Gingival growths, barring a few are mostly reactive and seldom exhibit significant true neoplastic potential. The common etiology is local irritation from dental plaque/calculus, trauma as well as medication-related overgrowth. Such lesions are easily distinguishable and categorized into diagnoses such as pyogenic granuloma, peripheral ossifying fibroma, etc. We present a previously undescribed, but commonly encountered, reactive gingival growth with unique histologic features and suggest the diagnostic term "gingival fibroma." Idiopathic gingival overgrowth can present itself as a part of syndrome or as an isolated entity. A 52-year-old man presented with a chronic erosive gingivitis associated with eroded genital lesions which caused preputial adhesions. Histological examination of the gingival lesions showed typical changes of erosive lichen planus. A few months later, pseudo-peladic alopecia of the vertex and lichen planus lesions of the mouth appeared. Local steroids were ineffective to treat oral erosions, but the lesions were not painful.
