Mechanisms of change during exposure therapy for persistent post-concussion symptoms: insights from session-level process data.

BACKGROUND: Persistent post-concussion symptoms affect a substantial subgroup of individuals following mild traumatic brain injury. Intensive exposure therapy may be an effective intervention by directly targeting psychological mechanisms underlying symptom maintenance. This study provides insight into the clinical processes of this therapy. MATERIALS AND METHODS: Therapy session-level data from two single-case experimental design studies with a total of eight participants was analyzed using mixed-methods. RESULTS: The most frequently targeted feared expectations included: cognitive failure,

Insomnia as a predictor of treatment outcomes in adolescents receiving concentrated exposure treatment for OCD.

BACKGROUND: Research suggests that individuals with obsessive-compulsive disorder (OCD) frequently experience insomnia. Some previous studies have suggested that insomnia may predict treatment outcomes, but the evidence is limited, especially for adolescents. This study examined the prevalence of insomnia in an adolescent OCD patient sample, explored the correlation between OCD and insomnia, and tested whether levels of insomnia at baseline predict outcomes for adolescent patients receiving the Bergen 4-Day Treatment (B4DT) for OCD. METHODS: Forty-three adolescent OCD patients who received B4

Modified Prolonged Exposure Therapy for Posttraumatic Stress Disorder with an 85-Year-Old Native American Cajun Man With Late-Onset Deafness: A Case Report.

OBJECTIVES: Few studies of exposure therapy with adults 85 years and older exist. This case report presents results of prolonged exposure (PE) for posttraumatic stress disorder (PTSD) modified for an 85-year-old Native American Cajun man with late-onset deafness, who used a cochlear implant, and reported high Western/non-Native American acculturation. The following modifications were made primarily in response to the client's individual, disability-related barriers to completing the standard PE protocol: (a) inclusion of the client's spouse in aspects of treatment planning and homework assignm

Effectiveness of 'Tackle Your Tics', a brief, intensive group-based exposure therapy programme for children with tic disorders: study protocol of a randomised controlled trial.

INTRODUCTION: This paper outlines the study protocol for the Dutch Tackle Your Tics study in youth with tic disorders. Tourette syndrome and chronic tic disorders are prevalent neurodevelopmental disorders, placing considerable burden on youth and their families. Behavioural treatment is the first-line, evidence-based intervention for tic disorders, but tic reduction and availability remain relatively low. Patient associations stress the need for more accessible high-quality treatments, also focusing on improving quality of life. Therefore, the brief, intensive group-based treatment Tackle You

Positive valence reduces susceptibility to return of fear and enhances approach behavior.

BACKGROUND AND OBJECTIVES: Although exposure therapy is highly efficacious for anxiety disorders, many individuals do not respond. Drawing from the science of fear extinction and reinstatement, the current study evaluated whether a training designed to increase valence of the feared stimulus improved the longevity of treatment outcomes. METHODS: Participants were 61 undergraduate students with fear of spiders (>10 on Spider Phobia Questionnaire, M = 20.45, SD = 3.98) who were randomized to receive positive valence training or control training. Participants completed exposure over two days, wi

Enhancing Therapist Training in the Delivery of Exposure Therapy for Individuals with Anxiety Disorders Using Virtual Reality Simulation: Randomized Feasibility Trial.

BACKGROUND: Exposure-based cognitive behavioral therapy is among the least used evidence-based practices for anxiety disorders in routine care. Providers' negative beliefs about exposure (eg, fears of harm or intolerability) are a major barrier. Experiential methods can reduce these beliefs but are limited by accessibility, standardization, and fidelity. Virtual reality (VR) offers a scalable way to deliver standardized experiential practice. Guided by an "exposure to exposure" (E2E) framework, we conceptualized VR training as an exposure intervention targeting therapists' own anxious beliefs

Virtual Reality Body Exposure and Attentional Bias Modification in the Treatment of Adolescents With Anorexia Nervosa.

Anorexia nervosa (AN) is a severe psychiatric disorder characterized by intense fear of gaining weight and persistent body image disturbance. Virtual reality-based mirror exposure therapy (VR-MET) may reduce body-related fear through embodied exposure, while attentional bias modification training (ABMT) may enhance exposure learning by promoting balanced attentional allocation. This controlled clinical study tested the hypothesis that adjunctive VR-MET would improve clinical outcomes beyond treatment as usual (TAU) alone, and that preceding VR-MET with ABMT would produce additional benefits in

Patient Adherence as a Mediator of Cognitive Behavioral Therapy Outcome in Adults With Obsessive-Compulsive Disorder.

Exposure and response prevention (EX/RP) is a first-line treatment for obsessive-compulsive disorder (OCD), yet many patients fail to achieve full remission. Among the strongest predictors of treatment outcome is patient adherence-particularly to between-session exposure exercises and ritual prevention. However, limited research has examined factors that influence adherence. This study aimed to replicate and extend prior findings by identifying predictors of both treatment outcome and adherence among medicated adults with OCD undergoing EX/RP, and by evaluating adherence as a potential mediato

Maximising the efficacy of in vivo exposure therapy using smartphone-assisted just-in-time adaptive intervention for anxiety disorders: study protocol for a randomised controlled trial.

INTRODUCTION: For anxiety disorders (ADs), the preferred treatment is cognitive-behavioural therapy (CBT), with in vivo exposure (IVE) as its main component. IVE aims to gradually expose the patient to feared stimuli and situations to encourage habituation. Despite its effectiveness, it is a hard component for patients, especially when applied on their own. Therefore, a (co-)therapist usually supports the exposure requiring considerable time and resources. Meanwhile, smartphones have become a promising tool to complement and deliver psychological treatments. METHODS AND ANALYSIS: The study pr

Cognitive rehabilitation and exposure/sorting therapy for hoarding disorder: A randomized controlled trial.

OBJECTIVE: Hoarding disorder (HD) is one of the most common and impairing neuropsychiatric conditions in late life. Cognitive Rehabilitation and Exposure/Sorting Therapy (CREST) for HD is effective in reducing HD symptoms and improving functioning when compared with case management. In the current trial (NCT02402647), we compared CREST to a robust control condition, exposure therapy (ET), to determine if CREST provides additional benefit to HD symptom reduction, functioning, and quality of life over ET. METHOD: One hundred fifteen Veterans with HD were randomized to receive 26 once weekly, 1-

Within-patient association between emotion regulation and outcome in prolonged exposure for posttraumatic stress disorder.

OBJECTIVE: Difficulties with emotion regulation (ER) are a risk factor for the development and maintenance of posttraumatic stress disorder (PTSD). Less is known about temporal relations between ER and PTSD symptom change during treatment, including whether ER may represent a more potent change ingredient for some patients relative to others. This study examined the association between within-patient changes in ER and next-session PTSD symptom change and whether this association was more pronounced for patients with poorer baseline ER, more severe depression, or higher borderline personality d

Effect of exposure-based vs traditional cognitive behavior therapy for fibromyalgia: a two-site single-blind randomized controlled trial.

Fibromyalgia is a debilitating pain condition for which treatment effects are typically modest. The most evaluated psychological treatment is traditional cognitive behavior therapy (T-CBT), but promising effects have recently been seen in exposure-based cognitive behavior therapy (Exp-CBT). We investigated whether Exp-CBT was superior to T-CBT in a randomized controlled trial. Self-referred participants with fibromyalgia (N = 274) were randomized (1:1) to 10 weeks of Exp-CBT or T-CBT. Treatments were delivered online and presented as "CBT for fibromyalgia." Participants were assessed at baseli

Efficacy of an Internet-based intervention with self-applied exposure therapy in virtual reality for people with panic disorder: study protocol for a randomized controlled trial.

BACKGROUND: Due to several treatment barriers, many individuals with panic disorder do not receive evidence-based treatment. One promising option to narrow this treatment gap is Internet-based psychotherapy, which has been shown particularly effective in guided formats. Still, there remains room for improvement to make these digital therapies more accessible, cost-efficient, and aligned with best practices for in-person interventions (e.g., exposure). The smartphone app "Invirto - Treatment for Anxiety" offers digitally guided, evidence-based treatment of panic disorders including virtual real

The CHALLENGE trial: the effects of a virtual reality-assisted exposure therapy for persistent auditory hallucinations versus supportive counselling in people with psychosis: study protocol for a randomised clinical trial.

BACKGROUND: Many patients suffering from schizophrenia spectrum disorders continue having distressing auditory hallucinations in spite of treatment with antipsychotic medication. The aim of this trial is to examine the effect of a targeted virtual reality therapy for persistent auditory hallucinations in individuals with psychosis. The trial explores whether this type of therapy can decrease the severity, frequency and distress of auditory hallucinations and, additionally, whether it can reduce clinical symptoms and enhance daily functioning in individuals with psychosis. METHODS: The study i

No Effects of D-Cycloserine Enhancement in Exposure With Response Prevention Therapy in Panic Disorder With Agoraphobia: A Double-Blind, Randomized Controlled Trial.

PURPOSE/BACKGROUND: D-cycloserine (DCS) is a partial N-methyl-D-aspartate receptor agonist that potentially augments response to exposure therapy in anxiety disorders by enhancing extinction learning. This randomized, double-blinded, placebo-controlled augmentation trial examined (1) the effectiveness of adding 125 mg of DCS to exposure therapy (before or directly after the first 6 treatment sessions) in patients with panic disorder with agoraphobia and (2) the effectiveness of DCS augmentation preceding exposure relative to DCS augmentation directly postexposure. METHODS/PROCEDURES: Fifty-se

Virtual reality compared with in vivo exposure in the treatment of social anxiety disorder: a three-arm randomised controlled trial.

BackgroundPeople with social anxiety disorder (SAD) fear social interactions and may be reluctant to seek treatments involving exposure to social situations. Social exposure conducted in virtual reality (VR), embedded in individual cognitive-behavioural therapy (CBT), could be an answer.AimsTo show that conducting VR exposure in CBT for SAD is effective and is more practical for therapists than conducting exposure in vivoMethodParticipants were randomly assigned to either VR exposure (n = 17), in vivo exposure (n = 22) or waiting list (n = 20). Participants in the active arms received individu

Reaching new heights: comparing interpretation bias modification to exposure therapy for extreme height fear.

OBJECTIVE: Cognitive models of anxiety disorders posit that biases in interpretation maintain, and potentially cause, anxiety. This study tested whether it is possible to decrease height fear symptoms through cognitive bias modification for interpretations (CBM-I). Additionally, the clinical utility of CBM-I was tested by comparing it to an already established treatment: exposure therapy. METHOD: Extremely height fearful individuals (N = 110) participated in the study. Acrophobic symptoms were measured before and after 2 sessions of CBM-I and were compared to the standard treatment for acroph

Reducing contamination by exposure plus safety behaviour.

BACKGROUND AND OBJECTIVES: It has been proposed that the judicious use of safety behaviour can facilitate improvements in the acceptability of cognitive behaviour therapy (CBT). It was decided to explore the possibility of facilitating CBT by introducing a form of safety behaviour. We sought to assess the degree to which Exposure plus Safety Behaviour (E + SB) is an effective intervention for contamination fears. METHODS: A comparison was made between the effects of a control condition (Exposure and Response Prevention; ERP) and an experimental condition (Exposure plus Safety Behaviour; E + S

One-session computer-based exposure treatment for spider-fearful individuals--efficacy of a minimal self-help intervention in a randomised controlled trial.

Computer-based self-help treatments have been proposed to provide greater access to treatment while requiring minimum input from a therapist. The authors employed a randomised controlled trial to investigate the efficacy of one-session computer-based exposure (CBE) as a self-help treatment for spider-fearful individuals. Spider-fearful participants in a CBE group underwent one 27-min session of standardised exposure to nine fear-eliciting spider pictures. Treatment outcome was compared to spider-fearful control participants exposed to nine neutral pictures. Fear reduction was quantified on a s
