First case report of Nocardia veterana causing nodular lymphangitis in an immunocompromised host.

We report a unique case of ascending cutaneous lymphangitis in a 72-year-old immunocompromised man from which a newly described Nocardia species was isolated by 16S ribosomal gene sequencing. Treatment with trimethoprim-sulfamethoxazole resulted in successful resolution of symptoms. To the best of our knowledge, this is the first case report of N. veterana implicated in causing ascending cutaneous lymphangitis.

Fatal cysticercosis by Taenia crassiceps (Cyclophyllidea: Taeniidae) in a presumed immunocompromised canine host.

Cysticercosis in a canine host (Canis familiaris) attributable to the taeniid cestode Taenia crassiceps is reported for the first time in North America. Numerous parent and daughter cysticerci occurred in a massive intrapleural and intraperitoneal infection in an apparently immunocompromised host. The largest cysticerci were ovoid to elongate, 5-9 mm in maximum length, and armed with 32-34 rostellar hooks in 2 rows; small hooks measured 114-143 microm long (x = 124+/-8.2 microm), and large hooks were 156-180 microm (x = 163+/-7.4 microm). Taenia crassiceps is widespread in boreal North America…

Acute gastric mucosal lesions associated with cytomegalovirus infection in a non-immunocompromised host.

A 75-year-old woman with epigastric pain and tarry stool was admitted to our hospital, where upper gastrointestinal endoscopic study revealed multiple gastric ulcers. The endoscopic biopsy specimens obtained on the seventh hospital day disclosed a few typical intranuclear cytomegalovirus inclusions. Cytomegalovirus-DNA was detected using polymerase chain reaction in a biopsy specimen. No immunologic abnormalities were demonstrated by any laboratory tests. While only a few cases of cytomegalovirus-associated gastric ulcer in non-immunocompromised hosts have been reported, this entity may be mor…

Rapidly progressive necrotizing cellulitis secondary to Candida tropicalis infection in an immunocompromised host.

The incidence of invasive fungal infection is increasing as the population of immunosuppressed patients grows. Many species that were previously thought to be benign are now known pathogens. The most commonly isolated organisms (>80 %) include Candida albicans, Cryptococcus neoformans and Aspergillus fumigatus. However, there are several new and emerging organisms. The Non-albicans species of Candida are one of the emerging invasive fungal organisms that are beginning to affect high-risk patients such as those with bone marrow transplant, neutropenia, HIV/AIDS or on immunosuppressive therapy.

Immunocompromised host: from the early events until the impact of acquired immunodeficiency syndrome.

The concept that microorganisms can modulate the host resistance was historically reviewed in the present article. The importance of African trypanosomiasis in the development of the research on immunosuppression as well as the impact of human immunodeficiency virus infection are discussed. Each day new opportunistic organisms establish a constant challenge for the correct diagnosis of concomitant infections in acquired immunodeficiency syndrome. The importance of parasite infection in the balance of host resistance in the third world was emphasized. Finally, some aspects of Leishmania as oppo

Immune evasion, dysregulation, and emerging immunotherapies for invasive fungal infections in the immunocompromised host.

Invasive fungal infections (IFIs) represent a persistent challenge to global health, particularly in immunocompromised individuals, among wh … unomodulatory effectors, which collectively facilitate fungal persistence and dissemination. In the immunocompromised host, these evasion tactics are compounded by profound immune dysregulation, characterized by aberrant activation of the NOD-, LRR-, and pyrin domain-containing protein 3 (NLRP3) inflammasome, deficiencies in T helper type 1 (Th1) and type 17 (Th17) resp …

Pulmonary mucormycosis and Achromobacter xylosoxidans co-infection in an immunocompromised host: a case report.

INTRODUCTION AND IMPORTANCE: Pulmonary mucormycosis (PM) is a severe fungal infection threatening immunocompromised individuals, especially with uncontrolled diabetes. Co-infection with bacteria like Achromobacter xylosoxidans complicates management. We report a rare and fatal case of PM co-infected with A. xylosoxidans, which was further complicated by a subsequent SARS-CoV-2 superinfection. CASE PRESENTATION: A 55-year-old female with poorly controlled type 2 diabetes mellitus and multiple comorbidities presented with worsening dyspnea. Investigations revealed severe hyperglycemia (927 mg/d

Antiviral combination regimens as rescue therapy in immunocompromised hosts with persistent COVID-19.

The management of severe/prolonged SARS-CoV-2 infections in immunocompromised hosts is still challenging. We describe nine patients with hematologic malignancies with a history of unsuccessful SARS-CoV-2 treatment receiving antiviral combination treatment for persistent infection at a tertiary hospital in central Italy (University Hospital of Careggi, Florence). Combination treatments consisted of nirmatrelvir/ritonavir plus molnupiravir (n = 4), nirmatrelvir/ritonavir plus remdesivir (n = 4) or remdesivir plus molnupiravir (n = 1) for 10 days, in some cases associated with sotrovimab. Combina…

Combination Antifungal Therapy for Invasive Mucormycosis in Immunocompromised Hosts: A Single-Center Experience.

Combination antifungal therapy for invasive mucormycosis remains controversial and is inconsistently defined in prior studies. In a cohort of patients with immunocompromised status and invasive mucormycosis, we found no difference in 6-week mortality with up-front or salvage combination therapy as compared with monotherapy.

Overlapping Infection by Strongyloides spp. and Cytomegalovirus in the Immunocompromised Host: A Comprehensive Review of the Literature.

Strongyloides and cytomegalovirus co-infections are rarely reported, even though they are distinguished by high morbidity and mortality, especially in immunocompromised hosts. We narratively reviewed the literature on reported cases of Strongyloides and CMV co-infections in immunosuppressed patients. Most cases occurred in males with a median age of 47 (IQR, 37-59). Strongyloides/CMV co-infections occurred among immunocompromised hosts, especially in solid organ transplants and hematological or rheumatological diseases. Most of the patients underwent a course of steroid treatment before the di…

The master of masquerade: A case series of unusual presentations of cutaneous tuberculosis in the immunocompromised host.

Cutaneous tuberculosis is known for its varied presentations, especially in the setting of immunosuppression. Clinical manifestations can be modified by the site of involvement as well as the type of cutaneous tuberculosis in a particular patient. Although cutaneous tuberculosis only accounts for a small percentage (1-2%) of extrapulmonary cases, it is encountered fairly frequently in dermatology practice in an endemic country like India. These cases are being presented to highlight unusual presentations of well-known forms of cutaneous TB (TB gumma and erythema induratum of Bazin) and rare mo

Skin and Soft Tissue Infections in Non-Human Immunodeficiency Virus Immunocompromised Hosts.

Skin and soft tissue infections among the non-human immunodeficiency virus infected immunosuppressed population are a serious and growing concern. Many pathogens can cause cutaneous infections in these patients owing to the highly varied and profound immune deficits. Although patients can be infected by typical organisms, the diversity and antimicrobial-resistant nature of the organisms causing these infections result in significant morbidity and mortality. The diagnostic approach to these infections in immunocompromised hosts can differ dramatically depending on the potential causative organi…
