Background/Objectives: Accurate patient positioning is essential for diagnostic quality in conventional knee radiography. Although established anatomical landmarks are routinely used to judge projection quality, assessment remains qualitative and observer-dependent. We therefore developed and validated the Radiographic Projection Quality Score for the Knee (RPQS-Knee), a standardized scoring system for objective projection quality assessment. Methods: The RPQS-Knee was derived from established radiographic positioning criteria, anatomical reference landmarks, and predefined geometrically based…

INTRODUCTION: Knee osteoarthritis is heterogeneous in radiographic severity, pain, and multimorbidity, and racial disparities in pain and knee replacement are documented. We derived knee osteoarthritis phenotypes from electronic health record and radiograph data and assessed their associations with race and incident total knee arthroplasty.
METHODS: We conducted a retrospective cohort study in the Emory Knee Radiograph dataset of adults aged ≥40 years with knee osteoarthritis (n = 33,553). Among participants with pain scores within ±90 days of the index radiograph (n = 20,030), we applied k-m…

INTRODUCTION: The optimal alignment strategy in total knee arthroplasty remains debated, particularly in valgus deformities where lateral soft-tissue laxity and posterolateral tibial deficiency increase risk of instability. Mechanical alignment has traditionally been favored in this population, however interest in kinematic alignment (KA) has grown due to its emphasis on restoring native joint line orientation and ligament balance. Despite this, outcomes in valgus knees remain incompletely characterized. This study evaluates radiographic correction, clinical outcomes, and patient-reported meas…

PURPOSE: Unicompartmental knee arthroplasty (UKA) is an established option for treating isolated unicompartmental knee osteoarthritis (OA), but controversies still exist about patient selection, indications, perioperative management and alignment goals. This survey was designed to understand the current trends of experienced arthroplasty knee surgeons performing UKA.
METHODS: An online questionnaire was created with SurveyMonkey® to assess global tendencies in the utilization of UKA. A link to the survey was shared with all ESSKA (European Society for Sports Traumatology, Knee Surgery and Art…

In this report, we describe how to revise a failed Oxford unicompartmental knee arthroplasty to kinematically aligned total knee arthroplasty (TKA). Its benefits are the maintenance of the native joint line along with the avoidance of supplemental parts, such as metal augments and stems. This can be applied to patients whose medial tibial cortex is well preserved. The distal cutting plane and rotation alignment are decided before the removal of the femoral component. The tibial cutting plane is up to 12 mm below the lateral joint surface and the varus is up to 5° below the extramedullary rod. …

Many surgical solutions for knee flexiondeformity in the pediatric population alter the anatomical bony alignment in the distal femur. Posterior knee capsule release has been presented as an alternative surgical procedurethat maintains the anatomical shape of relevant bones while solving the issue of knee flexion contracture. The aim of this study is to assess the results of a double-incision posteriorknee capsulotomy release performed on pediatric patients with neuromuscular or congenital severe knee flexion deformity. Thirty cases (24 patients, mean age 7.4 years) of severe knee flexion cont…

BACKGROUND: Bilateral total knee arthroplasty (BTKA) under the same anesthesia (simultaneous) or staged are options for patients with end-stage arthritis of the knee that carries advantages and limitations. Not all patients are candidates for simultaneous BTKA, and therefore, surgeons prefer to stage the two TKAs. The optimal safe interval between two TKAs is not known. The present systematic review aimed to determine the optimal time interval between the two stages of BTKA.
METHODS: Pubmed and Scopus databases were searched to identify publications from January 1979 to November 2017 in Engli…

AIMS: Unicompartmental knee arthroplasty (UKA) provides improved early functional outcomes and less postoperative morbidity and pain compared with total knee arthroplasty (TKA). Opioid prescribing has increased in the last two decades, and recently states in the USA have developed online Prescription Drug Monitoring Programs to prevent overprescribing of controlled substances. This study evaluates differences in opioid requirements between patients undergoing TKA and UKA.
PATIENTS AND METHODS: We retrospectively reviewed 676 consecutive TKAs and 241 UKAs. Opioid prescriptions in morphine mill…

BACKGROUND: Numerous series have documented short and mid-term successes with cemented, metal-backed modern unicompartmental knee arthroplasty (UKA) for avascular osteonecrosis of the knee (AVN). However, data are lacking regarding long-term implant fixation and patient function. The aim of this study is therefore to evaluate the long-term clinical outcome and implant survivorship of patients who underwent UKA for medial knee osteonecrosis (ON).
METHODS: Twenty-nine consecutive UKAs performed by 2 senior surgeons (>50 UKAs a year) in 28 patients (19 women and 9 men with a mean age of 67 years…

Multiplanar or global laxity in arthritic knee is rare, most of this patients have neuromuscular disorder (post poliomyelitis, spinal dystrophy) or history of knee trauma. Ligament insufficiency and severe bone loss is significant in this patient. The estimated prevalence for the concurrence of charcot marie-tooth (CMT) with myasthenia gravis (MG) suggests an extremely rare event. We have presented a 54-year-old female patient with CMT and MG complaining of progressive pain, swelling, and crepitation of the knee joints who had been undergone total knee arthroplasty (TKA) with rotating hinge pr…

Soft-tissue balancing for total knee arthroplasty (TKA) remains subjective and highly dependent on surgical expertise. Pre-operative planning may support the clinician in taking decisions by integrating subject-specific computer models that predict functional outcome. However, validation of these models is essential before they can be applied in clinical practice. The aim of this study was to evaluate a knee modelling workflow by comparing experimental cadaveric measures to model-based kinematics and ligament length changes. Subject-specific models for three cadaveric knees were constructed fr…
