Transmastoid decompression of the facial nerve in Bell's palsy.

We describe the technique of decompression of the facial nerve from the geniculate ganglion to the stylomastoid foramen. The decompression was made on 51 patients with Bell's palsy, and the results of follow-up study were compared with those of 77 patients in whom the pyramidal and vertical segments were decompressed. The decompressions were indicated when a severe denervation of the nerve was confirmed by electrodiagnostic examinations. Incidences of notable residual palsy and of pronounced sequelae were always lower in the former group of patients regardless of the operational timing. In eit…

Thalamic Visual Prosthesis.

Glaucoma is a neurological disorder leading to blindness initially through the loss of retinal ganglion cells, followed by loss of neurons higher in the visual system. Some work has been undertaken to develop prostheses for glaucoma patients targeting tissues along the visual pathway, including the lateral geniculate nucleus (LGN) of the thalamus, but especially the visual cortex. This review makes the case for a visual prosthesis that targets the LGN. The compact nature and orderly structure of this nucleus make it a potentially better target to restore vision than the visual cortex. Existing

[Aneurysmal cyst of the petrous bone. Apropos of a case].

Aneurysmal bone cyst (ABC) is a rare form of osseous dystrophy. A lesion of the petrous temporal bone appears to be extremely rare. It must be differentiated from a hemangioma which is a hamartoma affecting the geniculate ganglion or the internal auditory meatus. The case described involved a 23-year-old woman. The ABC presented with violent otalgia and severe endolabyrinthic-type perception deafness. It was associated with extensive bony destruction of the petrous temporal and occipital bones. Curettage only led to considerable through temporary regression of deafness and bone reconstruction …

The Diverse Role of ipRGCs in Visual Perception Beyond Non-Image-Forming Functions.

The intrinsically photosensitive retinal ganglion cells (ipRGCs) are the third class of photoreceptors apart from rods and cones, containing the photopigment melanopsin and are characterized into different subtypes (M1-M6) that support conscious visual perception. They transmit the light information to the brain through complex circuits serving both non-image-forming and image-forming functions. Of the total population, there are four main ipRGCs, such as M2, M4, M5 and M6 that project substantially to the image-forming visual pathway and target the dorsal lateral geniculate nucleus (dLGN). In addition, the optic nerve gets thinner and the cells of the lateral geniculate ganglion disappear partially. On average, ocular blood flow (OBF) is reduced in glaucoma patients in various tissues of the eye. Increased intraocular pressure (IOP) is a major risk factor for glaucomatous damage. Late-Onset Leber's Hereditary Optic Neuropathy with Concurrent Retinal Detachment and Retrobulbar Visual Pathway Involvement.

We report a case of late-onset Leber's hereditary optic neuropathy (LHON) with concurrent retinal detachment, mild retinal pigment epithelial changes, cataract and hyperintensity on fluid-attenuated inversion recovery magnetic resonance imaging affecting the entire retrobulbar visual pathway. We also documented that progression of the visual field defect correlated with retinal nerve fibre layer and ganglion cell layer-inner plexiform layer changes on optical coherence tomography. Our case provides further understanding of LHON as a disorder of the entire pre-geniculate pathways and also highl

Ramsay Hunt syndrome presenting as simple otitis externa.

Ramsay Hunt syndrome is a rare complication of herpes zoster in which reactivation of latent varicella zoster virus infection occurs in the geniculate ganglion, causing otalgia, auricular vesicles and peripheral facial paralysis. Because these symptoms do not always present at the onset, this syndrome can be misdiagnosed. We report the case of a patient who was diagnosed with simple otitis externa after presenting to the emergency department (ED) with a 3-day history of right-sided otalgia. Her condition subsequently evolved to include right-sided auricular vesicles and right-sided facial weak…

A biography of James Ramsay Hunt (1874-1937).

James Ramsay Hunt (1874-1937) was a pre-eminent twentieth-century American neurologist. The name of Ramsay Hunt is known today because several neurological disorders bear his name, including the herpetic geniculate ganglion syndrome and a form of ataxia and myoclonus. Despite his importance in the field of neurology, few biographical details have been recorded about Hunt's life. One of the authors of this report recently located Hunt's daughter. This biographical sketch was based on interviews conducted with her and review of documents in her possession, including letters written by Hunt. Deta…

Early postnatal expression of L1 by retinal fibers in the optic tract and synaptic targets of the Syrian hamster.

Previous immunohistochemical studies in mouse, rat, and chick have reported that the expression of the glycoprotein and cell adhesion molecule L1, a member of the immunoglobulin superfamily, shows regulation during development of retina and optic nerve. To extend our understanding of the role of L1 in developing neural circuitry, we have examined L1 expression in the optic tract and thalamic and midbrain synaptic targets of retinal fibers in the early postnatal Syrian hamster, a well-characterized developmental model of the primary visual projection. Metabolic labeling studies reveal that a sy

Perigeniculate Giant Cell Tumor of Temporal Bone.

Giant cell tumors of bone (GCTBs) are benign osteolytic neoplasms that can be treated with either gross-total resection or subtotal resection with adjuvant radiotherapy. For the rare GCTB of the temporal bone, close proximity to critical structures can produce functional deficits and make gross-total resection difficult to achieve without significant morbidity. We present the case of a 28-year-old woman with progressive facial paresis, otalgia, neck pain, imbalance, and subjective hearing loss. She was found to have a facial nerve mass centered at the geniculate ganglion extending into the lab…

Facial nerve recording from the eardrum: a possible method for evaluating idiopathic facial nerve paralysis.

A noninvasive technique has been developed for recording from the the facial nerve within the fallopian canal. Following transcutaneous stimulation of the nerve on the face, an antidromic nerve potential can be detected with electrodes placed on the eardrum and enhanced by the technique of averaging. In studies conducted on cats and normal human subjects it has been determined that the primary recording site of the nerve potential is along the tympanic segment of the nerve just distal to the geniculate ganglion. Disturbances in nerve conduction caused by an experimental lesion produced changes…

Region-specific Brain Targets Drive Circuit Formation and Maturation of Human Retinal Ganglion Cells.

UNLABELLED: Human vision relies on retinal ganglion cells (RGCs), and their connectivity with distinct brain regions enables higher order visual processing. RGCs vary considerably between species, and small model organisms display distinct RGC innervation patterns from that in humans. There is thus a need for robust models of human RGC circuit formation that preserves innervation specificity. Here, we developed an in vitro microfluidics eye-to-brain connectivity model using human pluripotent stem cell (hPSC)-derived RGCs to assess brain region-specific connectivity features. We find that cultu

Retinal decussation patterns in pigmented and albino ferrets.

The decussation patterns of retinal ganglion cells in adult pigmented and albino ferrets were determined from the distribution of cells labelled after large unilateral injections of horseradish peroxidase into the visual pathway, involving the lateral geniculate nucleus and fibres of passage to the superior colliculus. About 6000 retinal ganglion cells project ipsilaterally in pigmented ferrets compared with only about 1500 in albino ferrets. In both strains, the vast majority of these cells (99 and 87% in pigmented and albino animals, respectively) are located in the temporal crescent, althou

Structure-function correlates in anterior visual pathway lesions: a systematic review.

Visual field defects allow for the localisation of pathologic aetiologies along the entire visual system. However, various aetiologies and location of damage might lead to field defect at similar areas. We reviewed the literature systematically for cases of optic neuropathy with well-defined lesions along the anterior visual pathway from the retina to the lateral geniculate with the goal of correlating specific visual field manifestations with the underlying pathology at each segment. We searched MEDLINE, Scopus, Cochrane CENTRAL, and Web of Science databases on June 9, 2024. We included studi

Dendritic changes in visual pathways in glaucoma and other neurodegenerative conditions.

Visual information is sent from the retina to central visual targets through the optic nerve formed of retinal ganglion cells' (RGCs) axons. In rodents, the superior colliculus (SC) is the major site of termination of retinal axons, whilst in primates and felines, it is the lateral geniculate nucleus (LGN). Glaucoma is a progressive optic neuropathy characterized by RGC death. There is increasing evidence that neuronal changes occur both in retina and central visual targets in glaucoma and other neurodegenerative diseases such as Alzheimer's disease (AD) and amyotrophic lateral sclerosis (ALS)
