An epidemic model with a multistage vaccine.

Many diseases, such as the seasonal flu, tetanus, and smallpox, can be vaccinated against with a single dose of a vaccine. However, some diseases require multiple doses of a vaccine for immunity. For example, Hepatitis B requires three doses of a vaccine, with the second occurring about 1 month after the first and the third occurring about 5 months after the second, for immunity. Diseases requiring a multistage vaccine such as Hepatitis B can have extra complications with its vaccination program, as some who start the doses may forget to complete the program or could become infected before com

History repeats itself in Japan: Failure to learn from rubella epidemic leads to failure to provide the HPV vaccine.

Eradication of vaccine-preventable diseases is one of the most important goals of public health interventions. Herd immunity can be established by national vaccination programs. However, once the national vaccination program stops for any reason, unprotected group will be generated depending on an unfortunate year of birth. Currently, there are 2 major concerns regarding vaccine cohort gaps in Japan: one is for the rubella vaccine, the other for the human papillomavirus (HPV) vaccine.

Serogroup A meningococcal conjugate vaccines in Africa.

Serogroup A meningococcal epidemics have been a recurrent public health problem, especially in resource-poor countries of Africa. Recently, the administration in mass vaccination campaigns of a single dose of the monovalent meningococcal conjugate vaccine, MenAfriVac, to the 1-29 year-old population of sub-Saharan Africa has prevented epidemics of meningitis caused by serogroup A Neisseria meningitidis. This strategy has also been shown to provide herd protection of the non-vaccinated population. Development of meningococcal conjugate vaccines covering other serogroups and enhanced use of the

The rise in pertussis cases urges replacement of chemically-inactivated with genetically-inactivated toxoid for DTP.

The number of pertussis cases reported to the CDC increased from 5158 in 1995 to 21,503 in 2005. Most of the increase was in individuals greater than 10 years of age. This increase occurred also in other developed nations despite high coverage of infants and young children with the acellular pertussis vaccine. In Goteborg Sweden, virtual elimination of pertussis occurred following immunization of 70% of the children less than 10 years old with monocomponent pertussis toxoid (PTx). Immunity following disease or vaccination with either the cellular or acellular pertussis vaccine wanes gradually

Association of past dengue fever epidemics with the risk of Zika microcephaly at the population level in Brazil.

Despite all the research done on the first Zika virus (ZIKV) epidemics, it was only after the Brazilian epidemic that the Congenital Zika Syndrome was described. This was made possible due to the large number of babies born with microcephaly in the Northeast region (NE) in a narrow time. We hypothesize that the fivefold difference in the rate of microcephalic neonates between the NE and other regions is partially an effect of the population prior immunity against Dengue viruses (DENV), that cross-react with ZIKV. In this ecological study, we analysed the interaction between dengue fever epidem

Impact of vaccination on meningococcal epidemiology.

Neisseria meningitidis may cause invasive disease (meningitis and sepsis), leading to considerable disease burden and mortality. However, effective vaccines are available against most pathogenic serogroups. Large-scale vaccination campaigns with the MCC vaccine conducted in UK and with MenAfriVac in the Sahel have clearly demonstrated the direct and indirect effect of immunization programmes on disease and carriage. Moreover, the introduction of novel subcapsular vaccines against serogroup B, which may cross-protect against other serogroups, is likely to have a further effect on trends. Accura

Characterizing Newcastle disease virus transmission dynamics and the impact of vaccination in village settings.

Newcastle disease virus (NDV) is a virulent poultry pathogen with global circulation. Several vaccines reduce individual disease burden, including the attenuated, thermotolerant, avirulent, live-virus I-2 vaccine, but estimates of the population-scale impact of such vaccines in natural transmission settings are still lacking. Here, we leverage a unique 8 year dataset of poultry mortality from village settings in Madagascar, spanning pre-vaccination and vaccination periods with the I-2 vaccine, and fit a compartmental transmission model to characterize transmission and impact of vaccination. Pr

Transmissible vaccines whose dissemination rates vary through time, with applications to wildlife.

Transmission is a potential property of live viral vaccines that remains largely unexploited but may lie within the realm of many engineering designs. While likely unacceptable for vaccines of humans, transmission may be highly desirable for vaccines of wildlife, both to protect natural populations and also to limit zoonotic transmissions into humans. Defying intuition, transmission alone does not guarantee that a vaccine will perform well: the benefit of transmission over no transmission depends on and increases with the basic reproductive number of the vaccine, R0. The R0 of an infectious ag

Optimal control of vaccination dynamics during an influenza epidemic.

For emerging diseases like pandemic influenza, several factors could impact the outcome of vaccination programs, including a delay in vaccine availability, imperfect vaccine-induced protection, and inadequate number of vaccines to sufficiently lower the susceptibility of the population by raising the level of herd immunity. We sought to investigate the effect of these factors in determining optimal vaccination strategies during an emerging influenza infection for which the population is entirely susceptible. We developed a population dynamical model of disease transmission and vaccination, and

Herd protection against influenza.

Mortality and hospitalization rates due to influenza have risen despite increasing vaccine coverage for the most vulnerable population; however, those most vulnerable to complications and death are the least likely to respond to the vaccine. New strategies for influenza control are needed and indirect effectiveness (herd protection) has been demonstrated for several currently used vaccines - rubella, H. influenzae type b, pneumococcus varicella and hepatitis A. The Japanese schoolchildren program provided proof of concept of indirect effectiveness of influenza vaccine. The Central Texas field

Profiling natural antibodies to Mycoplasma hyosynoviae in commercial pigs.

Mycoplasma hyosynoviae (M. hyosynoviae) is a commensal bacterium that can induce lameness in growing and finishing pigs, contributing to welfare concerns in swine production. The objective of this study was to characterize the natural humoral immune response to M. hyosynoviae in various ages of pigs commercially raised in the United States. Three > 2500 sow farms and their downstream sites were enrolled in the study. One herd presented a history of M. hyosynoviae-associated lameness in growing/finishing age pigs (affected), while two farms and their downstream flows were apparently healthy (no

A critical appraisal of the recommendations for the use of meningococcal conjugate vaccines.

OBJECTIVES: To assess the epidemiology of meningococcal disease (MD) in Brazil and the impact that recent evidence and lessons learned from the introduction of meningococcal C conjugate (MCC) vaccines into immunization programs may have on different strategies of vaccine use. SOURCES: Non-systematic review of the MEDLINE, SciELO and LILACS databases covering the period from 2000 to 2011. SUMMARY OF THE FINDINGS: Meningococcal disease is endemic in Brazil, with periodic occurrence of outbreaks. Most cases are associated with serogroup C and the highest incidence rates are observed in infants,

Effects of boosting and waning in highly exposed populations on dengue epidemic dynamics.

Sequential infection with multiple dengue virus (DENV) serotypes is thought to induce enduring protection against dengue disease. However, long-term antibody waning has been observed after repeated DENV infection. Here, we provide evidence that highly immune Nicaraguan children and adults (n = 4478) experience boosting and waning of antibodies during and after major Zika and dengue epidemics. We develop a susceptible-infected-recovered-susceptible (SIRS-type) model that tracks immunity by titer rather than number of infections to show that boosts in highly immune individuals can contribute to

Age-related changes in the rate of disease transmission: implications for the design of vaccination programmes.

Mathematical models are developed to aid in the investigation of the implications of heterogeneity in contact with infection within a community, on the design of mass vaccination programmes for the control of childhood viral and bacterial infections in developed countries. Analyses are focused on age-dependency in the rate at which individuals acquire infection, the question of 'who acquires infection from whom', and the implications of genetic variability in susceptibility to infection. Throughout, theoretical predictions are based on parameter estimates obtained from epidemiological studies

Efficacy and safety of oral poliovirus vaccine and inactivated poliovirus vaccine.

Inactivated poliovirus vaccine (IPV) is the vaccine of choice for protection against paralytic poliomyelitis provided that it is used within the context of a program to increase and sustain the level of uptake to as close to 100% as possible. This means targeting the disadvantaged in society as well as those who have their own pediatrician. The reasons are that enhanced-potency killed polio vaccine is safe, whereas oral poliovirus vaccine (OPV) is associated with a low, but definite, risk of paralysis, especially after the first dose. The immunity, as measured by antibody concentrations, is at

The epidemiology of measles in England and Wales since the 1994 vaccination campaign.

The incidence of measles in England and Wales has fallen since the national vaccination campaign in November 1994, in which 92% of children aged 5 to 16 years were vaccinated. A total of 148 confirmed cases with onsets in the 18 months from January 1995 to June 1996 have been ascertained. Notified cases did not provide a reliable measure of incidence: 11,343 suspected cases were notified in the same period, 6426 (57%) of whom were tested for salivary antibody. Only 90 (1.4%) of cases tested were confirmed. Many confirmed cases occurred in small clusters; 12 imported cases were identified. The

A Prevalent Focused Human Antibody Response to the Influenza Virus Hemagglutinin Head Interface.

Novel animal influenza viruses emerge, initiate pandemics, and become endemic seasonal variants that have evolved to escape from prevalent herd immunity. These processes often outpace vaccine-elicited protection. Focusing immune responses on conserved epitopes may impart durable immunity. We describe a focused, protective antibody response, abundant in memory and serum repertoires, to a conserved region at the influenza virus hemagglutinin (HA) head interface. Structures of 11 examples, 8 reported here, from seven human donors demonstrate the convergence of responses on a single epitope. The 1

Evaluating the population measles susceptibility in Tianjin, China.

BACKGROUND: Measles is a highly infectious illness requiring herd immunity of 95% to interrupt transmission. China has not reached elimination goals despite high vaccination coverage. We estimated the population susceptibility against measles in Tianjin, China and to tailor awareness raising activities in the measles elimination plan. METHODS: Age-specific measles seroprevalence was evaluated by Enzyme-Linked Immunosorbent Assay (ELISA) on 12,164 individual aged 0-44 years in 2009-2018. Measles IgG avidity testing was performed to confirm the relationship of the waning immunity after vaccinat

Mathematical modelling of Echinococcus multilocularis abundance in foxes in Zurich, Switzerland.

BACKGROUND: In Europe, the red fox (Vulpes vulpes) is the main definitive host of Echinococcus multilocularis, the aetiological agent of a severe disease in humans called alveolar echinococcosis. The distribution of this zoonotic parasite among the fox population is remarkably aggregated with few heavily infected animals harbouring much of the parasite burdens and being responsible for most of the environmental parasitic egg contamination. Important research questions explored were: (i) spatial differences in parasite infection pressure related to the level of urbanization; (ii) temporal diffe

Is chickenpox so bad, what do we know about immunity to varicella zoster virus, and what does it tell us about the future?

Varicella and zoster continue to cause significant morbidity and even mortality in children and adults. Complications include bacterial superinfection, central nervous system manifestations such as meningitis, encephalitis, and cerebellar ataxia, and pain syndromes especially post herpetic neuralgia. Many developed countries but not all, are now administering live attenuated varicella vaccine routinely, with a decrease in the incidence of disease, providing personal and herd immunity. There is some controversy, however, in some countries concerning whether a decrease in the circulation of wild
