Posterior glottic stenosis with a calcified interarytenoid scar band: CT and laryngoscopic correlation.

A 52-year-old man with burn injuries and prolonged intubation developed PGS with hoarseness, dyspnea, and bilateral vocal cord immobility. On CT, a calcified interarytenoid scar band was identified, corresponding to an interarytenoid scar on laryngoscopy. Endoscopic laser lysis of the calcified scar band relieved the symptoms. We present laryngoscopic and CT findings of PGS with interarytenoid calcification along with the postlysis findings. The classification, clinical findings, imaging, and management of PGS are reviewed.

The efficacy of the Storz Miller 1 video laryngoscope in a simulated infant difficult intubation.

BACKGROUND: Several studies have shown video laryngoscopy to be a useful technique in the management of patients in whom glottic exposure by direct laryngoscopy is difficult. We conducted this study as a preliminary investigation comparing the Storz DCI Miller 1 video laryngoscope (VL, Karl Storz GmbH, Tuttlingen, Germany) and direct laryngoscopy with a Miller 1 laryngoscope (DL) in an infant manikin model simulating difficult direct laryngoscopy. We hypothesized that compared with DL, VL would provide a better glottic view but would be associated with a longer time to intubation because of th

Pediatric airway management: comparing the Berci-Kaplan Video Laryngoscope with direct laryngoscopy.

OBJECTIVES: To assess the utility of the Berci-Kaplan Video Laryngoscope (VL) in pediatric anesthesia. BACKGROUND: The VL is designed to improve visualization of the glottis during tracheal intubation of normal and difficult airways in adults. This study was designed to assess the visual quality and the ease of tracheal intubation in children using the VL when compared with direct laryngoscopy (DL). METHODS: Sixty children, aged 2-16 years requiring tracheal intubation, were enrolled in this prospective, randomized clinical trial. Following induction of anesthesia and muscle paralysis, the f

A Macintosh laryngoscope blade for videolaryngoscopy reduces stylet use in patients with normal airways.

BACKGROUND: Although most tracheal intubations with direct laryngoscopy are not performed with a styletted endotracheal tube, it is recommended that a stylet can be used with indirect videolaryngoscopy. Recently, there were several reports of complications associated with styletted endotracheal tubes and videolaryngoscopy. In this study, we compared three videolaryngoscopes (VLSs) in patients undergoing tracheal intubation for elective surgery: the GlideScope Ranger (GlideScope, Bothell, WA), the V-MAC Storz Berci DCI (Karl Storz, Tuttlingen, Germany), and the McGrath (McGrath series 5, Aircra…

Successful intubation using a simple fiberoptic assisted laryngoscope for Treacher Collins syndrome.

A fiberoptic-assisted laryngoscope (FLS) (Acoma Medical Industry Co. Ltd., Tokyo, Japan) is a modified Macintosh laryngoscope with a tubular holder into which a fiberoptic bronchoscope can be inserted. We present three cases of Treacher Collins syndrome whose tracheas were successfully intubated with the aid of the FLS. These cases suggest that the FLS may be a useful alternative in the case of difficult pediatric intubation.

A simple, inexpensive, and effective light- carrying laryngoscopic blade for orotracheal intubation of rats.

The research paradigm of using large laboratory animals, in which oroendotracheal intubations are relatively easy, is shifting toward the use of small animals, such as rodents, in which oropharyngeal access is limited, the arytenoid cartilage cycles are faster, and the glottis is much smaller. The considerable growth recently seen in preclinical imaging studies is accompanied by an increased number of rats and mice requiring in vivo intubation for airway management. Tracheal access is important for ventilation, administration of inhaled anesthetics, instillation of drugs or imaging agents, and

An evaluation of poor LMA CTrach views with a fibreoptic laryngoscope and the effectiveness of corrective measures.

BACKGROUND: The LMA CTrach++ system has integrated fibreoptics to enable viewing of the glottis to aid tracheal intubation via a laryngeal mask conduit. Earlier evaluations of this system had shown high success rates of insertion, ventilation and tracheal intubation, but frequent difficulty or failure to view the glottis. METHOD: We evaluated CTrach views in 100 patients. In 69 patients with partial or no views of the glottis, we evaluated the causes with the aid of an Olympus LF-DP fibreoptic laryngoscope passed through the CTrach. In those with poor views, we tested the effectiveness of sim

[Preliminary report: observation by video laryngoscope of glottal narrowing due to cricoid pressure].

BACKGROUND: When the applied cricoid pressure is too strong, or the place or direction of the pressure application is not appropriate, glottal closure may occur, but its details are unclear. METHODS: We evaluated possible changes in the size of the rima glottides due to backward pressure on the cricoid cartilage or backward pressure or backward, upward, and rightward pressure (BURP) on the thyroid cartilage using a video laryngoscope (Fine View' Laryngoscope, Tray Medical, Tokyo) in 6 adult males and 6 females with Cormack and Lehane grade 1. RESULTS: The right-to-left distance of the rima g

A videographic analysis of laryngeal exposure comparing the articulating laryngoscope and external laryngeal manipulation.

Activation of the articulating laryngoscope and external laryngeal manipulation (ELM) improve laryngeal exposure during direct laryngoscopy. We used a head-mounted direct laryngoscopy imaging system and a previously validated scoring system for assessing laryngeal view (the percentage of glottic opening or POGO score) on 33 adult patients undergoing laryngoscopy. On each patient, we videotaped the initial laryngeal exposure (blade not activated), the view with activation of the blade, and the view with operator-directed external laryngeal manipulation. The video recordings were reviewed and th

Analysis of the forces and position required for direct laryngoscopic exposure of the anterior vocal folds.

The sniffing position is traditionally considered optimal for direct laryngoscopic examination of the vocal folds. This study examined head and neck positions associated with ideal exposure of the anterior glottal commissure with a variety of laryngoscopes. A prospective investigation was done in 20 patients by comparing the force required to expose the anterior vocal folds by utilizing 3 head and neck positions with 3 different-sized tubular laryngoscopes. The completeness of anterior glottal exposure was rated and the force required to achieve this exposure was measured with a strain gauge. …

The IPA categories "pharyngeal" and "epiglottal": laryngoscopic observations of pharyngeal articulations and larynx height.

The phonetic problem is to describe accurately the articulatory mechanism, or mechanisms, responsible for the production of a series of sounds that are presently labelled on the chart of the International Phonetic Association as either pharyngeal or epiglottal. The sounds on which these categories are based are found in the Semitic languages, in the languages of the Caucasus, and in the languages of the Pacific Northwest coast of North America. In order to reconcile a variety of descriptive terms with a logical phonetic taxonomy, auditorily distinguishable parameters are deduced from a natural

Forces acting on the maxillary incisor teeth during laryngoscopy using the Macintosh laryngoscope.

We determined the forces on the maxillary incisors during routine laryngoscopy in 65 adult patients. The forces were measured by a strain gauge based sensor positioned between the handle and the blade of the laryngoscope. The mean maximal force acting on the maxillary incisors was 49 N. In patients without maxillary incisors, the force acting on the gums was significantly lower at 21 N (p < 0.001). These results suggest that, despite traditional advice to the contrary, a levering movement of the laryngoscope, using the maxillary incisors (or gums) as a fulcrum, is common practice. Biomechanica

Comparison of McGrath Series 3 and Macintosh Laryngoscopes for Tracheal Intubation in Patients With Normal Airway by Inexperienced Anesthetists: A Randomized Study.

Difficult and failed intubations account for the major causes of morbidity and mortality in current anesthetic practice. Several devices including McGrath Series 3 videolaryngoscope are available which may facilitate tracheal intubation by improving view of the larynx compared with Macintosh blade laryngoscopy. But no studies demonstrate whether McGrath Series 3 performs better than Macintosh laryngoscope in normal airway intubations by inexperienced anesthetists so far. We therefore designed this randomized controlled study to compare McGrath with Macintosh in routine tracheal intubation perf

Multiview Laryngoscopic Image Fusion for Patient-Level Stratification of Vocal Fold Lesions.

Malignant or high-grade vocal fold disease is usually judged across repeated laryngoscopic views at different observation distances and fields of view, whereas most artificial intelligence systems remain frame-based. We evaluated whether patient-level fusion of multiview laryngoscopic image sets could improve benign versus malignant/high-risk stratification of vocal fold lesions. In this retrospective multicenter study of 628 patients, internal data from the Cancer Hospital and Institute of the Chinese Academy of Medical Sciences collected between 2008 and 2025 were split into training (n = 38

Autoimmune Disorders Affecting the Larynx: Review of Laryngoscopic Findings and Approach to Multidisciplinary Management.

The prevalence of autoimmune diseases has been increasing globally. Many disorders have manifestations within the larynx and patients may present initially to the otolaryngologist with breathing, voice, or swallow concerns. This review focuses on the laryngotracheal manifestations of various autoimmune diseases and associated laryngoscopic abnormalities that commonly occur with autoimmune disease, including infraglottic stenosis, bamboo nodules, cricoarytenoid joint ankylosis, supraglottic stenosis, and multilevel airway stenosis. Commonly implicated autoimmune disorders including granulomatos
