[Corrected transposition of the great arteries in patients over 65].

Three cases with corrected transposition of the great arteries in patients older than 65 years are described. Two had atrial situs solitus and one atrial situs inversus. One had pulmonary valvular stenosis with valvular calcification and a small ventricular septal defect in association with ischemic heart disease. This patient died due to cardiac failure at the age of 80 years. The second case was associated with ventricular septal defect, atrial septal defect and pulmonary hypertension. The third patient presented with mild tricuspid regurgitation. Although this congenital heart malformation

[Non-surgical closure of type-II atrial-septal defect and patent foramen ovale].

In the past the standard therapy for significant atrial septal defect was surgical closure. In the presence of a patent foramen ovale with otherwise unexplained (cryptogenic) cerebral embolism, the therapy was oral anticoagulation or antiplatelet therapy. Surgery was considered only in the event of recurrence. Percutaneous transcatheter occlusion of atrial septal defect and patent foramen ovale is a new and valuable alternative to surgical closure. This report presents current knowledge and our own experience of nonsurgical closure of atrial septal defects and patent foramen ovale.

Assessment of atrioventricular septal defects by two dimensional echocardiography.

Ninety-six patients with an atrioventricular septal defect were assessed by two dimensional echocardiography. Forty-eight patients were judged as having two discrete valve orifices. In this group, 37 had an intact ventricular septum and 11 a ventricular septal defect. Two patterns of attachment of the valve leaflets to the interventricular septum were noted. Forty-eight patients had a common valve orifice. In 29 patients there was absence of tethering of the anterior bridging leaflet to the interventricular septum. A further 19 had varying degrees of tethering of the anterior bridging leaflet.

Heterogeneity of genetic modifiers ensures normal cardiac development.

BACKGROUND: Mutations of the transcription factor Nkx2-5 cause pleiotropic heart defects with incomplete penetrance. This variability suggests that additional factors can affect or prevent the mutant phenotype. We assess here the role of genetic modifiers and their interactions. METHODS AND RESULTS: Heterozygous Nkx2-5 knockout mice in the inbred strain background C57Bl/6 frequently have atrial and ventricular septal defects. The incidences are substantially reduced in the Nkx2-5(+/-) progeny of first-generation (F1) outcrosses to the strains FVB/N or A/J. Defects recur in the second generati

An interatrial tunnel: a rare form of atrial septal defects.

Atrial septal defects (ASDs) account for approximately 6%-10% of congenital heart defects. The well-known types of atrial septal communications are the ostium secundum, ostium primum, sinus venosus types, and coronary sinus defects. A 50-year-old female was referred for TEE for better assessment of MR severity and mechanism. 2D/3D-TEE showed a rare combination of different abnormalities; bi-leaflet mitral valve prolapse, cleft P2, cor triatriatum sinister, and a tunnel-shaped IAS communication. To the best of our knowledge, this is a very rare case with a rare form of atrial septal defect that

A case of left atrial myxoma associated with atrial septal defect.

Cardiac myxoma is the most frequent primary tumor of the heart. However, it is rarely associated with congenital cardiac anomalies such as atrial septal defect in the literature. We present a 72-year-old woman referred to the emergency department with loss of consciousness and finally diagnosed as a pedinculated mobile left atrial myxoma and concomitant occurrence of an ostium secundum type atrial septal defect. The mass was successfully excised, and atrial septal defect was safely repaired by primary suture. The patient is currently well after surgery. Atrial myxoma should be considered in th

Self- and informant-reported executive function in young adults operated for atrial or ventricular septal defects in childhood.

BACKGROUND: Adults with simple congenital heart defects (CHD) have increased risk of neurodevelopmental challenges including executive dysfunction. It is unknown if the executive dysfunction is universal or if it is driven by dysfunction in specific clinical subscales and how it might affect psychosocial aspects of everyday life. METHODS: The self-reported and informant-reported executive function of adults with an average age of 26 ± 5 (range 18-41) who underwent childhood surgery for atrial septal defects (n = 34) or ventricular septal defects (n = 32) and matched controls (n = 40) were eva

CE: Long-term outcomes after repair of congenital heart defects: part 1.

Many congenital heart defects can be repaired, but long-term monitoring is often required to forestall possible complications. This two-part article reviews 10 common congenital heart defects, their repairs, and their common long-term outcomes, along with the implications for nurses in both cardiac and noncardiac settings. Here, in part 1, the author reviews six defects: bicuspid aortic valve, atrial septal defect, ventricular septal defect, atrioventricular septal defect, coarctation of the aorta, and pulmonic stenosis.

[Large ostium secundum atrial septal defect discovered after severe mitral regurgitation. A case report].

UNLABELLED: Mitral regurgitation is associated with ostium secundum atrial septal defect in about 22% of cases. mitral valve prolapse induced by atrial shunt is the main cause of this regurgitation. Ususually, atrial septal defect discovery precedes that of mitral regurgitation. The aim of this paper is to focus on clinical, hemodynamic and evolutive details of atrial septal defect and mitral regurgitation association. We report the case of large atrial septal defect in 37 years old girl referred for hemodynamic investigation of mitral regurgitation. The divergence of clinical data, electrocar

Balanced Atrio Ventricular Canal Defect: Distilling the Data.

Complete atrioventricular canal defect (CAVC) is the seventh most common congenital heart defect, affecting 1 in 1712 live births. Embryologically CAVC defect arises from failure of normal fusion of the endocardial cushions resulting in a spectrum of abnormalities that involve the atrial septum, atrioventricular (AV) valve, subvalvar apparatus, and the ventricular septum. This article will discuss historic timelines, evolution of management strategies, and outcomes following current surgical approaches in well-balanced CAVC. Recent advances in genetics and imaging techniques will also be discu

Morphologic features of atrioventricular septal defect with only ventricular component: further observations pertinent to surgical repair.

OBJECTIVE: Atrioventricular septal defect with only a ventricular component of septal deficiency is the least common of the various forms of this malformation. We reviewed its morphology in heart specimens and compared our findings with the other forms for a better understanding of its surgical morphology. METHODS: We examined 78 cardiac specimens with atrioventricular septal defect; 56 (72%) had common atrioventricular valve orifice with both atrial and ventricular components (so-called "complete" form), and 22 (28%) had separate valve orifices (so-called "partial" or "incomplete" form) with

[Absence of right and left atrioventricular connexion].

Fifty seven hearts with absence of atrioventricular (A-V) connection were studied morphologically to specify their types of ventriculoarterial connection and their associated anomalies; the anatomic features of the hearts were correlated with their echocardiographic and cardioangiographic images in order to establish their mutual correspondence. Fifty six hearts had situs solitus; fifty specimens had right absent A-V connection and six had left absent A-V connection. One had situs inversus. All the specimens had: A deep A-V sulcus at the site of the absent A-V connection, a dimple in the muscu

Atrial septal defect and ventricular septal defect.

In summary, ASD and VSD sometimes do not require surgical intervention and have a good-to-excellent prognosis in animal patients in which surgical intervention is necessary. Animal patients with these diseases often do not show symptoms, but those that do and cannot be adequately managed medically are challenging but rewarding cardiac surgical cases.
