Upper pole calyx fornix subapical puncture in percutaneous nephrolithotomy.

BACKGROUND: This study aims to evaluate the effectiveness of upper pole calyx fornix subapical puncture in percutaneous nephrolithotomy (PCNL) compared to traditional middle calyx puncture. The subapical puncture technique offers the advantages of upper pole access while minimizing the risk of pleural injury. METHODS: We conducted a retrospective analysis of 194 patients who underwent PCNL at our hospital from May 2022 to November 2023. Patients were divided into two groups based on puncture technique: the Upper-PCNL group (n = 122) with upper pole calyx fornix subapical puncture and the Mid-

A unique case of extrarenal calyces and associated vascular variations in an adult female cadaver.

The following urogenital and vascular anomalies were observed in the left kidney of an 81-year-old female cadaver during routine dissection: three extrarenal calyces; an accessory renal artery originating directly from the abdominal aorta; and a circumaortic renal vein. The typical renal anatomical structures were identified, from anterior to posterior, as the renal vein, renal artery, and ureter appearing near the hilum of the left kidney. After closer examination, three extrarenal calyces were observed exiting from the hilum of the left kidney to form the pelvis, then narrowed and became the

Stone scattering during percutaneous nephrolithotomy: role of renal anatomical characteristics.

The purpose of this study was to evaluate how the upper calyx-lower calyx infundibular (ULI) angle influences intrarenal stone migration during percutaneous nephrolithotomy (PCNL) in patients with a solitary renal pelvis stone and significant hydronephrosis. 50 adult patients with a solitary renal pelvis stone larger than 20 mm were considered for PCNL with a pneumatic lithotriptor for stone fragmentation. Inclusion criteria were moderate to severe hydronephrosis and upper calyx infundibular width >10 mm, and access point was the lower calyx in all cases. The ULI angle as well as stone fragmen

[The technic and value of the 3-dimensional imaging of renal calyx staghorn calculi with spiral CT].

5 patients with staghorn calculi in the renal pelvis were examined by spiral CT. From the raw data three dimensional reconstructions of the stones were obtained. In all patients it was possible to compare the three dimensional model with the stone following performance of percutaneous litholapaxy and endoscopic removal of the fragments. In all cases the three dimensional reconstruction provided a realistic image of the stones and was of practical value for the urologist for preoperative diagnosis and intraoperative control.

Ureteroscopic fragmentation followed by extracorporeal shock wave lithotripsy: a treatment alternative for selected large or staghorn calculi.

Ureterorenoscopic fragmentation followed by extracorporeal shock wave lithotripsy (ESWL) is proposed as an addition to the alternatives of percutaneous nephrostolithotomy alone or with ESWL, or ESWL monotherapy for treatment of large (greater than 500 mm.2) or staghorn calculi. Six patients who failed percutaneous tube placement and 2 additional patients, all of whom had fragile-appearing, large volume (greater than 500 mm.2) renal calculi, were treated by primary ureteroscopy with semirigid or flexible instruments, fragmentation of the stone with the pulsed dye laser, ureteral stenting and su

[Epithelial tumors of the upper urinary tract].

The paper refers to thirteen patients with epithelial tumours of the upper urinary tract, diagnosed and treated at our unit during the last five years and emphasizes the need of early diagnosis, custumorized therapy and close follow-up. Evolution is clearly more favourable in well differentiated and low stage tumours, regardless the type of surgery performed, versus the poor results obtained in infiltrative and undifferentiated cases for which there is no effective treatment available. Results are analyzed based on a brief literature review and considering the short number of cases contributed

[Left renal pelvic tumor 32 years after calyco-ileo-vesiconeostomy].

This is a case report of left renal pelvic tumor found 32 years after left calico-ileo-vesiconeostomy. The patient has undergone right nephrectomy for absence of renal function at 26 years-old and left urinary reconstruction was performed using the intestine for pyelo-ureteral junction obstruction at 28 years old because of bilateral congenital hydronephrosis. Later he was treated for recurrent left renal stone with percutaneous nephrolithotripsy, nephrolithotomy and extracorporeal shock wave lithotripsy. When he was 56 years old, macrohematuria appeared. He received left nephrectomy under the

Neoinfundibulotomy for the management of symptomatic caliceal diverticula.

PURPOSE: Direct percutaneous access provides effective treatment for complex caliceal diverticula. Yet, access into the diverticulum alone is usually tenuous and passage of a guide wire across a stenotic infundibulum is often impossible. An alternative technique is described which creates a "neoinfundibulum" to assist in the management of symptomatic caliceal diverticula. MATERIALS AND METHODS: During a 6-year period 22 patients with symptomatic caliceal diverticula were treated via a percutaneous approach, of whom 21 had calculi within the diverticula. After accessing the diverticulum direct

Schinzel-Giedion syndrome and congenital megacalyces.

The Schinzel-Giedion syndrome is a rare autosomal recessive condition with typical facies, skeletal manifestations and congenital hydronephrosis. We report an infant with characteristic findings who had bilateral congenital megacalyces. Congenital megacalyces is believed to be a developmental abnormality, occurs in other malformation syndromes and has not previously been described in the Schinzel-Giedion syndrome.

[Differential x-ray and ultrasound diagnosis in cavernous renal tuberculosis].

It is virtually very difficult to make a differential diagnosis of cavernous nephrotuberculosis and caliceopyeloectasia (CPE), as well as a combination of caverns and CPE at primary renal ultrasound scanning. Therefore, a complex of diagnostic methods for nephrotuberculosis should include drug echography with a diuretic. Comparing X-ray findings, ultrasound and drug echography with a diuretic allows one to differentiate cavernous nephrotuberculosis from CPE and their combinations with confidence.

Safety and efficacy of a single middle calyx access (MCA) in mini-PCNL.

To compare outcomes of a single middle calyx access (MCA) with a single upper or lower calyceal access in mini-PCNL. From May 2015 through August 2018, patients' files who underwent a single renal access mini-PCNL were retrospectively reviewed. All patients underwent fluoroscopic-guided access (16-20 F) in the prone position. They were categorized into group 1 (MCA) and group 2 (either upper or lower calyceal access). Compared preoperative items included stone location, size, number and complexity (according to Guy's score). The compared outcome parameters were complication and stone-free rate

A Petal-like Calyceal Diverticulum.

Calyceal diverticula are rare outpouchings of the upper collecting system that likely have a congenital origin. They are usually single cystic lesions accompanied by stones inside them and located in the upper pole calyces. We present a case containing a petal-like cystic lesion with multiple septa in the lower pole of the left kidney, which was confirmed as calyceal diverticulum by computed tomography urography and intraoperative findings.

[The small urinary calculus: natural course and current treatment concepts].

Depending on size, duration of symptoms and their location in the collecting system urinary calculi may pass spontaneously in 16 to 93% of cases. The rate of infection and increase in size of stones in the renal pelvis or calix reaches 70% within months to years. Ureteral stones, however may lead to complications during a period of days to weeks. New minimally invasive treatment modalities have liberalized the indications for early intervention in cases of small stones.
