The pathophysiology of primary hyperparathyroidism.

The parathyroid glands play a key role in maintaining near constancy of the extracellular calcium concentration ( Ca(o)2+) through their capacity to sense even minute changes in the level of blood calcium from its normal level. The G protein-coupled, Ca(o)(2+)-sensing receptor (CaSR) is the mechanism through which the parathyroid chief cells senses changes in Ca(o)2+. In primary hyperparathyroidism (PHPT), Ca(o)2+ is reset upward from its normal level. This defect likely arises from increases in both the mass of pathological parathyroid tissue as well as the set point for Ca(o)(2+)-regulated p…

Unusual hyperparathyroidism in a cat.

A 5 month-old, male, domestic short hair cat was presented with inappetence and vomiting. it was depressed and reluctant to move. The cat had difficulties in keeping the standing position and grossly deformed thighs. Lytic changes and disruption of normal architecture of the bone were observed, involving mainly the femoral diaphyses. An inverse Ca/P ratio and kidney failure were diagnosed. The possibility of whether the bone changes could have been related to primary or secondary renal hyperparathyroidism is discussed.

Pathogenesis of anaemia in hyperparathyroidism.

It is suggested that parathyroid hormone (PTH), when in excessive amounts, interferes with normal erythropoiesis by downregulating the erythropoietin receptors on erythroid progenitor cells in the bone marrow. Therefore, physiologic concentrations of EPO can no longer sustain normal red cell counts, so normocytic and normochromic anaemia ensues. In primary hyperparathyroidism (HPT), this effect is observed with very high concentrations of PTH. In secondary HPT during chronic renal failure, this effect is more pronounced because erythropoietin synthesis is impaired.

[Results of surgery for hyperparathyroidism].

Since 1983, we have performed 110 parathyroidectomies in 109 patients. The hyperparathyroidism was diagnosed as primary in 102 (93.6%) patients, secondary in 6 (5.5%), and tertiary in 1 (0.9%). Sixty-one patients (55.9%) had single adenomas, 2 (1.8%) had double adenomas, 44 (40.3%) had hyperplasia and 1 (0.9%) had parathyroid cancer. Sixty-five adenomas were resected (59.6%). Subtotal parathyroidectomy was performed in 34 cases (31.2%) and total parathyroidectomy with autotransplant in 10 cases (9.2%). In two cases, primary surgery did not solve hypercalcemia; one underwent successful surgery …

The prevention of secondary hyperparathyroidism.

It has become increasingly evident that clear-cut disturbances of bone and mineral metabolism develop early in renal failure. Among these disturbances, hyperparathyroidism is well documented and is usually asymptomatic at that early stage. It is now accepted that early therapy using phosphate restriction, through diet and calcium-containing phosphate binders, and 1 alpha-hydroxylated vitamin D analogues is an effective means of preventing or even reversing hyperparathyroidism in early renal failure. The response to these therapies is both functional (reduced parathyroid hormone secretion) and …

Primary hyperparathyroidism in a dog.

A 6-year-old male Standard Poodle had hypercalcemia, hypophosphatemia, and a urinary concentrating defect. A parathyroid adenoma involving the left caudal parathyroid gland was surgically removed. Transient hypocalcemia followed, which resolved without treatment. A renal biopsy did not demonstrate renal parenchymal mineral deposition. Urinary concentrating ability returned after surgery.

The changing face of hyperparathyroidism.

Despite its earlier reputation as a relentlessly progressive disease, hyperparathyroidism may actually remain asymptomatic for many year. If its possible presence is signaled by hypercalcemia and supported by other laboratory findings, surgery is often indicated because no medical treatment is effective, and long-term follow-up of this frequently malignant condition is difficult; the surgical cure rate is about 95%.

Surgery for Normocalcemic Hyperparathyroidism.

Primary hyperparathyroidism is the most common cause of hypercalcemia, accounting for about 90% of all cases. This disorder is characterized by overactive parathyroid glands, leading to increased parathyroid hormone and excess serum calcium.

Bone and primary hyperparathyroidism.

Primary hyperparathyroidism (PHPT) is a disease caused by excessive and inappropriate secretion of parathyroid hormone resulting in hypercalcemia. It is usually diagnosed incidentally in case of hypercalcemia, osteoporosis or, more rarely, renal involvement such as lithiasis. The clinical presentation reflects hypercalcemia and involves several organs, mainly the cardiovascular system, bone, and kidneys. However, most patients with PHPT are asymptomatic. The diagnosis is biological, obvious when serum calcium and parathyroid hormone levels are high, but difficult when one of these two values i…

Tertiary hyperparathyroidism: a review.

Tertiary hyperparathyroidism (HPT III) occurs when an excess of parathyroid hormone (PTH) is secreted by parathyroid glands, usually after longstanding secondary hyperparathyroidism. Some authorities reserve the term for secondary hyperparathyroidism that persists after successful renal transplantation. Long-standing chronic kidney disease (CKD) is associated with several metabolic disturbances that lead to increased secretion of PTH, including hyperphosphatemia, calcit-riol deficiency, and hypocalcaemia. Hyperphosphatemia has a direct stimulatory effect on the parathyroid gland cell resulting…

Primary Hyperparathyroidism: Redefining Cure.

Primary hyperparathyroidism is the most common cause of hypercalcemia. Follow-up can be resource-intensive and costly. The aim of this study was to determine if there is a subset of patients who can be defined cured earlier than six months. This was a retrospective study of patients who underwent parathyroidectomy between January 2012 and March 2014. Patients with a history of multiple endocrine neoplasia syndrome, and secondary or tertiary hyperparathyroidism were excluded. Patients with normal preoperative calcium and parathyroid hormone (PTH) and those without six months follow-up were excl…
