Bacterial keratitis following phototherapeutic keratectomy.

BACKGROUND: Phototherapeutic keratectomy (PTK) is effective in the treatment of many superficial corneal disorders. The incidence of bacterial keratitis following PTK has not been assessed in a large, prospective clinical trial. METHODS: We report three cases of bacterial keratitis that occurred in a prospective study of 258 consecutive PTK procedures at King Khaled Eye Specialist Hospital. RESULTS: Three (1.2%) of 258 eyes developed bacterial keratitis during a period of follow up ranging from 1 to 24 months. All three cases were in 183 eyes (1.6%) with a diagnosis of climatic droplet kerat…

[Current trends in herpetic keratitis].

The virus simplex herpes (VSH) differs from other viruses in that it frequently affects the eyes and because of the gravity of lesions. It belongs to the herpes-virus family which also includes the varicella virus, the cytomegalovirus, zone and the Epstein-Barr virus. The main feature consists of the fact that after the first infection in latent form, the virus remains inside the body and may cause recidives. The type I determines oculofacial lesions. While the type II determines genital lesions. Seldom it may affects the eye in a severe form. In fact there is inside every type a great antigen

Peripheral ulcerative keratitis and malignancies.

Although systemic and local autoimmune diseases are the primary possibilities in the differential diagnosis of noninfectious peripheral ulcerative keratitis (PUK), other unusual etiologies such as systemic or local malignancies also must be considered. We report on two patients with PUK whose study led to the discovery and subsequent treatment of a chronic myelogenous leukemia and a sebaceous cell carcinoma of the eyelid, respectively. The results emphasize the need for meticulous diagnostic pursuit, including the possibility of malignancy in patients with peripheral ulcerative keratitis.

Contact lens-related infectious keratitis.

Infectious keratitis is the most serious complication of contact lens use. Virtually all contact lens wearers are at risk. Initial therapy consists of frequent broad-spectrum fortified antibiotic drops after appropriate laboratory workup. Pseudomonas and Acanthamoeba species are the most important causes of contact lens-associated ulcers. Acanthamoeba keratitis produces significant ocular morbidity, and treatment is not always effective. Recent studies have provided new insights regarding the incidence, risk factors, and pathogenesis of contact lens-related infectious keratitis. Extended-wear …

Delayed onset of varicella keratitis.

Although varicella is one of the most common infectious diseases in the United States, systemic and ocular complications are rare. We report a patient who developed disciform edema followed by microdendritic keratitis 1 and 2 months, respectively, after resolution of the acute phase of varicella. Cultures were negative, but serologic analysis found positive antibodies against varicella zoster virus and negative antibodies against herpes simplex virus. Based on this case and on a review of the literature, we believe that this delayed onset of keratitis represents a distinct category of varicell…

Filamentary keratitis--the clinical challenges.

Filamentary keratitis, while appearing to be a simple entity, can be a clinical challenge. Earlier investigators failed to understand the complexities of filament formation and its association with a variety of ocular and systemic conditions. Through trial and error a variety of treatment modalities were employed, ranging from the use of topical drops to the application of electrical current. In most cases filaments resolve with conventional therapy, but in other cases it can become chronic, eventually resulting in vascularization of the cornea.

Microbial keratitis complicating penetrating keratoplasty.

A retrospective review of 68 consecutive episodes of microbial keratitis complicating 66 penetrating keratoplasties (PKs) showed major risk associations: suture-related problems (50%), contact lens wear (26%), previous herpes simplex infection (15%), graft failure (15%), and persistent epithelial defects (15%). Topical steroid (85%) and antibiotic (59%) usage were common iatrogenic factors. Half the infections occurred more than 1 year after grafting. Bacterial infections involving gram-positive organisms (59%) predominated, except for patients with extended-wear hydrophilic contact lenses, wh…

Chronic alcoholism and microbial keratitis.

In a series of 227 consecutive, non-referred patients with microbial keratitis an analysis of the accumulated hospital records showed that one-third were associated with chronic alcoholism. The diagnosis of alcoholism was usually unsuspected on admission to hospital. The microbial pathogenesis in these patients was distinctive; coagulase-negative staphylococci, alpha- and beta-streptococci, moraxellae, enteric Gram-negative bacilli, and polymicrobial infections were unusually prominent. Pseudomonas aeruginosa was uncommon. Trauma, exposure, bullous keratopathy, other external ocular diseases, …

[KID syndrome (keratitis-ichthyosis-deafness)].

The KID syndrome is characterized by congenital ichthyosis, vascular keratitis and neurosensorial deafness. We report a 17 year old female patient, the first case of KID syndrome in Spanish literature. Red, dry, scaling skin was present at birth with sparse hair. At the age of six, malar erythema was prominent, with perioral ragades and onset of progressive neurosensory deafness. At the age of ten, vascularizing keratitis developed. At 12, treatment with etretinate failed to improve the ichthyosis. We review the clinical, pathological and analytical features of KID syndrome and discuss its rel…

[Bandage lenses in neuroparalytic keratitis].

The successful treatment of two cases of neuroparalytic keratitis with bandage lenses is described. The possible pathogenesis of epithelial damage and our experience with the problems of fitting soft contact lenses on anesthetic corneas are discussed.

Ichthyosiform dermatosis, keratitis, and deafness.

Two patients had an unusual ichthyosiform dermatosis, neurosensory deafness, and vascularizing keratitis. There was no family history of the disorder and no consanguinity. A squamous cell carcinoma of the tongue developed in one child. The association of such an eruption and deafness in previously reported cases is reviewed.
