Arthroscopic Autologous Minced Cartilage Implantation for Subchondral Insufficiency Fracture of the Knee.

Subchondral insufficiency fracture of the knee, previously known as spontaneous osteonecrosis of the knee, commonly affects the medial femoral condyle and may progress to subchondral collapse and eventually osteoarthritis. Joint-preserving options for focal medial femoral condyle lesions remain limited, particularly in middle-aged patients with concomitant meniscal pathology. We describe an all-arthroscopic technique using autologous minced cartilage implantation to treat a focal chondral defect overlying a medial femoral condyle subchondral insufficiency fracture of the knee. After diagnostic

Mesh Reconstruction of Extensor Mechanism for Fracture Nonunion of the Patella in an Osteoarthritic Knee.

For extensor mechanism deficiency in the osteoarthritic knee, mesh reconstruction of the extensor mechanism is an accessible option. Studies have shown that its rates of infection and graft failure are noninferior compared to allograft reconstruction. This case report presents a 51-year-old patient who presented with chronic fracture nonunion of the inferior pole of the patella, associated with pain, extension lag, and osteoarthritis. Since the anterior knee was previously treated with partial-thickness skin graft, soft tissue reconstruction was performed prior to mesh reconstruction of the ex

[Diagnosis and treatment of periprosthetic fracture after medial unicompartmental knee arthroplasty].

Unicompartmental knee arthroplasty (UKA) is an effective treatment for end-stage anteromedial osteoarthritis of the knee. Medial tibial plateau fracture or femoral condyle fracture may occur after UKA, and its treatment is very challenging. The causes leading to this complication include: surgical technique errors, such as the weakening of posterior cortical strength of the tibial platform during operation, the reduction of bone mass due to too much tibial osteotomy, and the stress concentration in the bone bed due to bad alignment of the prosthesis, etc. Prosthesis design factors, such as pre

Risk for hip fracture before and after total knee replacement in Sweden.

UNLABELLED: We studied the risk for hip fracture before and after total knee replacement (TKR) in the entire population in Sweden. Women and men had a low risk for hip fracture before TKR but an increased risk the first year after TKR. PURPOSE: It is known that osteoarthritis is associated with high bone mass. We therefore studied the risk of hip fracture before and after total knee replacement (TKR), risk of different hip fracture types, and risk subdivided in genders and age groups. METHODS: We followed the total Swedish population born between 1902 and 1952 (n = 4,258,934) during the peri

[Antero-medial incision of knee joint for the treatment of intercondylar fracture of femur].

OBJECTIVE: To explore the clinical effect of antero-medial incision of knee joint in treating intercondylar fracture of femur. METHODS: From September 2012 to March 2015, 24 patients with intercondylar fracture of femur were selected, including 17 males and 7 females, aged from 20 to 65 years old with an average of(38.3±9.5) years old. Among them, 12 cases were caused by traffic accident, 8 cases were caused by falling injury and 4 cases were caused by falling down. All patients were closed fractures. The time from injury to hospital was from 30 min to 8 h with an average of(2.2±0.3) h. Accor

Stress Fracture of the Lateral Femoral Condyle after Total Knee Arthroplasty.

BACKGROUND: This is to assess one of the rare complications after total knee replacement and to assess risk factors of failure. METHODS: 11 patients with varus knee and an average age of 67 years underwent TKA between 2005 and 2013. All patients returned with a sudden sharp knee pain, disability to walk and significant decrease in ROM about 4 to 8 weeks after surgery. Radiographic examination revealed a lateral femoral condylar stress fracture. RESULTS: After analyzing the images, we found common characteristics among all patients, which might be attributable to the later fracture including

Subchondral insufficiency fractures and spontaneous osteonecrosis of the knee may not be related to osteoporosis.

UNLABELLED: The term insufficiency fracture implies inadequate bone and is applied to some subchondral knee magnetic resonance images. We reviewed bone mineral density, body mass index, meniscal extrusion, comorbidities, and demographics in 32 knee insufficiency fracture patients. Only five were osteoporotic. Meniscal extrusion was predominant. PURPOSE: The literature supports systemic osteoporosis as a risk fracture for spontaneous osteonecrosis of the knee (SONK). SONK is also called a subchondral insufficiency fracture. Recognizing that insufficiency fracture and SONK are related, we desig

Incidence of venous thromboembolism in fractures around and below the knee with physical prophylaxis.

Fractures occurring at the distal part of the lower extremities are recognized to have a relatively lower risk of venous thromboembolism (VTE); however, few detailed reports exist on the subject. The purpose of this study was to investigate the incidence of VTE in fractures around and below the knee. Overall, 109 consecutive patients with fractures around and below the knee who were surgically treated at the authors' hospital were analyzed retrospectively. Physical prophylaxis was performed in all patients. Until April 2009, VTE screening was performed by contrast-enhanced computed tomography

[Conservative surgery as an alternative treatment of a knee osteosarcoma in the presence of a pathological fracture].

INTRODUCTION: classical osteosarcoma is defined as a very malignant spindle cell sarcoma, characterized by the production of osteoid matrix, it is the most common primary malignant bone tumor. It is most common among males in their twenties. Factors involved include those related with the skeletal growth and development, pathological fractures, and very young patients who are still growing. Conservative surgery is inappropriate; however, preoperative chemotherapy and a good choice of conservative treatment are an alternative. MATERIAL AND METHODS: a 15-year-old male patient presented at the o

Repair of supracondylar femur fracture and unilateral knee replacement at the same surgery.

In patients who are candidates for a total knee arthroplasty and suffer a periarticular fracture of the femur, the arthroplasty may be performed after bony union of the fracture or immediately, in conjunction with the fracture repair. Herein we present the case of a sixty-year-old female with rheumatoid arthritis and a supracondylar fracture of the right femur in whom total knee arthroplasty and retrograde nail insertion were addressed at one surgery; the outcome was favorable. The transverse extraarticular fracture pattern in this patient was advantageous for simultaneous procedures; had the

The effect of simulated fracture-angulations of the tibia on cartilage pressures in the knee joint.

The effects of angular deformities of the tibia on the contact areas and pressures on cartilage in the knee were measured with use of pressure-sensitive film. Six cadaver knees were mounted in a test-frame, such that the offset and tilt of the knee relative to the load-axis simulated that due to angular deformities of the tibia of 5, 10, 15, or 20 degrees of varus or valgus at the levels of the proximal, middle, and distal thirds. Angulation had little effect on the contact area. The change in pressure was least for fractures of the distal third and greatest for fractures of the proximal third

Fracture of a fenestrated metal backing of a tibial knee component. A case report.

The case of a metal-backed tibial component that failed by metal fracture is reported. Fracture occurred through fenestrations in the metal plate surrounding the central metal stem. The fracture followed loss of bony support beneath the medial tibial plateau, which had allowed varus deformity of the replaced joint. A high tibial osteotomy procedure had preceded replacement of the joint.

Fracture of the knee endoprosthesis. Report of three cases of tibial component failure.

Three cases of total knee replacement, in which the polyethylene tibial component was fractured, are presented. Two were Attenborough prostheses and the third was a Townley prosthesis. In all cases there were radiographic signs of loosening before the fracture occurred. They were all successfully reoperated with an arthrodesis, using the long Küntscher nail technique.

Treatment of osteochondritis dissecans and cartilaginous fractures of the knee by osteo-cartilaginous autografts.

Osteochondritis dissecans and cartilaginous fractures of the knee joint are lesions in which, on removal of the necrotic areas of cartilage, extensive loss of substance may result, such as to limit joint function and be a predisposing cause of arthrosis. In such cases it is suggested that, in addition to curettage of the necrotic focus, an autograft of cartilage and spongy bone should be used, obtained from another part of the condyle of the same knee which is not weight bearing. The surgical technique used in fifteen such cases is described, and in particular the surgical instruments which wh

Osteochondral fractures of the lateral femoral condyle: a result of indirect violence to the knee.

Twenty patients with an average age of eighteen and a half years sustained osteochondral fractures of the lateral femoral condyle as the result of a sudden twist and valgus strain to the straight or almost straight knee. All the patients felt sudden pain at the moment of injury, all had a haemarthrosis, and yet the fracture escaped early diagnosis in one-third of the cases. Internal fixation of the fragment with early mobilisation is recommended if the diagnosis is made within two weeks of injury, and excision of the fragment if it is only later identified.

Tibial shaft fracture with knee and proximal tibiofibular dislocation: Case report.

Tibial shaft fractures are the most common type of long bone fracture, frequently associated with high-energy traumas such as traffic accidents, which carry a high mortality rate in Colombia. The combination of an open tibial shaft fracture with knee and proximal tibiofibular joint dislocation is extremely rare. The presented case involves these injuries, which were managed surgically in a single-stage with a follow-up of six months and good functional outcomes. Intramedullary nailing of the tibia was performed using an extra-articular, extra-synovial, semi-extended technique, which optimized

Periprosthetic tibial fracture as a complication of unicompartmental knee arthroplasty: Current insights.

Periprosthetic fracture following knee arthroplasty is a rare but devastating complication associated with significant morbidity. With unicompartmental knee arthroplasty being performed far less frequently than total knee arthroplasty, periprosthetic fracture following unicompartmental knee arthroplasty presents a particular challenge to orthopaedic surgeons, due to clinical unfamiliarity and sparsity of literature. An up-to-date review of the epidemiology, risk factors, and management strategies for PPF after UKA is presented.
