Height and heart disease.

Height has a relationship with a number of medical conditions, including heart disease. Atrial fibrillation has been observed to be more common in taller individuals. Marfan syndrome, with its high prevalence of mitral valve disease and abnormalities of the aorta, is associated with increased height. Mitral valve prolapse in patients without Marfan syndrome may be more common in taller people. Conversely, congestive heart failure, coronary artery disease, and possibly aortic valve calcification are less prevalent with increasing height. The relationship between height and health will be of inc

Echocardiography of cardiac valves in pericardial effusion.

Nine adult patients with large pericardial effusions (PE) demonstrated echocardiographic motion abnormalites of cardiac valves in systole. In four cases (Group 1), the abnormal findings consisted of prominent systolic anterior movements of the mitral valve resembling those seen in idiopathic hypertrophic subaortic stenosis. In Group 2(five cases), typical mitral valve prolapse patterns with large posterior midsystolic displacements well below the C point were observed. Additional abnormalities in Group 2 included tricuspid valve prolapse patterns (four cases), early systolic movement of the ao

Repair of posterior mitral valve prolapse with a novel leaflet plication clip in an animal model.

OBJECTIVE: Recently, there has been increased interest in minimally invasive mitral valve prolapse repair techniques; however, these techniques have limitations. A new technique was developed for treating mitral valve prolapse that uses a novel leaflet plication clip to selectively plicate the prolapsed leaflet segment. The clip's efficacy was tested in an animal model. METHODS: Yorkshire pigs (n = 7) were placed on cardiopulmonary bypass (CPB), and mitral valve prolapse was created by cutting chordae supporting the P2 segment of the posterior leaflet. Animals were weaned off CPB and mitral r

[Current possibilities in time-motion echocardiography. Part 2: Cardiomyopathies, right-left ventricular volume overload, coronary disease, selected echocardiographic diagnosis, prosthetic heart valves].

Important information, needed in the judgement of congenital or acquired heart disease, can be obtained by means of time-motion-echocardiography. It is also occasionally a main factor in the diagnosis of pericardiac effusion, cardiac tumors, mitral valve prolapse or hypertrophic obstructive cardiomyopathy. One of the key values is the possibility for undangerous and numerous use in repeated examinations needed in observing and evaluating a patient's development or recovery from a cardiomyopathy or after placement of a prosthetic heart valve.

Mitral valve prolapse syndrome in children with Duchenne's progressive muscular dystrophy.

Myocardial function was evaluated prospectively by noninvasive methods in 20 boys with clinical, biochemical, muscle biopsy, and electromyographic evidence of Duchenne's progressive muscular dystrophy. Auscultatory evidence of a nonejection systolic click suggested mitral valve prolapse (MVP) syndrome in seven patients. Phonocardiography disclosed that the click was mid-systolic in four patients and early in three. Echocardiographic features consistent with this diagnosis were identified in all seven patients and in an additional four. One of these had an apical pansystolic murmur, suggestive

Ventricular arrhythmias and mitral valve prolapse in childhood.

Although ventricular arrhythmias are well described in adults with mitral valve prolapse, this association remains controversial in children. To assess the incidence of ventricular premature complexes (VPCs), 103 consecutive children with mitral valve (MV) prolapse confirmed by echocardiography were evaluated prospectively by treadmill exercise and ambulatory ECG. A group of 50 normal children with clinical, ECG, or echocardiographic evidence of heart disease who had undergone treadmill exercise and ambulatory ECG served as controls. In the group with MV prolapse, 16 patients had VPCs with tre

The relationship between prolapsing mitral leaflet syndrome and angina and normal coronary arteriograms.

Patients with a prolapsing mitral leaflet frequently have chest pain while their coronary arteriograms are normal. In this regard, these patients are similar to the group of patients with angina and normal coronary arteriograms. In the present study, clinical, electrocardiographic, cardiac hemodynamic, angiographic, and metabolic findings in 20 patients with a prolapsing mitral leaflet were compared to those of 16 patients with angina and normal coronary arteriograms. Except for the presence of mitral leaflet prolapse and systolic clicks, the findings were similar in both groups. We postulate

Long-term outcomes of artificial chordal replacement with tourniquet technique in mitral valve repair: a single-center experience of 700 cases.

OBJECTIVE: Artificial chordal replacement has been shown to be effective and durable, with numerous techniques reported. However, the outcomes of each technique have remained poorly defined. We report the long-term outcomes of the tourniquet technique. METHODS: We reviewed the data from 700 patients who had undergone mitral valve repair with the tourniquet technique from 1992 to 2010. We analyzed the operative outcomes, long-term survival rate, freedom from reoperation, and freedom from recurrent moderate or severe mitral regurgitation (MR). We also performed Cox regression analysis to explor

Determinants of early decline in ejection fraction after surgical correction of mitral regurgitation.

OBJECTIVE: We sought to echocardiographically examine the early changes in left ventricular size and function after mitral valve repair or replacement for mitral regurgitation caused by leaflet prolapse. METHODS: Preoperative and early postoperative echocardiograms of 861 patients with mitral regurgitation caused by leaflet prolapse who underwent mitral valve repair or replacement (with or without coronary revascularization) were studied. Among the patients, 625 (73%) were men and 779 (90%) had mitral valve repair. RESULTS: The rate of valve repair increased from 78% in the first decade of t

All anterior and bileaflet mitral valve prolapses are repairable in the modern era of reconstructive surgery.

OBJECTIVE: Although mitral valve repair is the preferred treatment for degenerative mitral valve disease, valve replacement still remains prevalent, particularly in the setting of anterior leaflet prolapse. We sought to determine the feasibility and mid-term durability of a lesion-based surgical strategy applied systematically in a consecutive and nonexclusionary (all comers) series of patients with degenerative mitral valve disease and either isolated anterior leaflet or bileaflet prolapse. METHODS: From January 2002 to December 2010, 188 consecutive patients [mean age 56 ± 14 years (range 1

Demonstration of mitral valve prolapse with CT for planning of mitral valve repair.

Mitral valve prolapse (MVP), the most frequent cause of severe nonischemic mitral regurgitation, often warrants surgical or interventional valve repair. The severity of mitral regurgitation positively correlates with the development of heart failure and death. Even in patients who are asymptomatic, severe mitral regurgitation causes higher rates of death, heart failure, and atrial fibrillation. Repair procedures for mitral regurgitation have progressed to include leaflet repair, chordal transfer, ring or band annuloplasty, and new percutaneous procedures. In planning for mitral valve repair, d

Isolated pulmonary valve prolapse: a sign of pulmonary hypertension.

Isolated pulmonary valve prolapse may be a sign of pulmonary hypertension. Three patients with pulmonary hypertension as a result of left ventricular failure, chronic obstructive pulmonary disease, and primary pulmonary hypertension, respectively, are described in the case reports. It is likely that the morphological change of the pulmonary valve is due to exaggeration of the normal convexity of the elastic pulmonary leaflets as a result of a high pulmonary artery diastolic pressure. This two-dimensional echocardiographic sign may prove to be a useful qualitative hallmark for pulmonary hyperte

Recommendations for the management of individuals with acquired valvular heart diseases who are involved in leisure-time physical activities or competitive sports.

Physical check-ups among athletes with valvular heart disease are of significant relevance. In athletes with mitral valve stenosis the extent of allowed physical activity is dependant on the size of the left atrium and the severity of the valve defect. Patients with mild-to-moderate mitral valve regurgitation can participate in all types of sport associated with low and moderate isometric stress and moderate dynamic stress. Patients under anticoagulation should not participate in any type of contact sport. Asymptomatic athletes with mild aortic valve stenosis can take part in all types of spor

Aberrant right ventricular branch of right coronary artery with mitral valve prolapse.

A 37-year-old man presented with a three-week history of chest pain. Transthoracic echocardiography demonstrated a mitral valve prolapse and mild mitral insufficiency. Coronary angiography showed normal left main, circumflex, left anterior descending and right coronary arteries; however, the right ventricular branch of the right coronary artery had a separate ostium. Concomitant congenital heart abnormalities have been observed with coronary artery anomalies. Primary congenital coronary and valvular anomalies may have genetic heredity. In the present case, mitral valve prolapse was accompanied

Two women presenting aborted sudden cardiac arrest as the first event of mitral valve disease.

Mitral valve prolapse (MVP) is a relatively common valvular heart disease and is known to have a benign course. However, a certain subtype of MVP has a pathologic prognosis and can be accompanied by malignant cardiac arrhythmia causing sudden cardiac arrest, which can be characterized by bileaflet mitral valvular thickening and prolapse and frequent premature ventricular ectopic activity upon electrocardiography. Herein, we present two patients with bileaflet mitral prolapse who survived aborted sudden cardiac arrest. These cases show a precise MVP diagnosis that may prevent a devastating life

Real-time 3-dimensional echocardiography provides new insight into mechanisms of tricuspid valve regurgitation in patients with hypoplastic left heart syndrome.

BACKGROUND: Tricuspid regurgitation in hypoplastic left heart syndrome has an impact on outcome, but its mechanisms remain unclear. METHODS AND RESULTS: Real-time 3-dimensional echocardiography was performed in 35 patients with hypoplastic left heart syndrome (age, 1 month to 10 years; 10 after first-stage Norwood, 12 after superior cavopulmonary shunt, 13 after Fontan). From the 3-dimensional data set, we marked the annulus in systole and diastole. At mid systole, we marked the location of the papillary muscle tip and point of chordal attachment to the leaflet. We traced the surfaces of the

"The Starry Sky." A new intraprocedural three-dimensional echocardiographic technique.

The NeoChord procedure is a micro-invasive, trans-ventricular, beating-heart chordal replacement technique for patients with severe degenerative mitral valve regurgitation resulting from prolapsed or flail leaflets. Three dimensional transoesophageal echocardiographic guidance is crucial to assist the operator during the procedure. Equidistant placement of neochordae along the free edge of the prolapsing leaflet segment is important to ensure uniform stress distribution and to avoid damaging any of the previously placed neochordae. Lowering the image gain associated with the three-dimensional

Considerations on the mechanism of the systolic click of mitral valve prolapse.

The Authors briefly discuss the mechanism of production of the systolic click of mitral valve prolapse. A "valvular" mechanism seems inadequate to explain the genesis of vibrations that can be recorded, not only in the external phonocardiogram, but also in the intraventricular pressure tracing, in the apex cardiogram, and even in the left atrial pulse (esophagus). It seems more logical to postulate that the force of deceleration created by the sudden eversion of a mitral leaflet set the whole cardiohemic system (blood, myocardial walls, and the mitral apparatus) into vibration, thus producing
