[Renal affection in gout].

The study of the problem of impairment of the kidneys in gout is important because investigation of gout nephropathy specifies the role of hyperuricemia (a known risk factor) in development of renal damage, on the one side, and emphasizes the role of early detection of hyperuricemic genesis of nephropathy allowing prognosis improvement due to administration of uricostatic treatment, on the other side. Earlier data on tubulointerstitial lesion are now supplied with information on essential glomerular damage due to endothelial dysfunction in the presence of insulin resistance often associated wi…

[Mechanisms of gout inflammation].

Gout inflammation is an acute and self-resolving reaction. MSU crystals can stimulate cells through either crystal-cell membrane interaction or after their phagocytosis. The onset of gout inflammation relies on non-hematopoietic resident cells whereas the amplification of the reaction is driven by phagocytic cells of immune innate system. Interleukin-1β (IL-1β) and polynuclear neutrophils play central role in gout inflammation. In vitro, MSU crystal-induced IL-1β secretion is secondary mainly to NLRP3 inflammasome activation although numerous proteases are also involved. Mechanisms of NLRP3 in…

[The epidemiology and genetic of gout].

Gout affects 1-2% of adults in developed countries, where it is the most common inflammatory arthritis in men. Epidemiological data are consistent with a rise in the prevalence of gout. Diet and genetic predisposition seem to be the main causal factors of primary gout. Gout and hyperuricaemia are associated with hypertension, diabetes mellitus, metabolic syndrome, and renal and cardiovascular diseases. Genetic studies have found that variants of GLUT9 and ABCG2 were associated with urate levels.

[Current management of gout].

Gout, one of the most prevalent rheumatic diseases in the world, results from deposition of uric acid crystals in several locations, particularly in joints, subcutaneous tissues and kidney. The classical treatments, although effective, are often poorly tolerated or contraindicated. Recently, new drugs have emerged for the treatment of hyperuricemia and gout, including febuxostat and uricase, which proved to be quite promising. Some drugs already used in other diseases, such as losartan, atorvastatin, fenofibrate and amlodipine also seem to have a role in monitoring the serum uric acid. This ar…

Progress in the pharmacotherapy of gout.

PURPOSE OF REVIEW: To summarize new knowledge on approved and emerging drugs used to treat hyperuricemia or the clinical manifestations of gout. RECENT FINDINGS: Results of several clinical trials provide new data on the efficacy and safety of the approved urate-lowering drugs, allopurinol and febuxostat. New recommendations have been presented on appropriate dosing of colchicine for acute gout flares and potential toxicities of combining colchicine with medications such as clarithromycin. Emerging therapies, including pegloticase, the uricosuric agent RDEA596, and the interleukin-1 inhibitor…

Gout and its comorbidities.

Gout is a chronic disease in which excessively high levels of serum urate (hyperuricemia) result in tissue depositions of sodium urate crystals and intermittent inflammatory attacks. Patients who have gout frequently experience a range of comorbidities, which complicates management and affects long-term prognosis. We review some of the more important of these comorbidities and consider the extent to which gout or hyperuricemia may be either a consequence or a cause of these related conditions. In addition, we briefly consider several neurological conditions in which the presence of gout or a h…

Colchicine for the treatment of gout.

IMPORTANCE OF THE FIELD: Colchicine, used for a long period in gout, was approved for the first time in 2009 by the FDA for the prophylaxis and the treatment of acute attack, on the basis of a pivotal trial that showed the efficacy in the very short term - that is 24 h of a well-tolerated, low-dose regimen of Colcrys (colchicine, URL Pharma, Philadelphia, USA) to reduce pain in patients with acute gout - when given early. AREAS COVERED IN THIS REVIEW: We searched the Cochrane Library and Medline for articles published in English from January 2000 to August 2010 using the following search term…

Compliance in gout patients.

INTRODUCTION: Despite its relative high prevalence, potential devastating clinical consequences and socio-economic impact, the existence of effective drugs to treat it, and the well recognised direct relation between acute flares and treatment interruptions and its resumption, gout is still often considered the chronic disease with the worst rate of adherence to therapy. The reason for this is unknown. We proposed to thoroughly evaluate a subgroup of patients, aiming at identifying the clinical features predictive of non-compliance, and 5 different ways to assess those. METHODS: We analysed a …

Epidemiology of gout: an update.

Gout is the most common inflammatory joint disease in men, characterised by formation of monosodium urate (MSU) crystals in the synovial fluid of joints and in other tissues. The epidemiology of gout provides us with the understanding of the disease distribution and its determinants. In an attempt to update the knowledge on the topic, more recent research reports on the descriptive epidemiology of gout are reviewed in this article. The review describes clinical characteristics and case definitions of gout, including the Rome and New York diagnosis criteria of gout, '1977 American Rheumatism As…

Pegloticase for chronic gout.

BACKGROUND: Pegloticase is a potential new treatment option for patients with chronic gout intolerant to other urate-lowering therapies. OBJECTIVES: To assess safety (adverse events, death) and efficacy (pain, function, frequency of flares, quality of life, uric acid level, radiographic damage) of pegloticase in various doses or as compared to placebo or other interventions for treatment of hyperuricemia in patients with chronic gout. SEARCH STRATEGY: We searched six databases: The Cochrane Central Register of Controlled Trials (CENTRAL), via The Cochrane Library, OVID MEDLINE, CINAHL (via E…

The modern management of gout.

Gout is an inflammatory arthritis characterized by self-limiting but excruciatingly painful acute attacks. These are a consequence of monosodium urate (MSU) crystals being deposited within articular or periarticular tissue. Chronic tophaceous gout can develop after years of acute intermittent gout. Recent discoveries, including the role of the inflammasome and intracellular events demonstrating that pro-inflammatory cytokines, IL-1 beta, -8 and TNF-alpha, promote neutrophil influx. Also, genetic advances with the identification of the URAT-1 transporter and genetic variation in SLC 2A9 as a ke…

A brief history of gout.

Gout is relatively unique among rheumatological diseases in that its effects were common and, at least in history, disproportionately represented in prominent authority figures. Effective treatments have only been available for just over a century and therefore for the most part, the disease has run relatively unchecked. Consequently, it may have had global ramifications. The history of gout, from apocryphal orthodoxy to medical clarity, is reviewed, using a few of the more colourful characters in its considerable pantheon.
