Hallux valgus deformity treated with the extensor hallucis longus tendon transfer by dynamic correction.

BACKGROUND: Recurrence of hallux valgus is considered to be the most common problem experienced postoperatively. We designed and carried out operations to correct hallux valgus by transferring the extensor hallucis longus (EHL) tendon to reduce the likelihood of recurrence. METHODS: Twenty-five patients (38 feet) with the average age of (46.3 ± 12.3) (range, 22 to 60) years underwent the operation. The American Orthopaedic Foot and Ankle Society score and weight-bearing radiographs of the foot were applied to assess the feet pre- and postoperatively with a mean duration of follow-up of (38.2

[The heat-reinforcing needling for Kashin-Beck disease with cold-dampness blocking collaterals syndrome].

OBJECTIVE: To compare the efficacy differences between heat-reinforcing needling and conventional treatment of western medicine on Kashin-Beck disease (KBD) with cold-dampness blocking collaterals syndrome. METHODS: Sixty KBD patients of cold-dampness blocking collaterals syndrome were randomly assigned into a heat-reinforcing needling group and a western medication group, 30 cases in each one. In the heat-reinforcing needling group, the heat-reinforcing needling was applied at local painful sites, combined with the acupoints based on the syndrome differentiation and the distal acupoints on t

Compartment Syndrome Following Arthroscopic Removal of a Bullet in the Knee Joint after a Low-Velocity Gunshot Injury.

Gunshot injuries are getting more frequently reported while the civilian (nongovernmental) armament increases in the world. A 42-year-old male patient presented to emergency room of Istanbul Medipol University Hospital due to a low-velocity gunshot injury. We detected one entry point on the posterior aspect of the thigh, just superior to the popliteal groove. No exit wound was detected on his physical examination. There was swelling around the knee and range of motion was limited due to pain and swelling. Neurological and vascular examinations were intact. Following the initial assessment, the

The effect of chevron osteotomy with lateral capsular release on the blood supply to the first metatarsal head.

The chevron osteotomy is one of the most widely used distal metatarsal osteotomies for the treatment of hallux valgus in adults. Because the osteotomy interrupts the intraosseous blood supply to the metatarsal head, there has always been a concern that the operation could produce osteonecrosis of the metatarsal head, particularly if the important extraosseous blood supply was also damaged. We used latex injection and a modified Spalteholz technique in cadaveric specimens to demonstrate the effect of the chevron osteotomy, with and without lateral capsular release, on the vascular supply to the

Modified volar approach for proximal row carpectomy.

OBJECTIVES: Proximal row carpectomy is a well-accepted surgical procedure for the management of traumatic and degenerative wrist pathologies. It is routinely performed through a dorsal approach; a volar surgical access was presented in order to enable concomitant carpal tunnel release and avoid flexion limitation or disabilities caused by adhesions of the dorsal capsule and extensor tendons. We propose a modification to the volar approach, with detailed description of skin incision (reproducing the standard palmar access to the scaphoid), capsular section (beginning with a longitudinal cut rad

Interface tissue fibroblasts from loose total hip replacement prosthesis produce receptor activator of nuclear factor-kappaB ligand, osteoprotegerin, and cathepsin K.

OBJECTIVE: The highly osteolytic interface tissue between the bone and loosening total hip prosthesis is characterized by low pH, formation of foreign body giant cells, osteoclasts, and production of receptor activator of nuclear factor-kappaB (RANKL) and cathepsin K. We hypothesized that fibroblasts in the interface tissue may form a source for RANKL production. METHODS: Primary interface tissue fibroblasts, fibrous joint capsule fibroblasts, and trabecular bone osteoblasts were stimulated with tumor necrosis factor-alpha (TNF-alpha), interleukin 1beta (IL-1beta), IL-6, IL-11, or 1alpha,25-(

Postoperative oxycodone toxicity in a patient with chronic pain and end-stage renal disease.

We present this case to review the metabolism of oxycodone and the effects of end-stage renal disease on the elimination of oxycodone and its metabolites. A 42-year-old female with end-stage renal disease who was dependent on hemodialysis presented for left hamstring posterior capsule release. She had been receiving methadone for 2 years for chronic leg pain. On postoperative day 1, the patient's medication was changed from IV hydromorphone to oral oxycodone to treat breakthrough pain. By the next day, the patient was unarousable with notable respiratory depression. She did not fully recover a

Posterior Sternoclavicular Joint Reconstruction With Semitendinosus Allograft for Symptomatic Posterior Sternoclavicular Joint Dislocations.

BACKGROUND: Posterior sternoclavicular joint (SCJ) dislocations are uncommon because of the strength of the posterior joint capsule but pose a significant risk of neurovascular and mediastinal compression. Various surgical techniques have been described, with the sternal docking reconstruction using tendon allografts demonstrating improved biomechanical stability, elimination of anterior bulk, and ease of graft passage. This study outlines the intramedullary posterior SCJ reconstruction with a semitendinosus allograft for recurrent symptomatic posterior SCJ dislocations. INDICATIONS: Surgical

Multimedia article. The rotator interval: pathology and management.

The rotator interval describes the anatomic space bounded by the subscapularis, supraspinatus, and coracoid. This space contains the coracohumeral and superior glenohumeral ligament, the biceps tendon, and anterior joint capsule. Although a definitive role of the rotator interval structures has not been established, it is apparent that they contribute to shoulder dysfunction. Contracture or scarring of rotator interval structures can manifest as adhesive capsulitis. It is typically managed nonsurgically with local injections and gentle shoulder therapy. Recalcitrant cases have been successfull

Arthroscopic Management of Posterior Elbow Impingement.

BACKGROUND: Posterior elbow impingement is often seen in athletes who engage in overhead sports that involve repetitive elbow extension; for these patients with persistent symptoms, arthroscopic debridement is often recommended. While traditionally performed in the supine or prone position, arthroscopic elbow debridement performed in the lateral decubitus position has distinct advantages that make it our preferred method. INDICATIONS: Arthroscopic debridement in the context of posterior elbow impingement is utilized when conservative management with rest, physical therapy, and nonsteroidal an

Adhesive capsulitis: a review.

Adhesive capsulitis is a common, yet poorly understood, condition causing pain and loss of range of motion in the shoulder. It can occur in isolation or concomitantly with other shoulder conditions (e.g., rotator cuff tendinopathy, bursitis) or diabetes mellitus. It is often self-limited, but can persist for years and may never fully resolve. The diagnosis is usually clinical, although imaging can help rule out other conditions. The differential diagnosis includes acromioclavicular arthropathy, autoimmune disease (e.g., systemic lupus erythematosus, rheumatoid arthritis), biceps tendinopathy,

Activation of Bone Remodeling Compartments in BMP-2-Injected Knees Supports a Local Vascular Mechanism for Arthritis-Related Bone Changes.

BACKGROUND: Synovial membrane-derived factors are implicated in arthritis-related bone changes. The route that synovial factors use to access subchondral bone and the mechanisms responsible for these bone changes remain unclear. A safety study involving intra-articular injection of bone morphogenetic protein-2 (BMP-2)/calcium phosphate matrix (CPM) or CPM addresses these issues. METHODS: Knee joints in 21 monkeys were injected with CPM or 1.5 or 4.5 mg/mL BMP-2/CPM and were evaluated at 1 and 8 weeks. Contralateral joints were injected with saline solution. Knee joints in 4 animals each were

Shoulder stiffness and rotator cuff repair.

INTRODUCTION: Shoulder stiffness is a frequent complication of surgical repair of rotator cuff tears. Post-operative stiffness negatively affects surgical outcomes leading to a substantial comorbidity and to the failure of surgical treatment. Also, a stiff shoulder could commonly be concomitant with an rotator cuff tear (RCT). SOURCES OF DATA: We performed a comprehensive search of CINAHL, Embase, Medline and the Cochrane Central Registry of Controlled Trials, from inception of the database to 31 July 2011. Sixteen articles published in peer-reviewed journals were included in this comprehensi

Management of missed Monteggia fractures with ulnar osteotomy, open reduction, and dual-socket external fixation.

BACKGROUND: Open reduction of the radial head is usually necessary in the treatment of a missed Monteggia fracture. However, the best way to stabilize the reduction remains controversial. The purpose of this study is to present our experience using the dual-socket external fixator to stabilize the ulna osteotomy and capture the reduction of the radial head. METHODS: We reviewed 33 patients with missed Monteggia fracture who underwent open reduction, ulnar osteotomy, and dual-socket external fixation from 2004 to 2010. The average age of this group was 7 years and the average time to surgical

Morphological Changes in Femoral Trochlea After Extensor Realignment Surgery in Children With Congenital Patellar Dislocation.

BACKGROUND: Femoral trochlear dysplasia (FTD) is a major factor affecting the stability of the knee joint. Congenital patellar dislocation (CPD) is usually accompanied by distal FTD. Few studies have investigated trochlear development following surgery in skeletally immature patients. PURPOSE: To observe trochlear remodeling following surgical extensor realignment and stabilization in children with CPD and to analyze the factors associated with this remodeling. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: A retrospective analysis was conducted on surgical cases of CPD treated a

Xinfeng Capsule ameliorates RA joint injury by inhibiting cuproptosis via the METTL3/miR-221/222-3p/ATP7A Axis.

BACKGROUND: Xinfeng Capsule (XFC), a traditional Chinese herbal formulation, has long been used clinically for rheumatoid arthritis (RA); however, its underlying mechanism remains unclear. OBJECTIVE: This study explored whether XFC alleviates RA-induced joint damage by suppressing cuproptosis via modulation of the METTL3/miR-221/222-3p/ATP7A axis. METHODS: Potential therapeutic targets of XFC against RA were predicted using bioinformatic analyses. Adjuvant-induced arthritis (AA) rat models and in vitro chondrocyte injury models were established and allocated to different treatment groups. D

Treatment with posterior capsular release, botulinum toxin injection, hamstring tenotomy, and peroneal nerve decompression improves flexion contracture after total knee arthroplasty: minimum 2-year follow-up.

PURPOSE: No definite treatment option with reasonable outcome has been presented for old and refractory flexion contracture after total knee arthroplasty (TKA). We describe a surgical technique for 21 refractory cases of knee flexion contracture, including 12 patients with history of failed manipulation under anesthesia (MUA). METHODS: Retrospective review was conducted for procedures performed by a single surgeon between 2005 and 2016. Twenty-one knees (19 patients) with knee flexion contracture after primary TKA were treated with all the following procedures: posterior capsular release, ham
