The Eisenmenger malformation: a morphologic study.

We studied 11 autopsied cases of the Eisenmenger malformation, comparing the findings with 11 hearts with intact ventricular septal structures, and nine hearts having perimembranous ventricular septal defects in the absence of aortic overriding. We found variable lengths for the subpulmonary infundibulum in the hearts with Eisenmenger defects. It was well developed in three hearts, of intermediate length in seven, and very short in one of the specimens. The muscular outlet septum was also of variable length compared with the free-standing subpulmonary infundibular sleeve. Except for one, all h

Genetic Insights into Congenital Cardiac Septal Defects-A Narrative Review.

Congenital heart diseases (CHDs) are a group of complex diseases characterized by structural and functional malformations during development in the human heart; they represent an important problem for public health worldwide. Within these malformations, septal defects such as ventricular (VSD) and atrial septal defects (ASD) are the most common forms of CHDs. Studies have reported that CHDs are the result of genetic and environmental factors. Here, we review and summarize the role of genetics involved in cardiogenesis and congenital cardiac septal defects. Moreover, treatment regarding these c

Closure of a muscular ventricular septal defect using the Amplatzer ventricular occluder.

Surgical closure of ventricular septal defect is safe, however, the inherent risks associated with cardiopulmonary bypass and the potential early and late postsurgical complications including complete heart block, arrhythmias, postpericardiotomy syndrome, and rare deaths have led physicians to prefer transcatheter occlusion technique for closure of such defects. The use of Amplatzer muscular ventricular septal defect occluder is safe and associated with a higher success rate than other previously used devices. For the first time in Iran, we report an 18-year-old patient with ventricular septal

Descriptive epidemiology of selected congenital heart defects, Hawaii, 1986-1999.

Congenital heart defects are the most common type of birth defect and contribute the most to infant mortality due to birth defects. This study examined the relationship between several demographic factors and selected congenital heart defects among the unique multiethnic population in Hawaii during 1986-99, using data from a population-based birth defects registry. Rates were significantly higher in 1993-99 than in 1986-92 for transposition of the great arteries and Ebstein's anomaly, and significantly lower for tetralogy of Fallot. Significantly elevated rates were found with maternal age of

Management of the ventricular septal defect during double switch operation for atrioventricular discordant connections.

Management of the ventricular septal defect at the time of the double switch operation can be challenging and deserves careful attention to the anatomic details of ventricular septal defect location, alignment with the appropriate subtended ventricles and their respective atrioventricular valves, and the disposition of the specialized conduction tissue. These anatomic details can differ considerably from hearts with concordant atrioventricular alignments, and further variations of right-left geometry and septal alignment can be exhibited. A case is described that underscores some of the partic

Circulatory adjustments after birth: effects on ventricular septal defect.

The haemodynamic effects and clinical manifestations of congenital heart lesions may be greatly influenced by developmental changes in the circulation. The sequential changes associated with abnormal communications between the systemic and pulmonary circulations are well typified by the effect on ventricular septal defect. The possible influences of ventricular septal defect on the foetal circulation are first presented, and the interrelationship between ventricular septal defect and postnatal adjustments of the circulation are discussed.

Evaluation of multiple ventricular septal defects using three-dimensional reconstruction models†.

OBJECTIVES: Although there has been rapid development in the field of three-dimensional morphological analyses of congenital heart disease, with the three-dimensional volume-rendered images providing visualization of the external vascular anatomy, the precise reproduction of 'Swiss-cheese' ventricular septum is not well established. We created three-dimensional printed models and computer graphics based on multi-slice computed tomography of patients with complex multiple ventricular septal defects for surgical decision planning of this difficult cardiac defect. METHODS: Seven patients with co

The left ventricular outflow tract in atrioventricular septal defect revisited: surgical considerations regarding preservation of aortic valve integrity in the perspective of anatomic observations.

OBJECTIVE: The anatomy of the left ventricular outflow tract in hearts with atrioventricular septal defect has been widely investigated, but controversies remain regarding detailed aspects of left ventricular outflow tract anatomy in the perspective of operative techniques to either prevent or relieve outflow tract obstruction. METHODS: We investigated 29 postmortem hearts with an atrioventricular septal defect. Measurements were taken of the circumferences and of the widths of the components that make up the outflow tract, that is, the interventricular septum, the superior bridging leaflet,

Percutaneous closure of a complex posttraumatic muscular ventricular septal defect.

We report the percutaneous closure of a complex traumatic ventricular septal defect resulting from a stab wound to the anterior chest. The patient presented with heart failure 3 months after the initial trauma. Evaluation revealed a new large ventricular septal defect with diminished cardiac function. Closure was accomplished with a combination of an Amplatzer muscular ventricular septal defect occluder and two Amplatzer vascular plugs.

Aortopulmonary window with pumonary atresia with ventricular septal defect with D-transposition of great arteries: extremely rare anomaly.

Aortopulmonary window (APW) is rare a congenital heart disease accounting for 0.1%-0.2% of all congenital heart defects. The 35% of the APW has been associated with wide variety of other structural heart diseases such as ventricular septal defect, persistent ductus arteriosus, arch anomalies and coronary artery anomalies. To the best of our knowledge, only six cases of APW with pulmonary atresia with ventricular septal defect has been described in the literature. It resembles the type 1 truncus arteriosus, and differentiation from this condition is important prior to surgical correction. We pr

Changes in serum natriuretic peptide levels after percutaneous closure of small to moderate ventricular septal defects.

Background. B-type natriuretic peptide has been shown to be a very sensitive and specific marker of heart failure. In this study, we aimed to investigate the effect of percutaneous closure of ventricular septal defects with Amplatzer septal occluders on brain natriuretic peptide levels. Methods. Between 2008 and 2011, 23 patients underwent successfully percutaneous ventricular septal defect closure in 4 cardiology centers. Brain natriuretic peptide levels were measured in nine patients (4 male, mean ages were 25.3 ± 14.3) who underwent percutaneous closure with Amplatzer occluders for membrano

Surgical anatomy of the infundibular septum in transposition of the great arteries with ventricular septal defect.

Anatomic variation of the infundibular septum was studied in transposition of the great arteries with ventricular septal defect in 23 hearts and double-outlet right ventricle with anterior position of the aorta in two hearts. Anterior displacement of the infundibular septum (i.e., "false" Taussig-Bing heart) was associated with coarctation or interruption of the aortic arch in 88% of the cases, whereas posterior displacement resulted in subpulmonary narrowing in 100% of the cases. Anterior displacement makes intraventricular rerouting from the left ventricle to the aorta difficult because of a

Open-heart surgery in 48 patients via a small right anterolateral thoracotomy.

To limit the trauma to the chest and to achieve a pleasing cosmetic result, we used 2 types of right anterolateral thoracotomy in 48 patients who required open-heart surgery: 1 was a curved incision along the lower edge of the right breast in women with developed breasts; the other was a slanted incision for men and children. These surgical procedures took place between July 1996 and November 1997. Intraoperatively, a right atriotomy was used to repair 11 atrial septal defects and 11 ventricular septal defects, 2 combined atrial and ventricular septal defects, 1 case of a single atrium, and 1

Heterogeneity of genetic modifiers ensures normal cardiac development.

BACKGROUND: Mutations of the transcription factor Nkx2-5 cause pleiotropic heart defects with incomplete penetrance. This variability suggests that additional factors can affect or prevent the mutant phenotype. We assess here the role of genetic modifiers and their interactions. METHODS AND RESULTS: Heterozygous Nkx2-5 knockout mice in the inbred strain background C57Bl/6 frequently have atrial and ventricular septal defects. The incidences are substantially reduced in the Nkx2-5(+/-) progeny of first-generation (F1) outcrosses to the strains FVB/N or A/J. Defects recur in the second generati

Cantrell's syndrome in an infant.

Cantrell's syndrome is a rare combination of congenital midline thoracoabdominal defects and congenital heart disease. surgical repair can correct many of the anomalies. We report a neonate who presented with a short sternum, defective upper abdominal muscle, ectopic umbilicus, anterior diaphragmatic defect, ventricular septal defect and an intra-abdominal left ventricular diverticulum. Initially, the infant was treated with digoxin and diuretics for 5 months. Due to poor control of heart failure, one-stage surgical correction was performed with patch repair of the ventricular septal defect, l

Transposition of the great arteries with ventricular septal defects. Surgical considerations concerning the Rastelli operation.

We studied the anatomy of the ventricular septal defect in 20 heart specimens and eight operated patients with transposition of the great arteries regarding the feasibility of the Rastelli operation. They were divided into three groups. In Group I, comprising eight cases, creation of a left ventricle-aorta connection was not prevented by interposition of the atrioventricular valve tissue, and the ventricular septal defect was large or could be enlarged sufficiently. Thus, the Rastelli operation was feasible in all cases. In Group II, comprising 12 cases, interposition of the atrioventricular v

M-mode and two-dimensional echocardiography--contributions and limitations in management of interatrial and interventricular septal defects and conotruncal anomalies.

The relative contributions of M-mode and two-dimensional echocardiography to the management of a congenital cardiac anomaly reflects their individual sensitivity to the physiologic and anatomic alterations of the lesions. The integration of these imaging techniques should not be based on their historical relationship. Two-dimensional imaging has, in most cardiac laboratories, become the cardiac ultrasound technique of choice for patients suspected of having defects in the atrial and ventricular septum or anomalies of the conotruncus. Two-dimensional examination of intra- and extracardiovascula

Perimembranous and muscular ventricular septal defects--morphology revisited in the era of device closure.

Transcatheter closure of ventricular septal defects has advanced rapidly with improvements in device designs. We review the morphology of ventricular septal defects in heart specimens for a closer look at the shapes, sizes, and proximity to important structures. The three main types of defects, perimembranous, muscular, doubly committed and juxta-arterial, all have variable morphologies within each group. Oval rather than round shapes are more common. The shapes of muscular defects, in particular, can change from one side of the septum to the other. Perimembranous defects are close to the aort
