Healthcare Providers' Experiences with Hiccups in Patients with Cancer: Report of a United States National Survey.

Hiccups occur in 15-40% of cancer patients, but previous research has not sought the perspectives of cancer healthcare providers. The objective of this research is to report on United States cancer healthcare providers' awareness of their patients' hiccups and these healthcare providers' perceived need for further palliation options. A survey was developed and then distributed throughout the United States via email to cancer healthcare providers; results are reported descriptively. Six hundred eighty-four cancer healthcare providers completed 2 eligibility screening questions which required th…

Intractable hiccups and neck pain due to left C4 radiculopathy decreased with posterior foraminotomy.

BACKGROUND: Intractable hiccups can last for more than 1-2 months and can occur, as shown in this case study, due to cervical nerve root compression. CASE DESCRIPTION: A 76-year-old male presented with intractable hiccups and neck pain of 7 months' duration. The patient underwent magnetic resonance imaging studies of the entire neuraxis. The only abnormality found was on the cervical magnetic resonance images that demonstrated left C4 nerve root compression due to the C3- C4 lateral/foraminal osteophyte. Following a left-sided C3-C4 laminoforaminotomy, the hiccups and the neck pain improved. …

Persistent hiccups and vomiting with multiple cranial nerve palsy in a case of zoster sine herpete.

A 76-year-old man came to our hospital complaining of hiccups and vomiting lasting for five days. A neurological examination showed dysfunction of cranial nerves V, VII, VIII, IX and X on the left side. Cerebrospinal fluid polymerase chain reaction for varicella zoster virus-DNA was positive. The patient responded well to treatment with intravenous acyclovir and steroids. To the best of our knowledge, this is the first case report of zoster sine herpete presenting with persistent hiccups and vomiting. It is important to keep in mind that herpes zoster can present with symptoms that closely res…

Persistent hiccup after surgical resection of a brainstem arteriovenous malformation: a case successfully treated with gabapentin during rehabilitation. Case report.

Persistent hiccup rarely occurs during rehabilitation, but its management can prove to be very difficult, particularly in presence of associated dysphagia, requiring longer hospitalization and higher risk of severe clinical complications. We present a case of persistent hiccup after surgical resection of a brainstem arteriovenous malformation successfully treated with gabapentin during rehabilitation.

When Hiccups Speak Louder Than Cough: A Case Report of An Unusual Presentation of Lung Abscess.

Hiccups are commonly benign, self-limiting events often triggered by transient gastrointestinal or central nervous system irritants. However, when persistent, they may indicate underlying pathology involving the thoracic, gastrointestinal, or neurological systems. We report the case of a 65-year-old male smoker who presented with a 4-day history of persistent hiccups. He denied fever, chest pain, hemoptysis, or other systemic symptoms. Physical examination was largely unremarkable except for decreased air entry in the right middle and lower lung zones. Initial investigations, including gastros…

Unilateral change of laryngeal adductor reflex and hiccups resolution after removal of bulbar hemangioblastoma: is there a connection?

Intractable hiccups, although typically benign, occasionally present as a clinical challenge requiring deeper investigation. We report a case of persistent hiccups as the sole manifestation of a cystic hemangioblastoma within the medulla oblongata. Conventional hiccup treatments were ineffective, prompting surgical intervention. We employed intraoperative neurophysiological monitoring of the Laryngeal Adductor Reflex (LAR) during the procedure, revealing intriguing asymmetries in reflex responses. At the end of the surgical excision, we recorded a significant LAR decrease on the left side, wit…

Nasocardiac reflex-induced resolution of persistent hiccups via intranasal swab stimulation: A case report.

Hiccups are a common respiratory reflex experienced by most individuals at some point in their lives and are typically self-limiting. In emergency department treatment, vagal maneuvers are usually the first-line approach; if unsuccessful, pharmacologic therapies are considered. We present a case of persistent hiccups successfully resolved through vagal stimulation induced by the nasocardiac reflex following nasopharyngeal stimulation with an intranasal swab. A 31-year-old female presented to the emergency department with a 72-h history of hiccups. Prior to arrival, she had attempted various va…

Influence of fetal hiccups on Doppler blood flow waveform of fetal arteries: Difference among arteries.

BACKGROUND: A few studies have been reported on the influence of fetal hiccups on umbilical artery. The aim of this study is to clarify the influence of fetal hiccups on Doppler blood flow waveform (DBFW) of some fetal arteries, and to show the difference in these influences among fetal arteries. OBJECTIVE: DBFW of umbilical artery, descending aorta, and middle cerebral artery were recorded at hiccups in normal fetuses between 34th and 40th gestational weeks. The changes on DBFW were classified into three shapes by the direction and the size of the changes. Shape 1: sharp decrease but not to …

Phrenic nerve block combined with stellate ganglion block for postoperative intractable hiccups: a case report.

Postoperative intractable hiccups slow patient recovery and generate multiple adverse effects, highlighting the importance of investigating the pathogenesis and terminating the hiccups in a timely manner. At present, medical and physical therapies account for the main treatments. We encountered a case in which postoperative intractable hiccups after biliary T-tube drainage removal ceased with the application of an ultrasound-guided block of the unilateral phrenic nerve and stellate ganglion. No complications developed, and the therapeutic effect was remarkable. To our knowledge, this approach …

Persistent Hiccups Induced by Supratentorial Infarcts and Successful Treatment With a Combination of Perampanel and Baclofen: A Case Report.

CASE: A 52-year-old man developed a cerebral infarction from the right middle cerebral artery occlusion, and the infarction extensively damaged the right insula. Three months after the onset of the cerebral infarction, persistent hiccups appeared, occurring during sleep. The thoracic and abdominal cavities showed no lesions; hence, the hiccups were considered to be caused by central nervous system dysfunction. Administration of metoclopramide, chlorpromazine, and diazepam were ineffective, while levetiracetam had a partial effect. Combining perampanel with baclofen finally suppressed the sympt…

Intractable Hiccups After Coil Embolization of Partially Thrombosed Posterior Inferior Cerebellar Artery Aneurysm.

BACKGROUND: Hiccups are defined as sudden-onset involuntary contractions of the diaphragm followed by immediate inspiration and laryngeal closure, and they are considered intractable if prolonged beyond 1 month. A reflux arc involving phrenic, vagal, and central midbrain modulation is likely responsible for hiccups. We herein report a case of intractable hiccups caused by compression of the dorsal aspect of the medulla oblongata after treatment of a partially thrombosed distal posterior inferior cerebellar artery (PICA) aneurysm. CASE DESCRIPTION: A 51-year-old man presented with severe heada…

[Area postrema lesion as a cause of intractable nausea, vomiting and hiccups in neuromyelitis optica spectrum disorders].

Neuromyelitis optica (Devic's disease) is a chronic autoimmune disease associated with the production of anti-bodies to aquaporin-4 (AQP4). Area postrema lesions is the third, after optic neuritis and myelitis, syndrome of opticomyelitis-related disorders. Clinical symptoms of this disorder include intractable nausea, vomiting and hiccups. In many cases, area postrema syndrome manifests as the first clinical symptom of a neuromyelitis optica spectrum disorder that hampers the diagnosis. The authors present a case report of a female patient with area postrema lesions developed several months be…

A Therapeutic Paradox: Intractable Hiccups in Acute Stroke and Vascular Parkinsonism Following Initiation of Carbidopa/Levodopa.

Intractable hiccups are an uncommon but clinically significant condition that can lead to sleep disturbance, fatigue, impaired oral intake, and reduced quality of life. The hiccup reflex arc involves complex peripheral and central pathways, with dopaminergic signaling believed to play a modulatory role. In neurologically complex patients, distinguishing medication-related hiccups from structural central nervous system etiologies remains challenging. We present a 94-year-old male with a history of vascular Parkinsonism and multiple comorbidities who was admitted for acute ischemic stroke involv…

Successful Management of Intractable Hiccups in Diabetic Ketoacidosis: A Case Report on the Efficacy of Inhaled Nebulized Lidocaine.

Intractable hiccups are a rare but distressing symptom that can significantly impair a patient's quality of life, particularly in those with underlying medical conditions such as diabetic ketoacidosis (DKA). This case report describes a 59-year-old male patient with a history of type 2 diabetes mellitus and hypertension who presented to the emergency department with persistent hiccups lasting for five days, coinciding with an exacerbation of his diabetes leading to DKA. Despite initial treatments with metoclopramide, chlorpromazine, and baclofen, the hiccups persisted, causing considerable dis…

Dorsal medullary cavernous hemangioma presenting as obstinate hiccups and its surgical treatment: illustrative case.

BACKGROUND: Hiccups are characterized by involuntary, intermittent, repetitive, myoclonic, and spasmodic contractions of the diaphragm. Hiccups are termed "intractable" when they last for over 1 month. OBSERVATIONS: A rare case of intractable hiccups due to an uncommon location of cavernous hemangioma in the dorsal medulla is illustrated. With respect to the management, surgical excision was performed, and postsurgical complete recovery was witnessed, which has been reported only in six cases worldwide to date. LESSONS: A mechanism of the hiccups reflex arc is discussed in detail with specia…

Hemophagocytic Lymphohistiocytosis Caused by a Severe Epstein-Barr Virus Infection in a Young Patient Presenting With Hiccups.

Hemophagocytic lymphohistiocytosis (HLH) is a life-threatening hyperinflammatory syndrome characterized by a pathologic immune response in the setting of infection, malignancy, acute illness, or any immunological stimulus. Infection is the most common etiology of HLH. HLH involves aberrant activation of lymphocytes and macrophages with resultant hypercytokinemia due to an inappropriately stimulated and ineffective immune response. Here, we present the case of a previously healthy 19-year-old male presenting with hiccups and scleral icterus, who was found to have HLH due to a severe Epstein-Bar…
