INTRODUCTION: As highly active antiretroviral therapy (HAART) transforms human immunodeficiency virus (HIV) into a manageable chronic disease, new challenges are emerging in treating children born with HIV, including a number of risks to their physical and psychological health due to HIV infection and its lifelong treatment.
METHODS: We conducted a literature review to evaluate the evidence on the physical and psychological effects of perinatal HIV (PHIV+) infection and its treatment in the era of HAART, including major chronic comorbidities.
RESULTS AND DISCUSSION: Perinatally infected chil

OBJECTIVE: To describe the clinical, epidemiological and molecular evidence for transmission of HIV-1 infection from a person with unrecognized HIV infection to a family member in two unconnected families where the route of transmission could not be conclusively determined.
DESIGN: Case studies, molecular analysis of viral strains and a clinical and laboratory investigation of risk factors for transmission.
SETTING: State referral centres for HIV/AIDS in two Australian teaching hospitals.
RESULTS: Previously unrecognized HIV-1 infection was diagnosed in two unconnected females following blo

Drug addicts have, in general, a less complicated course of HIV infection than homosexual HIV patients. They show fewer opportunistic infections and tumors. But this advantage is lost by unnecessary complications due to their psychic disorders. Their non-compliance and concealment of signs of disease lead to worse outcomes of infections, which could be well-treated or prevented.

IMPORTANCE: Pre-exposure prophylaxis (PrEP) is a proven effective intervention to reduce the risk for HIV infection. Critically, changes in policies that lead to increased out-of-pocket PrEP costs or that decrease access to proximate PrEP locations could reduce PrEP coverage, resulting in excess HIV infections and costs.
OBJECTIVE: To estimate how decreases in PrEP coverage that would be likely results of federal policy changes may be associated with new HIV infections and their costs.
DESIGN, SETTING, AND PARTICIPANTS: In this economic evaluation, US population-based data sources were used …

BACKGROUND: Sexually transmitted infections (STIs) including Chlamydia trachomatis (CT), Neisseria gonorrhoeae (NG), Treponema pallidum (TP), and cytomegalovirus (CMV) may lead to adverse pregnancy and infant outcomes. The role of combined maternal STIs in HIV mother-to-child transmission (MTCT) was evaluated in mother-infant pairs from NICHD HPTN 040.
METHODOLOGY: Urine samples from HIV-infected pregnant women during labor were tested by polymerase chain reaction (PCR) for CT, NG, and CMV. Infant HIV infection was determined by serial HIV DNA PCR testing. Maternal syphilis was tested by VDRL

The magnitude of intestinal parasitic infection in human immunodeficiency virus (HIV) /AIDS patients requires careful consideration in the developing world. However, there have been very few studies addressing this issue in Ethiopia. This study was conducted to determine the prevalence of intestinal parasitic infection in HIV/AIDS patients at Jimma Hospital, Southwest Ethiopia, between January and February 2002. Stool specimens from HIV/AIDS patients and control groups were screened for intestinal parasitic infections using direct and formalin-ether sedimentation concentration methods. Out of

Human Immunodeficiency Virus Type 1 (HIV-1) continues to pose a significant global public health challenge. Marked inter-individual variability in susceptibility to infection, viral load, and disease progression has long highlighted an important role for host genetic factors. To date, only a small number of germline loci show consistent, reproducible effects on HIV-1 control, most notably CCR5 and HLA class I genes. Recent studies have refined the mechanistic basis of these associations, clarifying how CCR5 regulation and specific HLA-B residues shape CD8⁺ T-cell and natural killer cell respon

In the summer of 1981, young gay men began falling ill and dying of opportunistic infections. The strange thing was that these infections should have been easily rebuffed by their immune systems (Greene WC, Eur J Immunol 37(Suppl 1):S94-S102, 2007). These men become emaciated and developed dark purple lesions primarily on their arms and faces. These lesions were due to Kaposi's sarcoma, a relatively rare and aggressive form of cancer (Antman K and Chang Y, N Engl J Med 342:1027-1038, 2000). Physicians in cities such as New York, Los Angeles, and San Francisco were dumbfounded by the presence o

Rhabdomyolysis is a rare but potentially life-threatening complication of acute HIV infection. We present a case report of a young adult male who presented with fever, myalgia, and elevated creatine phosphokinase levels, ultimately diagnosed with acute HIV infection-associated rhabdomyolysis. This case highlights the importance of considering HIV infection in the differential diagnosis of rhabdomyolysis, particularly in at-risk populations, even in the absence of typical HIV-related symptoms.

Multiple infection of target cells by human immunodeficiency virus (HIV) may lead to viral escape from host immune responses and drug resistance to antiretroviral therapy, bringing more challenges to the control of infection. The mechanisms underlying HIV multiple infection and their relative contributions are not fully understood. In this paper, we develop and analyze a mathematical model that includes sequential cell-free virus infection (i.e.one virus is transmitted each time in a sequential infection of target cells by virus) and cell-to-cell transmission (i.e.multiple viral genomes are tr

PURPOSE OF REVIEW: Immune-mediated adverse drug reactions (IM-ADRs) are many times more common in HIV-infected patients. Usual offending drugs include antiretroviral and antiinfectives, but the burden of specific drug IM-ADRs is population-specific; changing as new and fixed dose combinations enter the market, and drug-resistance patterns demand. This review considers recent literature on epidemiology, mechanisms, clinical management and prevention of IM-ADRs amongst persons living with HIV/AIDS.
RECENT FINDINGS: Epidemiological studies continue to describe high rates of delayed hypersensitiv

Tuberculosis (TB) has recently surpassed HIV as the primary infectious disease killer worldwide, but the two diseases continue to display lethal synergy. The burden of TB is disproportionately borne by people living with HIV, particularly where HIV and poverty coexist. The impact of these diseases on one another is bidirectional, with HIV increasing risk of TB infection and disease progression and TB slowing CD4 recovery and increasing progression to AIDS and death among the HIV infected. Both antiretroviral therapy (ART) and latent TB infection (LTBI) treatment mitigate the impact of coinfect

In Europe depending on the country 15-80 % of all individuals infected with human immunodeficiency virus (HIV) are either not aware of the diagnosis or are diagnosed later. An early HIV diagnosis could, however, considerably improve the prognosis of individuals infected with HIV and decrease the risk of new infections; therefore, in the presence of indicator diseases, such as sexually transmitted diseases, oral thrush, herpes zoster and lymphoma, the performance of a HIV test is of utmost importance. A newly diagnosed HIV infection represents an indication for starting antiretroviral combinati

Opportunistic infections have been reported infrequently in primary HIV infection. We report a case of cryptococcemia in primary HIV infection. To our knowledge there has not been such a case reported. Our case highlights the need for clinicians to be wary of other opportunistic infections, including cryptococcosis, in primary HIV infection.
