[Primary hyperparathyroidism. Pathogenesis--diagnosis--therapy].

A report is given on 33 patients with primary hyperparathyroidism who underwent surgical treatment in our institution in the period from 1980 to 1989. In 57 per cent of the cases a hypercalcaemic syndrome was present. The primary hyperparathyroidism manifested itself in 75 per cent at the kidneys in the form of nephrolithiasis. One third of the patients showed a renal manifestation form exclusively. Remarkable is the relatively high number of exclusively gastrointestinal manifestations (11 per cent). In all patients the diagnosis was established on the coincidence of hypercalcaemia and increas

Primary hyperparathyroidism: problems in management.

Most hyperparathyroidism is subclinical, with no complaints of bone pain, constipation, mental confusion, or depression, no skeletal findings on x-ray, and no history of kidney stones. Routine hyperparathyroidectomy for asymptomatic hypercalcemia, with normal bone density and normal calciuria, particularly with moderate elevations of serum calcium, is now generally rejected.

[Primary hyperparathyroidism. Mechanisms of hypercalcemia].

In primary hyperparathyroidism, hypercalcaemia is due to inappropriate hypersecretion of parathormone (PTH). Yet, the intestinal or osseous origin of the excess in plasma calcium and the symptoms of the disease are largely conditioned by vitamin D reserve and metabolism. In cases with sufficient vitamin D reserve and normal metabolism, the primary disorder is hyperabsorption of calcium by the intestine, and there is a risk of renal stone formation. In patients with vitamin D deficiency, there is a significant increase of bone resorption accompanied by osteoarticular symptoms. In addition, othe

Primary hyperparathyroidism and monoclonal gammopathy.

Coexistent primary hyperparathyroidism and monoclonal gammopathy, although rare, has been reported previously by a number of investigators. We report four patients with such an occurrence who were seen between 1976 and 1988. Another patient with primary hyperparathyroidism also had multiple myeloma and was in remission for 12 years. These patients represent approximately 1% of the 386 patients with primary hyperparathyroidism seen during the same 12-year period. Although several mechanisms have been proposed to explain this concurrence, we believe it is the result of a chance occurrence. A rev

[Primary hyperparathyroidism: the recent findings].

Primary hyperparathyroidism is a quite frequent disease, the incidence of which has been found to increase over the past two decades. At the same time there has been a remarkable change in its clinical presentation; therefore this endocrine disorder earlier considered rare but almost always associated with complications, now appears a quite common and uncomplicated disease. In our series of 150 patients diagnosed between 1966 and 1989 the female/male ratio was found to be 1.77. The ratio has significantly increased (2.09), beginning from 1983; this was mainly due to the availability of a bone

Animal model of primary hyperparathyroidism.

An experimental model of hyperparathyroidism was developed in the rat to simulate primary hyperparathyroidism in humans. In this model thyroparathyroidectomized (TPTX) or parathyroidectomized (PTX) animals were infused for 6 days with an amount of bovine synthetic parathyroid hormone (PTH)-(1-34) fragment to restore plasma calcium levels to normal (0.7 U X h-1) or with PTH at twofold (1.4 U X h-1) or threefold (2.1 U X h-1) this basal level. Animals infused with 2.1 U X h-1 of bovine PTH-(1-34) exhibited hypercalcemia, hypophosphatemia, a reduction in theoretical renal threshold for phosphate

Neuromuscular involvement in primary hyperparathyroidism.

Sixteen consecutive patients with primary hyperparathyroidism (HPT) were examined by a combination of electrophysiological, morphological and biochemical measurements. Six patients had, preoperatively, subjective impairment of neuromuscular function. Three of them were improved by the normalization of parathyroid function after surgery. In the other patients, who either had vague symptoms or appeared to be asymptomatic, the operation did not cause subjective changes in their neuromuscular function during a 3-month follow-up period. Altogether seven patients, three of whom had symptoms, had imp

[Endocrinological characteristic of primary hyperparathyroidism].

Parathyroid hormone is mainly regulated by the serum calcium concentration and not by another hormone which is usually the case for other hormones. We examined whether the parathyroid hormone could also be regulated by a hormone such as adrenocorticotropic hormone (ACTH). Experiment I: A two-hour urine sample was collected from 6 AM to 8 AM. At 8 AM one mg of synthetic ACTH was injected intramuscularly. Blood and urine was collected two hours after the injection for determination of the concentration of serum calcium, phosphate, parathyroid hormone and cortisol. Experiment II: Adenoma tissue w

Surgical aspects of primary hyperparathyroidism.

A series of 217 operations performed for primary hyperparathyroidism between 1965 and 1978, was studied to evaluate the peroperative findings and to assess the results of surgical treatment. There was a greater incidence in women than in men, a ratio of 2.5:1. Although occurring at any age, the incidence was higher for women over the age of 45. During all 217 neck explorations it was attempted to identify all parathyroid glands. In 89% of cases three or more glands were found. Of the 746 glands, 74% were situated normally and 12% were found inside the thymus. Histological examination confirmed

Primary hyperparathyroidism in paraneoplastic hypercalcaemia.

Hypercalcaemia is often associated with malignant disease. Causes of elevated serum-calcium levels in the absence of bony metastases include parathyroid-hormone production by the tumour, osteolytic factors made by the tumour, and coexistent primary hyperparathyroidism. By measuring nephrogenous cyclic-A.M.P. excretion to assess parathyroid-hormone function, we have determined the mechanism of such hypercalcaemia in 15 patients. Nephrogenous cyclic A.M.P. ranges from 0.05 to 2.40 mumol/g of creatinine in normal subjects, from 2.27 to 8.45 mumol/g in patients with primary hyperparathyroidism, an

Primary hyperparathyroidism exacerbated by pregnancy.

Primary hyperparathyroidism may remain subclinical as long as there is adequate calcium in the diet. If the demand for calcium increases, as in pregnancy, the symptoms become evident. Any giant cell growths should alert the oral surgeon to the possibility of hyperparathyroidism. Untreated, hyperparathyroidism ultimately leads to fractures, irreversible kidney damage, and death by renal failure. Tetany must be guarded against after parathyroid surgery. Finally, the brown tumors of hyperparathyroidism need not be removed surgically since bone will regenerate if the disease is correctly treated b

Masked primary (or tertiary) hyperparathyroidism.

Two patients are described in whom the preliminary clinical and laboratory investigations suggested a diagnosis of osteomalacia, from gluten-sensitive enteropathy in one and from anticonvulsant therapy in the other. However, when the primary disease was corrected by diet and extra vitamin D, respectively, both patients developed hypercalcaemia. A standard hydrocortisone test in the second patient failed to reduce the hypercalcaemia. In both patients parathyroid tumours were found at operation. It is suggested that both patients had tertiary hyperparathyroidism in which the normally tell-tale h

A meta-analysis comparing the biochemistry of primary hyperparathyroidism in youths to the biochemistry of primary hyperparathyroidism in adults.

CONTEXT: The distinctive presentation of primary hyperparathyroidism (PHPT) in adults and youths suggest that PHPT is a fundamentally different disease in these two groups. OBJECTIVE: To understand the difference in PHPT between adults and youths we compared the biochemistry of PHPT in these two groups. DESIGN: This study is a systematic review and meta-analysis of retrospective studies published 1966-2014 on PHPT. DATA SOURCES: All studies were obtained through Medline (1966-2014). STUDY SELECTION AND DATA EXTRACTION: Only studies that included post-surgical subjects and that explicitly d

Cardiovascular Involvement in Primary Hyperparathyroidism.

Increased serum calcium with elevated or nonsuppressed PTH levels is the biochemical hallmark of primary hyperparathyroidism (PHPT). A large body of evidence linked both hypercalcemia and high serum levels of PTH with cardiovascular diseases, due to their actions on cardiomyocytes, endothelial, pancreatic β, and fat cells. However, controversial epidemiological data exist regarding the higher prevalence of hypertension, arrhythmias, dyslipidemia, echocardiographic, and vascular parameter impairments in PHPT patients compared to controls. Different cardiovascular risk factors related to sex, ag

Individualized management of primary hyperparathyroidism.

Primary hyperparathyroidism (PHPT) is a common endocrine disease that typically presents in peri- and postmenopausal women. PHPT is due to a single parathyroid adenoma in most cases, which leads to stable hypercalcemia over years. Nowadays, the diagnosis is often coincidental, reached by chance in subjects without specific symptoms-asymptomatic PHPT. However, the patients might have subtle disease-related manifestations. Management standards have been reviewed through international workshops over more than three decades, leading to absolute and relative indications for parathyroidectomy (PTX),

Intentional Imaging for Primary Hyperparathyroidism.

IMPORTANCE: Preoperative parathyroid localization has been a great advance in the success of surgery for patients with primary hyperparathyroidism (pHPT) over the past 40 years. OBJECTIVE: To highlight the amount of radiation exposure from contemporary parathyroid imaging options and to suggest intentional imaging use in planning for parathyroidectomy. EVIDENCE REVIEW: Contemporary parathyroid imaging modalities and their respective radiation exposure were reviewed. The analysis focused on patients undergoing preoperative parathyroid imaging related to pHPT and persistent/recurrent pHPT. Exp
