BACKGROUND: Hemorrhoids are an extremely rare condition in children, and data on its incidence and treatment in the pediatric population remains scarce. We retrospectively reviewed children who underwent sclerotherapy for internal hemorrhoids, and analyzed patients' characteristics and outcomes.
METHODS: A total of 14 pediatric patients who underwent sclerotherapy were included. Patients' ages and the required amount of polidocanol, depending on the grade of hemorrhoids, and the correlation between age and volume of sclerosant, were statistically analyzed.

We present an unusual case of advanced hemorrhoidal disease in an 57-year-old man presenting with anemia, persistent bleeding, anal protrusion. Conventional hemorrhoidectomy was not feasible because of the risk of a massive hemorrhage caused by hypertrophic protruded vascular anal cushions. A two-stage resection was then undertaken. The aim of the first procedure was to reduce the vascularization of the protruded mucosa by a circular series of interrupted mass stitches. Hemorrhoidal banding is a well-established and safe outpatient procedure. Septic complications of hemorrhoidal banding are rare but can be fatal. The first case of pylephlebitis (septic portal vein thrombosis) and pyogenic liver abscess following hemorrhoidal banding in a 49-year-old man with diabetes is reported in the present study. Risk factors, management and the role of prophylaxis in immunocompromised patients are discussed. Caution against hemorrhoidal banding in immunosuppressed patients, including patients with diabetes, is warranted.

Rubber band ligation is a commonly employed office procedure for the eradication of symptomatic internal hemorrhoids. Since 1980 increasingly frequent reports of an often fatal complication--"pelvic cellulitis"--have appeared. Death has been avoided for some of these patients by early recognition and treatment. One such survivor is reported here, in a case report that illustrates the value of early recognition of symptoms and appropriate diagnostic and therapeutic intervention.

In 90 preparations of the human rectum, venous vessels were studied. In all age groups, newborns and children including, trabecular veins with cone-shaped protrusions, intimal "cushons" and microvalves were revealed. In the columnar zone, on the lateral walls of the anal canal, they form cavernous bodies in arterio-venous anastomoses. In elderly and old-aged persons, as a result of myoelastofibrosis, thinning of the cavernous vein walls and partial hollowing of their lacunae take place.
