[Heart rhythm variability of healthy newborns with congenital heart defects].

Short-term changes in heart rate (heart rate variability) are seen at every age and represent an important sign of normal homeostatic mechanisms of the cardiovascular system. The influence of premature birth on the maturation of the autonomic nervous system was established by studying heart rate variability in 12 pre-term newborns. The results were compared with a group of 88 full-term newborns and with a group of 14 newborns with congenital heart defects. Heart rate variability was measured by spectral- and time-domain analysis of interbeat intervals in 4-hour continuous ECG recordings. In co…

Home inotrope treatment for intractable heart failure following heart transplantation.

A 49 year old man developed intractable heart failure three years after undergoing heart transplantation. Coronary angiography showed no evidence of graft vascular disease. An initial cardiac biopsy identified one episode of rejection which responded to augmented immunosuppressive treatment. The patient became inotrope dependent and has now survived at home for 22 months using an ambulatory delivery system for intravenous adrenaline (epinephrine), without significant complications. There has been a noticeable improvement in symptoms and left ventricular systolic performance, both clinically an…

[Heart injury. Description of a traumatologic heart surgery patient sample].

Thoracic injuries can always involve cardiac structures. If the heart is affected, penetrating or blunt injuries need urgent treatment. An unstable cardiac situation with shock symptoms is life-threatening. The diagnostic method of choice is ultrasound examination. This can show pericardial effusion, enlargement of cardiac chambers, wall motion changes and intracardiac defects. Pathologic ultrasound and ECG findings or abnormal enzyme values arising after blunt traumas need intensive care monitoring. Surgical interventions should be done immediately by the first surgeon involved: time should n…

Valvular heart disease. A correctable cause of congestive heart failure.

Surgical repair of mitral, aortic, and tricuspid valves and balloon valvuloplasty for mitral stenosis are being done more and more often. These complex procedures require accurate and high-quality imaging techniques to guide the physician in decision making before surgery and in assessment of success during surgery. Dr Currie describes the use of echocardiography in diagnosis and management of patients with various forms of valvular heart disease.

Does fetal heart rate predict subsequent heart rate in childhood?

An investigation to determine whether there is any relationship between extremes of fetal heart rate during labour and subsequent heart rate at the age of 10 was carried out using data from the 1970 cohort of British Births. In 11,000 nationally representative children it was found that low fetal heart rate (below 120 beats/min) was associated with a heart rate at age 10 which was significantly lower than in those children whose fetal heart rate had remained between 120 and 160 beats/min (P less than 0.01). This relationship could not be explained by fetal asphyxiation, maternal antenatal hypo…

Permanent cardiac pacing after open-heart surgery: acquired heart disease.

Retrospective review of 5,942 patients who underwent open-heart surgery for acquired heart disease revealed that 123 patients (2.1%) required permanent cardiac pacing postoperatively; 4.6% of these underwent predominantly valvular surgery and 0.6% had coronary bypass. The most important factors appeared to be: 1) preoperative evidence of a conduction disorder; 2) advanced patient age; 3) dense calcium in the aortic annulus; 4) valvular surgery and, especially, tricuspid valve surgery; and 5) poor myocardial protection. Postoperative permanent pacing had a considerable impact on patient morbidi…

Outcomes of adults with congenital heart disease undergoing heart and heart-liver transplantation: A single-center experience.

OBJECTIVE: The number of adults with congenital heart disease (ACHD) continues to rise, often presenting with multiorgan dysfunction. This is the largest single-center series reporting outcomes of ACHD who underwent either heart transplantation (HT) or combined heart-liver transplantation (CHLT). METHODS: Adults (aged 18 years and older) with congenital heart disease were identified in retrospective review. All-cause mortality was compared in a Cox-proportional hazards model. Mann-Whitney and χ2 tests were used to analyze differences between groups. RESULTS: Between 1987 and 2024, 81 ACHD un…

Improved 2-year heart transplant survival with moderate hypothermic donor heart preservation in the GUARDIAN heart registry.

BACKGROUND: Multiple advanced preservation technologies are now available and have demonstrated utility in organ assessment and preservation. The Paragonix SherpaPak Cardiac Transport System (SCTS) has become the most common method of static preservation, demonstrating superior outcomes to historic ice storage. To date, no preservation method has reported improved post-transplant survival. METHODS: Data from the GUARDIAN-Heart Registry, the largest real-world registry focused on organ preservation, were analyzed to quantify post-transplant clinical outcomes and survival in transplant cases ut…

Rationale and design of the Optimal Macro-Nutrient Intake Heart Trial to Prevent Heart Disease (OMNI-Heart).

BACKGROUND: The DASH (Dietary Approaches to Stop Hypertension) diet is a carbohydrate-rich, reduced-fat diet that lowers blood pressure (BP) and LDL-cholesterol. Whether partial replacement of some carbohydrate (C) with either protein (P) or unsaturated fat (U) can further improve these and other cardiovascular (CVD) risk factors is unknown. METHODS: OmniHeart is a randomized, three-period, crossover feeding study designed to compare the effects on BP and blood lipids of a carbohydrate-rich diet (CARB, similar to the DASH diet) with a diet rich in protein (PROT, predominantly from nonmeat sou…

Heart transplantation for end-stage heart failure combined with Q fever isolated to the heart: a case report.

BACKGROUND: Active infection is generally considered a contraindication for heart transplantation. The rare combination of a patient with an active Coxiella burnetii infection and a congenital corrected transposition of the great arteries requiring heart transplantation impose challenging treatment decisions. We would like to demonstrate that if Q fever is restricted to the heart only, heart transplantation is also beneficial from an infectious point of view, therefore treating two severe conditions simultaneously. CASE SUMMARY: A patient with end-stage heart failure due to congenital correct…

A prospective survey of patients with valvular heart disease in Europe: The Euro Heart Survey on Valvular Heart Disease.

AIMS: To identify the characteristics, treatment, and outcomes of contemporary patients with valvular heart disease (VHD) in Europe, and to examine adherence to guidelines. METHODS AND RESULTS: The Euro Heart Survey on VHD was conducted from April to July 2001 in 92 centres from 25 countries; it included prospectively 5001 adults with moderate to severe native VHD, infective endocarditis, or previous valve intervention. VHD was native in 71.9% of patients and 28.1% had had a previous intervention. Mean age was 64+/-14 years. Degenerative aetiologies were the most frequent in aortic VHD and mi…

Change in heart rate and heart rate variability during treatment for depression in patients with coronary heart disease.

OBJECTIVE: Major depression is a common problem in patients with coronary heart disease (CHD) and is associated with an increased risk for cardiac morbidity and mortality. It is not known whether treating depression will improve medical prognosis in patients with CHD. Depression is also associated with elevated heart rate and reduced heart rate variability (HRV), which are known risk factors for cardiac morbidity and mortality that may explain the increased risk associated with depression. The purpose of this study was to determine whether treatment for depression with cognitive behavior thera…

Pathology of cardiac transplantation: recipient hearts (chronic heart failure) and donor hearts (acute and chronic rejection).

Pathologic examination of both the recipient and the donor heart is critical to the success of cardiac transplantation. Idiopathic dilated cardiomyopathy and end-stage ischemic heart disease are still the most common diagnoses for which cardiac transplantation is performed in the United States. After transplantation, the donor heart is susceptible to immunologic (acute and chronic rejection), infectious, and ischemic injury. Other long-term changes, such as hypertrophy of the myocytes and interstitial fibrosis, may also affect the function of the transplanted heart. Periodic endomyocardial bio…

[Noise and the heart].

Traffic-related environmental noise represents a widespread and relevant environmental factor in Europe that is associated with a substantial burden of disease. Current estimates indicate that a significant proportion of the population is exposed to noise levels linked to adverse health effects. Cardiovascular diseases are among the most extensively studied outcomes in this context. Epidemiological studies consistently demonstrate associations between long-term exposure to traffic noise and an increased risk of arterial hypertension, ischemic heart disease, myocardial infarction, heart failure…

SPeak to the heart.

Neuroimmune regulation modulates responses to cardiovascular stress and injury. In this issue of Immunity, Perrotta et al. delineate a heart-brain-spleen axis that induces adaptive cardiac remodeling in response to pressure overload, highlighting a SPeak mechanism (spleen-derived PlGF efflux activates cardiac macrophages).
