Aortic disease in giant cell arteritis.

The increasing use of non-invasive vascular imaging has allowed for improved detection of aortic disease in patients with vasculitis. Indeed, up to 70 % of patients have radiographic evidence of aortitis at GCA onset and aortic dilatation is already identified in about 15 %. Aortic inflammation is generally diagnosed by imaging studies such as CT angiography, MR angiography or FDG-PET, and occasionally by histopathology following resection of the aorta for aneurysm repair. Aortic inflammation may be clinically silent but is a risk factor for progressive aortic dilatation and aneurysm formation

Revisiting the epidemiology of giant cell arteritis.

In this article, we review the epidemiology of giant cell arteritis (GCA), the most common form of vasculitis in older adults, which primarily affects large and medium-sized arteries. We discuss various epidemiological aspects of GCA, including its incidence, genetic factors, the potential role of seasonality, environmental influences, and infections as possible triggers. Additionally, we explore its association with polymyalgia rheumatica and the significance of major complications, such as the development of aneurysmal disease in GCA, and the decline in the frequency of visual loss. The impa

Adrenal insufficiency in giant cell arteritis.

OBJECTIVES: To ascertain the frequency of adrenal insufficiency in patients with GCA treated using the Norwich prednisolone regimen. METHODS: Consecutive patients diagnosed with GCA between 1 January 2012 and 31 May 2022 were included. All patients were treated with the Norwich prednisolone regimen, educated about the benefits and risks of long-term prednisolone use and followed up in dedicated vasculitis clinics. When patients contacted the advice line to report being unwell, tests for adrenal function were performed after ruling out relapsing vasculitis or polymyalgia rheumatica. A 9 a.m. s

Updates in the Management of Giant Cell Arteritis.

PURPOSE OF THE REVIEW: To briefly review the latest updates in management in giant cell arteritis, an autoimmune vasculitis affecting the medium to large vessels. RECENT FINDINGS: Here, we review the known and newer trends in management of giant cell arteritis. While high dose glucocorticoids remain the mainstay of therapy, immunosuppressive medications are increasingly utilized to reduce the burden and risk of long-term glucocorticoid use. Published guidelines by the American College of Rheumatology (ACR) and European League Against Rheumatism (EULAR) suggest early use of steroid-sparing imm

[Giant-cell tumor of bone in 2022].

Giant cell tumors of bone (GCTs) are rare mesenchymal tumors classified as intermediate in the WHO 2020 classification, i.e. neither completely benign nor definitely malignant, due to recurrence (frequent) and pulmonary metastases (rare). They involve the end of long bones as well as the axial bones of mature skeletons. They are made of mononuclear stromal tumor cells of (pre-) osteoblastic phenotype, mononuclear cells of the monocyte-macrophage lineage and osteoclast-like multinuclear giant cells responsible for tumor osteolysis. In 95% of cases, the stromal cells have a specific mutation in …

An Unusual Case of Giant Cell Arteritis.

Giant cell arteritis (GCA), also known as temporal arteritis (TA), is a systemic autoimmune inflammation of medium and large arteries. It is the most common vasculitis affecting adults older than 50, with an incidence of 20/100,000 and an average age of onset of 70. Typically, patients initially present with new-onset headaches, visual changes and disturbances, jaw claudication, arthralgias, and tender or swollen temporal or occipital arteries. Our patient is a 73-year-old male who presented to the emergency room with 10 days of bilateral headache radiating to the occipital area associated wit

Lethal giant basal cell carcinoma.

Basal cell carcinomas are the most common malignancy in Caucasian populations with a very low predisposition to metastatic disease and an excellent prognosis if appropriately treated. Given the rarity of a lethal outcome two cases are reported. Case 1: A 61-year-old reclusive man who had an untreated facial basal cell carcinoma for 10 years died of hypothermia and sepsis complicating the extensively ulcerated and infected tumour. He also had underlying cardiomegaly, ischaemic heart disease and chronic obstructive pulmonary disease. Case 2: A 60-year-old man who had an untreated lower lumbar ba

[CME/Answers: Giant Cell Arteritis].

CME/Answers: Giant Cell Arteritis Abstract. Giant cell arteritis (GCA) is the most common vasculitis among patients over the age of 50. Mainly large vessels are targeted. GCA can be differentiated into cranial and extra-cranial types; thus the symptoms can range from headache, blurred vision and jaw claudication to non-specific symptoms like fatigue, polymyalgia and fever. Complications such as an irreversible loss of vision are critical, which is why timeous diagnosis and treatment are essential. There are some recommendations for treatment, but no defined guidelines exist. Steroids have been

Vision loss in giant cell arteritis.

Almost two-thirds of patients with giant cell arteritis (GCA) develop ocular symptoms and up to 30% suffer permanent visual loss. We review the three most common mechanisms for visual loss in GCA, describing the relevant ophthalmic arterial anatomy and emphasising how ophthalmoscopy holds the key to a rapid diagnosis. The short posterior ciliary arteries supply the optic nerve head, while the central retinal artery and its branches supply the inner retina. GCA has a predilection to affect branches of posterior ciliary arteries. The most common mechanism of visual loss in GCA is anterior arteri
