these workers use heavy equipments and carry people or things. Furthermore always present is the risk of poisoning by inhalation of toxic combustion gas of the fire atmosphere.

Simultaneous poisoning by carbon monoxide (CO) and hydrogen cyanide is the major cause of mortality in fire gas accidents. Here, we report on the invention of an injectable antidote against CO and cyanide (CN-) mixed poisoning. The solution contains four compounds: iron(III)porphyrin (FeIIITPPS, F), two methyl-β-cyclodextrin (CD) dimers linked by pyridine (Py3CD, P) and imidazole (Im3CD, I), and a reducing agent (Na2S2O4, S). When these compounds are dissolved in saline, the solution contains two synthetic heme models including a complex of F with P (hemoCD-P) and another one of F with I (hemo

Background Ethylene glycol is a highly toxic compound found in various household products. Cases of poisoning are rare but may be fatal unless diagnosed and treated promptly. Early recognition of poisoning is critical for the management and recovery of patients. Indirect testing is not specific for the presence of ethylene glycol. Therefore, urgent and accurate measurement should be sought if ingestion is suspected in order to determine the need for treatment with an antidote. Here, we present the validation of an automated assay for measurement of ethylene glycol on an Abbott Architect using

This paper reports three rare cases of accidental sodium nitrite poisoning, including one fatality, caused by the consumption of aspic purchased from a private vendor. Clinical symptoms included cyanosis, hypotension, and respiratory distress, with methemoglobin (MetHb) levels ranging from 5 % to 41.7 %. Toxicological analysis, performed using gas chromatography coupled with a mass spectrometry in the negative chemical ionization mode (GC-NCI-MS), revealed nitrite concentrations in blood samples ranging from 4.2 to 44 µg/mL and nitrate concentrations from 36 to 256 µg/mL. Urine analysis in the

Zinc phosphide is commonly used as a rodenticide and often abused for intentional self harm. For years, Scientist believed that, It gets converted to phosphine gas by hydrolysis in acidic circumference of the stomach upon ingestion. It is absorbed into the bloodstream and can affect cardiovascular, respiratory, hepato-biliary hematological and gastrointestinal tract. It also results into electrolyte imbalance and ultimately leads to multi organ failure. We present a case of 46 year-old-male with acute pancreatitis due to zinc phosphide ingestion that was successfully treated conservatively. Ac

Traditionally, the carboxyhemoglobin (HbCO) level has been used to estimate the severity of carbon monoxide (CO) poisoning. Through extensive clinical experience, we have found this to be an inaccurate correlation; thus, other variables such as pHa or psychometric abnormalities have been assessed to determine severity from CO poisoning. In a 5-yr experience with 247 patients, on admission 114 had an arterial blood gas and HbCO determinations, and 88 also had psychometric testing. The data were evaluated to determine any relationship between pH and HbCO level; a weak correlation (r approximatel

Cerebral arterial gas embolism (CAGE) is a rare but serious cause for acute neurologic deficit that occurs most often in divers who breathe compressed gas at depth or iatrogenically from a variety of invasive medical procedures. We present a rare case of CAGE caused by inhaling helium from an unregulated, high-pressure gas cylinder. Following inhalation, the patient experienced loss of consciousness, neurologic deficits, pneumomediastinum, and pneumothorax requiring transfer and treatment at a hyperbaric facility with resulting resolution of neurologic symptoms. This case highlights the import

Hydrogen sulfide (H2S), a well-known poisonous gas, has been recognized as a critical endogenous gas transmitter in the past decade. To provide a quick and efficient detection method for hydrogen sulfide, a novel fluorescent probe DCI-NCN was designed based on an isophoronitrile scaffold featured with the cyanoxy group as the detection group. DCI-NCN shows promising results including high selectivity, high sensitivity, good linear relationship and pH stability in vitro. In addition, good biosystem imaging performance of DCI-NCN is observed in Kunming mice.

Sodium cyanoborohydride (NaBH3CN) is a commonly used reducing agent. However, its metabolism and mechanism of toxicity are still unclear. In this study, we first demonstrated that NaBH3CN underwent rapid hydrolysis in aqueous solution and produced hydrocyanic acid. Cyanide was detected in the gastric contents of rats following oral administration of NaBH₃CN, and cyanide poisoning symptoms such as tachypnea, limb convulsions, and opisthotonos were observed. Combined with a gas chromatography-mass spectrometry (GC-MS) method and rat poisoning models, we systematically investigated the metabolism

Co-upcycling plastics with lithium-ion batteries (LIBs) into carbon nanotubes (CNTs) offers a promising high-value approach; however, it is hindered by the challenge of poisoning metal catalysts by carbon deposition and the uncontrollable particle growth. Here, we propose a co-upcycling strategy to convert spent LiNixCoyMn1-x-yO2 (NCM) and binary plastics-polyethylene terephthalate (PET) and other plastics-into NiCoMnOx/CNTs composites (or materials) for microwave absorption. During the pyrolysis, the generated pyrolysis gas reduces the NCM to NiCoMnOx and Li2CO3, and the NiCoMnOx catalyzes th

The unintentional poisoning of off-target animals by bromadiolone, a second generation anticoagulant rodenticide, is an undesirable outcome requiring sensitive analytical methods. In this study, a rapid and sensitive method for the determination of bromadiolone in liver and blood plasma by means of gas chromatography coupled with tandem mass spectrometry without need for derivatization was developed. The method is based on the in-injector pyrolysis of bromadiolone and subsequent gas chromatography coupled with ion trap tandem mass spectrometry with electron ionization. Sample preparation inclu

Outbreaks of methanol poisoning have been described in the medical literature worldwide. However, the few outbreaks that have occurred in Saudi Arabia remain undocumented. This is especially noteworthy in light of the fact that Saudi Arabia is among the countries that explicitly prohibit the usage of alcoholic beverages and recreational drugs. Herein, we describe five cases of methanol poisoning in Saudi Arabia. The first three comprise patients admitted to the emergency room (ER) with signs of methanol toxicity, such as heart palpitations, vomiting, and blurred vision; otherwise, those patien

Two commonly used pesticides in agriculture are phosphides of aluminium and zinc. Both of these metal phosphides act through elaboration of toxic phosphine gas. The poisoning in Iran is mostly oral and suicidal. Phosphine is rapidly absorbed throughout the gastrointestinal tract after ingestion and it is partly carried to the liver by the portal vein. In this study the liver histopathology of fatal poisoning is scrutinized. A descriptive, retrospective study was performed on 38 fatal phosphine poisonings. The slides of liver specimens of the cases were retrieved and studied separately by two p

Hyperbaric oxygen (HBO2) is the inhalation of 100% oxygen at pressures > 1.4 times atmospheric pressure. Hyperbaric oxygen can be delivered in monoplace (single person) or multiplace (multi-person) chambers. Most clinical HBO2 exposures are between 2 and 2.4 atm abs for approximately 2 hours. Hyperbaric oxygen causes the blood and tissue oxygen levels to increase, reduces the volume of intravascular and tissue bubbles (to treat decompression sickness [DCS] and arterial gas embolism [AGE]), and accelerates wash-out of other gases, such as nitrogen or carbon monoxide (CO), which is important for

CONTEXT: Cyanide is a component of smoke in residential and industrial fires, and accidental exposure to cyanide occurs in a variety of industries. Moreover, cyanide has the potential to be used by terrorists, particularly in a closed space such as an airport or train station. Current therapies for cyanide poisoning must be given by intravenous administration, limiting their use in treating mass casualties.
OBJECTIVE: We are developing two new cyanide antidotes--cobinamide, a vitamin B(12) analog, and sulfanegen, a 3-mercaptopyruvate prodrug. Both drugs can be given by intramuscular administr

For several decades, hydrogen sulfide (H2S) has been a toxic gas affecting people, particularly in workplaces. However, no effective therapy is available to counteract H2S poisoning. Herein, we report the case of a 34-year-old male field worker who experienced H2S poisoning due to an accident at work. He presented to the emergency room with dyspnea, drowsiness, and dizziness. Computed tomography revealed a normal brain mass. An initial electrocardiogram revealed sinus tachycardia. Therefore, 10 mL nitrite was administered intravenously. However, the symptoms were not relieved as expected. Hype
