A geriatric Labrador retriever dog was presented for acute collapse. The dog was conscious but lethargic, tachypneic, tachycardic with weak femoral pulses, occasional pulse deficits, and pale mucous membranes. Radiography, ultrasonography, quick assessment tests, and a complete blood (cell) count (CBC)/biochemistry panel indicated internal hemorrhage and potential problems with hemostasis. The dog was euthanized. A necropsy, histopathology, and immunohistochemistry for CD31 and Factor VIII-related antigen cell markers supported a diagnosis of splenic hemangiosarcoma.

Population aging has been accompanied by a growing burden of chronic medical conditions. Elderly individuals represent an increasing proportion of the total number of patients with chronic kidney disease, acute renal failure, and end-stage renal disease. In addition, age-related changes in the kidneys and other organ systems predispose the older person to disturbances in fluid and electrolyte balance, which exacerbate renal dysfunction. This article reviews the evolving epidemiology of renal disorders among the elderly and summarizes evidence put forward to explain this phenomenon.

Failure to recognize psychiatric conditions in any age group particularly in the elderly could lead to a very complicated clinical picture and may lead to increased health expenditures. Psychiatric emergencies can be secondary to numerous factors: geriatric depression and suicide, behavioral disturbance secondary to underlying organic conditions, substance abuse, elder abuse, and medication-induced adverse events. Older patients often suffer from sleep disturbances caused by age-related physiological changes, polypharmacy, changes in circadian rhythm, retirement, and loss of spouse. Many seniors have readily diagnosed sleep disorders that can be treated to achieve improvement in daytime somnolence or cognitive impairments. After a discussion of normal age-related changes in sleep, specific sleep pathologies common in the elderly will be reviewed, including sleep apnea, restless legs syndrome, advanced sleep phase syndrome, and rapid eye movement sleep behavior disorder.

EPIDEMIOLOGY: In long term geriatric facilities, the prevalence of nosocomial infections (to the order of 10% in the surveys) is greater than that observed in short-term medical care centers. An endemia of methicillin-resistant Staphylococcus aureus, of between 24 and 57% of hospitalized patients, is noted in many geriatric facilities. In decreasing order, the infections that predominate are: urinary (1/3), lower respiratory tract (1/4) and cutaneous tissue infections (1/5). However, these are often manifested by misleading semiology, and orient towards an organ other that which is infected.
