Actinobaculum schaalii - invasive pathogen or innocent bystander? A retrospective observational study.

BACKGROUND: Actinobaculum schaalii is a Gram-positive, facultative anaerobic coccoid rod, classified as a new genus in 1997. It grows slowly and therefore is easily overgrown by other pathogens, which are often found concomitantly. Since 1999, Actinobaculum schaalii is routinely investigated at our hospital, whenever its presence is suspected due to the detection of minute grey colonies on blood agar plates and negative reactions for catalase. The objective of this study was to determine the clinical significance of Actinobaculum schaalii, identified in our microbiology laboratory over the las

MALDI-TOF mass spectrometry following short incubation on a solid medium is a valuable tool for rapid pathogen identification from positive blood cultures.

INTRODUCTION: Rapid identification of the causative microorganism is a key element in appropriate antimicrobial therapy of bloodstream infec … ood cultures, in 72.7% one of the pathogens was correctly identified. Results were not reliable for Gram-positive rods and yeasts. The approach was easy to implement in diagnostic routine. In cases with available clinical data and successful pathogen identification, in 51.1% our approach allowed an optimized treatment recommendation. CONCLUSION: MALDI-TOF MS follow …

First Korean case of Robinsoniella peoriensis bacteremia in a patient with aspiration pneumonia.

Robinsoniella peoriensis has recently been identified as a Gram-positive, spore-forming, anaerobic rod originally recovered from swine manure storage pits. To date, 6 cases of R. peoriensis infection have been reported, including 2 cases of bacteremia, 1 of abdominal fluid collection, and 3 of wound infection. In the present study, we report a 76-yr-old man with R. peoriensis bacteremia who developed aspiration pneumonia. Gram staining of a purified colony revealed Gram-positive, rod-shaped bacteria. Biochemical identification using API 20 A (bioMérieux, France) indicated presence of Clostridi

Corynebacterium mustelae Endocarditis in a Dog.

A 7-year-old male neutered Labrador Retriever dog presented with acute-onset fever, shifting limb lameness and anorexia, with development of acute respiratory distress. At necropsy, there was vegetative endocarditis, which effaced the aortic valve. Gram staining of impression smears from the aortic valve and kidney revealed numerous gram-positive rods with some coryneform bacteria. Similar coryneform bacteria were isolated on aerobic culture of the aortic valve. Identification was attempted by matrix-assisted laser desorption ionization time-of-flight mass spectrometry and 16S sequencing, the …

Presumed clostridial and aerobic bacterial infections of the cornea in two horses.

Microscopic examination of Gram-stained tissue specimens collected from severe corneal ulcers in 2 horses revealed large gram-positive rods suggestive of Clostridium spp. Clostridium perfringens was isolated from specimens collected from horse 1; anaerobic organisms were not detected in specimens from horse 2. Aerobic bacterial culture revealed Aeromonas hydrophila and Enterobacter cloacae in specimens collected from horses 1 and 2, respectively. An insect exoskeleton was presumed to be the underlying cause of ulceration in horse 1. Cause of ulceration in horse 2 was not determined. Antibiotic…

Actinomyces turicensis infection mimicking ovarian tumour.

This case report explores an unusual presentation of a commensal organism, Actinomyces, which mimicked a presentation of ovarian cancer. A 73-year-old woman presented to a tertiary level hospital with persistent left iliac fossa abdominal pain, anorexia and fever lasting over one week, with a three-month history of bright rectal bleeding. Imaging was suggestive of malignancy. Fine needle aspiration of an enlarged lymph node was non-diagnostic. Blood cultures taken at presentation became positive after two days for Gram-positive rods, which were most likely Actinomyces. The patient was treated …

A rare cause of bacteremia in a pediatric patient with Down syndrome: Sphingomonas paucimobilis.

Sphingomonas paucimobilis, is a yellow-pigmented, aerobic, non fermentative, gram negative motile bacillus. S. paucimobilis which is widely found in nature and hospital environments rarely cause serious or life threatening infections. In this report, a case of hospital acquired bloodstream infection due to S. paucimobilis in a patient with Down syndrome who was on treatment for presumed pneumonia is presented. A one year-old child patient who was a known case of Down syndrome and had previously experienced cardiac surgery was hospitalized and treated for pneumonia. On the 12th day of hospitali

Scanning electron microscopy of black stain on human permanent teeth.

Black stain may develop on the coronal surfaces of human teeth, and this type of stain is common in the Hong Kong Chinese population. The present study was undertaken to ascertain if the deposit conforms in composition to the black stain found elsewhere, and to describe its ultrastructure using the scanning electron microscope. Gram-stained smears were made from black stain on the teeth of 11 adult Hong Kong Chinese and studied microscopically. From another 15 persons extracted teeth exhibiting black stain were obtained and fixed. Two ground sections were made from each tooth, one was stained

A prospective longitudinal study of "Candidatus arthromitus"-associated rainbow trout gastroenteritis in the UK.

Rainbow trout gastroenteritis (RTGE) is an emerging disease of farmed rainbow trout (Oncorhynchus mykiss) reported in Croatia, France, Italy, Spain and the UK. The impact of RTGE at the site-level varies, and daily mortalities of 0.5-1% are common. Gross lesions include severe enteritis with congestion and oedema, and the segmented filamentous bacterium "Candidatus arthromitus" has been suggested as a possible aetiological agent. This paper presents findings from a prospective longitudinal epidemiology study that took place from June to November 2006 in 12 RTGE-positive UK sites, examining the

An evaluation of three processing methods and the effect of reduced culture times for faster direct identification of pathogens from BacT/ALERT blood cultures by MALDI-TOF MS.

Matrix-assisted laser desorption/ionisation time-of-flight mass spectrometry (MALDI-TOF MS) is a fast and reliable method for the identification of bacteria from agar media. Direct identification from positive blood cultures should decrease the time to obtaining the result. In this study, three different processing methods for the rapid direct identification of bacteria from positive blood culture bottles were compared. In total, 101 positive aerobe BacT/ALERT bottles were included in this study. Aliquots from all bottles were used for three bacterial processing methods, i.e. the commercially

Anthrax of the eyelids.

Anthrax is a disease caused by Bacillus anthracis. The disease affects primarily herbivores including sheep, cattle, horses, and other domestic animals. Humans may rarely be affected. We examined one male and two female patients with a localised itchy erythematous papule of the eyelid. A necrotising ulcer formed in each of the three cases resulting in a black lesion. Scraping in each case showed Gram positive rods and culture grew Bacillus anthracis. All three patients responded to the intravenous administration of penicillin G, and the lesion resolved leaving scars in two cases. Anthrax is a

Postoperative gas gangrene.

Gas gangrene is a rare complication of elective surgery. Caused by several species of Clostridia, it is an acute, highly invasive necrotizing infection of muscle and fascia often leading to coma and death. Gas gangrene should be suspected if the patient has unremitting pain, high temperature, tachycardia, and prostration. Edema, brown watery exudate emanating from he wound, evidence of gas in tissues, and gram-positive rods on smear support the clinical diagnosis. Rapid surgical decompression and debridement is the basis of treatment. Adjuvant antibiotic, hyperbaric oxygen, antitoxin, local, a…
